loperamide hydrochloride 2 mg
Auxiliary substances: polymethylsiloxane, potato starch, lactose, sucrose, microcrystalline cellulose, silicon dioxide colloid (aerosil), aspartame, povidone (Kollidon 90), xylitab 300, lactitol, kollidone SR, menthol, magnesium stearate, anise oil.

pharmachologic effect

Antidiarrhoeal drug. Loperamide, by binding to opioid receptors of the intestinal wall (modulation of cholinergic and adrenergic neurons via guanine nucleotides), reduces tone and motility of smooth intestinal muscles (by inhibiting the release of acetylcholine and prostaglandins). Slows down the peristalsis and increases the transit time of intestinal contents. Increases the tone of the anal sphincter, promotes retention of fecal masses and reduced urge to defecate.
The action comes quickly and lasts for 4-6 hours.

Pharmacokinetics

Loperamide hydrochloride is almost completely metabolized by the liver by conjugation. T1 / 2 is 9-14 hours. It is excreted mainly with bile and urine in the form of conjugated metabolites.

Indications for Diara

Symptomatic treatment of acute and chronic diarrhea:
- Allergic, emotional, medicinal, radiation genesis;
- with a change in diet and quality of food, in violation of metabolism and absorption;
- as an aid for diarrhea of ​​infectious genesis.
Regulation of stool in patients with ileostomy.

Contraindications

- diverticulosis;

Dosage

Diara should be taken orally, washed down with water.
Adults with acute and chronic diarrhea are prescribed in an initial dose of 4 mg (2 tablets or 2 capsules), then 2 mg (1 tablet or 1 capsule) after each act of defecation in the case of a loose stool. The maximum daily dose is 16 mg (8 tablets or 8 caps.).
Children over the age of 6 years are prescribed 2 mg (1 tablet or 1 capsule) after each act of defecation in the case of a loose stool. The maximum daily dose is 6 mg (3 tablets or 3 capsules).
With the normalization of the stool or the absence of stool for more than 12 hours, treatment with Diara should be discontinued.

Side effects of Diara

On the part of the digestive system: gastralgia, dry mouth, intestinal colic, nausea, vomiting, constipation; very rarely - intestinal obstruction.
From the side of the central nervous system: drowsiness, dizziness.
Allergic reactions: skin rash.

Overdose

Symptoms: depression of the central nervous system (stupor, impaired coordination of movements, drowsiness, miosis, muscle hypertension, respiratory depression), intestinal obstruction.
Treatment: antidote - naloxone. Given that the duration of action of loperamide is greater than that of naloxone, it is possible to re-administer the latter.
Symptomatic treatment: activated charcoal, gastric lavage, IVL. Medical observation is necessary for at least 48 hours.
