Contraindications

- Hypersensitivity to the active substances, other sulfonamides, dihydropyridine derivatives or any of the excipients;
- severe renal failure (creatinine clearance <30 ml / min);
- severe liver failure or hepatic encephalopathy;
- hypokalemia;

- severe hypotension;

How to apply: dosage and treatment

Tablets arifam taken orally, 1 tablet 1 time per day, preferably in the morning. The tablet must be swallowed without chewing and drinking water.

Arifam is a combination drug for treating hypertension: the mechanism of antihypertensive action of amlodipine is due to a direct relaxing effect on vascular smooth muscle, while Indapamide increases the excretion of sodium and chlorides in the urine and, to a lesser extent, the excretion of potassium and magnesium, thereby increasing diuresis and exerting an antihypertensive effect .

Interaction

Amlodipine

Grapefruit juice can reduce the concentration of amlodipine in the blood plasma, but this decrease is so small that it does not significantly change the effect of amlodipine.
Procainamide, quinidine, and other drugs that prolong the QT interval increase the negative inotropic effect and may increase the risk of significant prolongation of the QT interval.
Inhibitors of microsomal oxidation increase the concentration of amlodipine in the blood plasma, increasing the risk of side effects, and inducers of microsomal liver enzymes reduce.
Non-steroidal anti-inflammatory drugs, especially indomethacin (sodium retention and blockade of prostaglandin synthesis by the kidneys), alpha-adrenostimulants, estrogens (sodium retention), and sympathomimetics weaken the antihypertensive effect.
Thiazide and "loopback" diuretics, beta-blockers, verapamil, ACE inhibitors and nitrates enhance the antianginal and hypotensive effects.
Amiodarone, quinidine, alpha1-blockers, antipsychotic drugs (neuroleptics) and blockers of "slow" calcium channels can enhance the hypotensive effect.
When used together with lithium preparations, it is possible to increase the manifestations of their neurotoxicity (nausea, vomiting, diarrhea, ataxia, tremor, tinnitus).
Calcium preparations can reduce the effect of blockers of “slow” calcium channels.

Indapamide

Increases the concentration of lithium ions in the plasma (reduction of excretion with urine), lithium has a nephrotoxic effect.
Increases the risk of developing renal dysfunction when using iodine-containing contrast drugs in high doses (dehydration). Before using iodine-containing contrast drugs, patients need to restore fluid loss.
Reduces the effect of indirect anticoagulants (coumarin or indanedione derivatives) due to increased concentration of coagulation factors as a result of a decrease in BCC and an increase in their production by the liver (dose adjustment may be required).
Strengthens the blockade of neuromuscular transmission, which develops under the influence of non-depolarizing muscle relaxants.

Storage conditions

Arifam should be stored out of the reach of children at a temperature not higher than 30 ° C.
