Composition

The tablet contains the active substance felodipine at a dose of 2.5, 5.0 and 10.0 mg.
Additional components: propyl gallate, hypromellose, lactose monohydrate, magnesium stearate, povidone, colloidal silicon dioxide.
Composition of the film coating: propylene glycol, yellow and red dyes, iron oxide, titanium dioxide.

Form of issue

Felodip is available in tablet form. The shell provides a longer, prolonged action of the drug.
Each blister has 10 tablets. The cardboard pack contains instructions from the manufacturer and 3 (or 10) blisters made of aluminum.

pharmachologic effect

A hypotensive agent. By the mechanism of action is a blocker of slow calcium channels. The active component is a dihydropyridine derivative. The drug has not only antihypertensive, but also anti-anginal effect. Reduction of blood pressure level is achieved due to a decrease in OPSS.
The anti-ischemic effect is dose-dependent. Felodipine is able to protect against complications of reperfusion, reduce the size of the myocardium.
Concerning the conductive system, a minimal effect is recorded, the active substance does not have a negative inotropic effect.

Pharmacodynamics and pharmacokinetics

The active substance is almost completely absorbed, absorbed from the digestive system. At the "first passage" through the hepatic system, intensive metabolism is noted. The level of binding with proteins is maximal - 99%.
The active ingredient is released during lactation with breast milk. The active component is able to penetrate the placental barrier, the BBB. The known metabolites formed after metabolism in the hepatic system do not possess pharmacological activity. 0.5% is removed in its original form, 70% - through the renal system in the form of metabolites, the rest - through the intestine. In the alpha phase, the elimination half-life is 4 hours, in the beta phase, 24 hours.
It is noted that in elderly patients and in persons with liver diseases, the concentration of active substance felodipine in the blood is much higher than in young patients.
Long-term therapy does not lead to cumulation of the active substance. The only exception is patients with chronic renal insufficiency, in which the accumulation of inactive metabolites is registered (dose adjustment in an individual order is required).

Indications for use

    angina pectoris;
    hypertonic disease;
    angina of prinzmetal.

Contraindications

    aortic stenosis (clinically significant course);
    myocardial infarction (acute form, up to 1 month);
    heart failure (decompensation);
    severe arterial hypotension;
    unstable angina;
    breast-feeding;
    individual hypersensitivity;
    cardiogenic shock;
    age limit - up to 18 years (lack of data on safety, effectiveness);
    pregnancy.
With the pathology of the hepatic and renal systems, Felodip is used with caution under the control of biochemical blood parameters.

Side effects

Treatment with MKC blockers may be accompanied by the following negative symptoms:
    fast fatiguability;
    severe migraine headaches;
    redness of facial skin;
    cardiopalmus;
    dizziness.
The above described manifestations pass independently after 2-3 weeks of treatment and can again manifest themselves with an increase in the dose of felodipine. As a result of precapillary vasodilation, peripheral edema may appear.
With periodontitis, a weakly expressed puffiness of the gums is recorded. With careful hygiene of the oral cavity, the negative symptomatology disappears.
Common side effects:
    myalgia;
    photosensitization;
    arthralgia;
    itching;
    paresthesia;
    hives;
    gingival hyperplasia;
    increase of ALT, AST;
    peripheral edema;
    angioedema.


Instructions for use Felodip (Method and dosage)

The recommended time of reception is in the morning before meals, or after breakfast. It is unacceptable to separate the tablets themselves, covered with a film protective coating.

Hypotensive therapy begins with a minimum dose of 5 mg. If the dose is ineffective, raise to 10 mg. The medication is taken once a day.

Treatment of Stable Angina

Selection of the dose of Felodip is carried out individually, starting at 5 mg / day. Maximum can take 20 mg per day.
With simultaneous reception of beta-blockers, diuretics, ACE inhibitors, an increase in the severity of the hypotensive effect is recorded, which may require correction of the dosing regimen to prevent the development of arterial hypotension.
With pathology of the liver system, individual dose selection is carried out.

Overdose

Recorded bradycardia, arterial hypotension. First aid: put the patient on a horizontal surface, lift the legs. At registration of a bradycardia intravenously enter Atropine 0.5-1.0 mg.
An increase in the volume of plasma is achieved by the infusion of a solution of dextran, Na chloride, glucose. In the absence of the expected effect, alpha-adrenostimulators are prescribed.

Interaction

Drugs that slow the metabolism of felodipine and increase its concentration in the blood:
    Itraconazole;
    Cimetidine;
    Ketoconazole;
    Erythromycin.
It is noted that with simultaneous reception with Digoxin, an increase in the concentration of the latter is recorded, but the correction of the dosing regimen of Felodip is not required.
The concentration of the active substance is reduced with the following medicines:
    Rifampicin;
    Carbamazepine;
    Phenytoin;
    barbiturates.
Preparations of the NSAID group do not affect the severity of the hypotensive effect of the drug. A high index of binding of the active substance to plasma proteins does not affect the process of binding fractions of other drugs, including Warfarin.
The hypotensive effect is enhanced by simultaneous reception:
    diuretic drugs;
    beta-blockers;
    tricyclic antidepressants;
    Verapamil.
Grapefruit juice contains flavonoids that are capable of increasing the bioavailability of the active substance of felodipine, therefore it is unacceptable to take tablets with grapefruit juice.
