Form of issue

Galvus Met is available in the form of film-coated tablets in doses of 50 mg + 500 mg, 50 mg + 850 mg or 50 mg + 1000 mg. Tablets are packaged in blisters of 6 or 10 pieces, 1, 3, 5, 6, 12, 18, 36 blisters in the package.

This drug has a hypoglycemic pharmacological effect.

Pharmacodynamics and pharmacokinetics

Galvus Met includes 2 hypoglycemic agents with different mechanisms of action. In this case, vildagliptin is an inhibitor of dipeptidyl peptidase-4 (DPP-4), and metformin hydrochloride belongs to biguanides. In combination, these components contribute to an effective control during the day of the glucose concentration in the blood of type 2 diabetes mellitus.
The use of this drug leads to a statistically significant persistent decrease in blood glucose concentration. At the same time, only isolated cases of hypoglycemia develop.
It is established that eating does not affect the speed and degree of absorption of the drug, and the concentration of active substances is somewhat reduced, but, in general, depends on the dose taken.
Absorption Galvus Met is rapid, the bioavailability of the components is about 85%. When the medicine is taken on an empty stomach, the presence of its constituents in the blood plasma is detected after 1-1.5 hours. In the body, the drug is converted into metabolites, which are excreted by the kidneys and a small part with feces.

Indications for use of the medicine

The main indication for taking Galvus Met is therapy for type 2 diabetes mellitus if:
    monotherapy with vildagliptin or metformin is not effective;
    it is not possible to adequately control glycemia in other treatment options and so on.

Contraindications to use

Galvus Met is not assigned when:
    high sensitivity to its components;
    kidney failure and other disorders in the kidneys;
    acute forms of diseases that can cause the development of violations of kidney functions - dehydration, fever, infections, hypoxia and so on;
    violations of liver function;
    diabetes mellitus type 1;
    chronic alcoholism, acute alcohol poisoning;
    lactation, pregnancy;
    adherence to a hypocaloric diet;
    children under the age of 18 years.
With care, tablets are prescribed to patients from 60 years old who work in heavy physical production, since lactoacidosis is possible.

Side effects

In the treatment of Galvus Met, development of undesirable phenomena is possible, which are manifested: severe headache, dizziness, tremor. Also, there may be abnormalities in the activity of the digestive tract: nausea, diarrhea, gastroesophageal reflux, flatulence. Although taking this medication minimizes the appearance of hypoglycemia, one should not exclude the development of her symptoms.

Galvus Met - instructions for use (Method and dosage)

This drug is intended for ingestion. Dosage Galvus Met is selected individually and depends on the effectiveness of the treatment and the tolerability of the components. During therapy, it is recommended to strictly adhere to the prescribed daily dose of vildagliptin, which should not exceed 100 mg.
At the beginning of treatment, the dose is selected taking into account the duration of the course of the disease, the level of glycemia, the state of the patient and the therapeutic regimen used before. To reduce the severity of side effects associated with the work of the digestive system, the medicine is taken with food.
Usually, treatment starts with a single tablet of 50 mg + 500 mg, which is taken 2 times a day. Evaluating the therapeutic effect, you can gradually increase the dosage.
If monotherapy with metformin was ineffective, then taking into account the previously taken metformin, one tablet can be prescribed the most suitable dosage for taking 2 times a day.
When appointing an initial dose of the drug, patients who received combined therapy with individual tablets of vildagliptin and metformin, the closest dosage is selected.
In the combination therapy Galvus Met and derivatives of sulfonylureas or insulin, the dose is calculated from the amount of vildagliptin 50 mg × 2 times, that is, 100 mg per day, and metformin in a dosage approximate to that taken in monotherapy.

Overdose

As is known, vildagliptin in the composition of this drug is well tolerated when taken in a daily dosage of up to 200 mg. In other cases, there may be muscle pain, swelling and fever. Usually the symptoms of overdose can be eliminated by canceling the drug.
In cases of metformin overdose, the symptoms of which can develop when taking the drug from 50 g, hypoglycemia, lactic acidosis may occur, accompanied by nausea, vomiting, diarrhea, lower body temperature, abdominal and muscle pains, increased breathing, dizziness. Heavy forms lead to impaired consciousness and development of coma.
At the same time, symptomatic treatment is performed, hemodialysis procedure is performed and so on.
It should be noted that for patients who receive insulin, Galvus Met's appointment is not a substitute for insulin.

Interaction

Vildagliptin does not belong to the substrates of cytochrome P450 enzymes, it is not an inhibitor and inducer of these enzymes, therefore it practically does not interact with substrates, inducers or P450 inhibitors. In this case, its simultaneous use with substrates of certain enzymes does not affect the metabolic rate of these components.
Also simultaneous use of vildagliptin and other drugs prescribed for type 2 diabetes, for example: Glibenclamide, pioglitazone, metformin and drugs with a narrow therapeutic range - amlodipine, digoxin, ramipril, simvastatin, valsartan, warfarin does not cause clinically significant interaction.
The combination of furosemide and metformin exerts a mutual influence on the concentration of these substances in the body. Nifedipine increases the absorption and excretion of metformin in the urine.
Organic cations such as Amyloride, Digoxin, Prokainamide, Quinidine, Morphine, Quinine, Ranitidine, trimethoprim, Vancomycin, Triamteren and others, when interacting with metformin because of competing for the general transport of the renal tubules, can increase its concentration in the blood plasma. Therefore, the use of Galvus Met in such combinations requires caution.
The combination of the drug with thiazides, other diuretics, phenothiazines, thyroid hormone drugs, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, calcium antagonists and isoniazid, can provoke hyperglycemia and lower the effectiveness of hypoglycemic agents.
Therefore, when simultaneously prescribe or cancel such drugs, careful monitoring of the effectiveness of metformin - its hypoglycemic action and, if necessary, dosage adjustment. From the combination with danazol it is recommended to abstain in order to avoid the manifestation of its hyperglycemic action.
Taking high doses of chlorpromazine can increase glycemia, as it lowers the release of insulin. Treatment with neuroleptics also requires correction of dosing and control of glucose concentration.
Combination therapy with iodine-containing radiopaque means, for example, radiological research with their use, often causes the development of lactic acidosis in diabetes mellitus and functional renal failure.
To increase glycemia, injectable β2-sympathomimetics are possible as a result of stimulation of β2-receptors. For this reason it is necessary to control the glycemia, possibly the appointment of insulin.
The simultaneous administration of Metformin and derivatives of sulfonylurea, insulin, acarbose, salicylates can enhance hypoglycemic action.
