Form of release, composition and packaging
Eye drops are transparent, colorless.
1 ml tetrisoline hydrochloride 500 ?g
Excipients: boric acid - 12.3 mg, sodium tetraborate (borax) - 0.25 mg, sodium chloride - 2.23 mg, water d / and - up to 1 ml.
0.5 ml - ampoules transparent for single use of low density polyethylene, welded in the form of a strip (5) - strips (2) - bags of paper / PE / aluminum / film (1) - cardboard packs.
pharmachologic effect
Alpha-adrenomimetic. Tetrisolin stimulates ?-adrenoreceptors of the sympathetic nervous system, does not or has little effect on ?-adrenoreceptors. Being an adrenomimetic amine, tetriozolin has a vasoconstrictive effect and reduces edema of tissues.
The effect of the drug begins 60 seconds after instillation and lasts 4-8 hours.
Pharmacokinetics
With topical application, systemic absorption is possible in patients with mucosal lesions and epithelium.
Detailed pharmacokinetic studies with the use of eye drops were not carried out.
Indications
- for removing the edema and congestion hyperemia caused by chemical and physical factors (smoke, wind, dust, chlorinated water, light, cosmetic products, contact lenses), as well as arising from allergic reactions such as pollinosis.
Contraindications
- an angle-closure glaucoma;
- Children under 2 years;
- Hypersensitivity to the components of the drug.
Caution should be used in patients with severe cardiovascular diseases (eg, IHD, hypertension), pheochromocytoma, prostatic hyperplasia, hyperthyroidism, diabetes mellitus, porphyria, dry rhinitis, dry keratoconjunctivitis, glaucoma and in patients receiving MAO inhibitors or other means that can raise blood pressure.
The use of the drug in closed-angle glaucoma is contraindicated. For other types of glaucoma, the drug is used with extreme caution and under the supervision of a doctor.
Dosage
Locally. Adults and children over 2 years of 1 drop in the affected eye 2-3 times / day.
The use of the drug for more than 4 days should only be carried out under the supervision of a doctor.
Instructions for the use of single-use ampoules
Each ampule of the drug Vizin Classical is intended for single use only. The drug should be used immediately after opening the ampoule. The amount of the drug in one ampoule is sufficient for a single injection into both eyes.
1. It is necessary to break off 1 ampoule from the strip, and put the remaining ampoules back into the foil bag.
2. Then open the ampoule by unscrewing the top, unfilled portion of the ampoule.
3. Inject the drug into the conjunctival sac, lightly pressing on the filled part of the ampoule.
Side effects
Very often (> 1/10), often (> 1/100, 1/10), infrequently (> 1/1000, <1/100), rarely (> 1/10 000, <1/1000) and very rarely (<1/10 000).
Infrequent: reactive hyperemia, burning sensation, dry mucous membrane, systemic disorders (eg, palpitations, headache, tremor, weakness, sweating, and increased blood pressure). Perhaps the appearance of blurred vision, irritation of the conjunctiva or rarely - mydriasis. In addition, systemic adrenomimetic effects are rarely observed.
In some cases, after long-term use of tetrisoline, keratinization of the conjunctival epithelium (xerosis) with lacrimal occlusion and epiphory has been reported.
Overdose
Symptoms
Excessive systemic absorption of alpha-adrenomimetic imidazole derivatives can lead to CNS depression, which is manifested by drowsiness, decreased body temperature, bradycardia, shock-like hypotension, apnea, and coma.
Symptoms of overdose are mydriasis, nausea, cyanosis, fever, convulsions, tachycardia, arrhythmia, cardiac arrest, increased blood pressure, pulmonary edema, respiratory disorders, mental disorders.
The risk of overdose symptoms associated with drug absorption is especially high in newborns and young children with swallowing of drops, which can lead to abnormalities in the central nervous system, as well as respiratory depression and collapse.
The toxic dose of tetrisoline is 0.01 mg / kg body weight.
Treatment
If any symptoms of an overdose occur, you should immediately consult a doctor.
When an overdose is prescribed activated charcoal, gastric lavage, inhalation with oxygen, antipyretic and anticonvulsants. To reduce blood pressure apply phentolamine 5 mg on physiological solution slowly iv or 100 mg orally. Patients with low blood pressure vasopressor are contraindicated. In the case of symptoms arising from blockade of m-cholinergic receptors, the antidote physostigmine is used.
Drug Interactions
The drug Vizin Classical should not be used simultaneously with MAO inhibitors such as tranylcypromine, tricyclic antidepressants, and also drugs that increase blood pressure. Combination therapy with these groups of drugs can lead to increased vasoconstrictive effect and increased blood pressure.
special instructions
Before instillation of the drug, it is necessary to remove contact lenses and install them 15 minutes after instillation. It is necessary to avoid direct contact of the drug with soft contact lenses due to a possible violation of their transparency.
Possible development of reactive hyperemia of the conjunctiva and nasal mucosa (medicamentous rhinitis) due to improper application of the drug.
The patient should be informed that if there are intense pains in the eyes, severe acute or unilateral redness of the eyes, headache, visual impairment, appearance of spots in front of eyes or double vision, you should immediately consult a doctor.
If irritation or congestion is associated with serious eye diseases such as infection, foreign body or chemical injury of the cornea, an ophthalmologist should be consulted.
Avoid prolonged use and overdose of the drug, especially in children.
The patient should be informed that if the drug has become unusable or has expired, then do not throw it into sewage or into the street. It is necessary to place the drug in a bag and put it in a garbage container. These measures will help protect the environment.
Impact on the ability to drive vehicles and manage mechanisms
In rare cases, after the application of eye drops, blurred vision occurs, which may affect the ability to drive or operate machinery.
Pregnancy and lactemia
Data on the penetration of the drug through the placenta and into breast milk are absent. Given the potential for the development of systemic side effects, the drug can be used during pregnancy and during breastfeeding only if, in the opinion of the doctor, the expected benefit to the mother exceeds the potential risk to the fetus and the baby.
Application in childhood
Contraindicated in children under 2 years.
