Composition

The tablet contains 10 or 20 mg of lercanidipine hydrochloride.
Additional components: povidone K30, lactose monohydrate, sodium carboxymethyl starch, cellulose, magnesium stearate. Each tablet is coated with a special coating, which includes: iron oxide, titanium dioxide, talc, opadrai and macrogol.

Form of issue

The drug is only available in tablet form. Dosage of the drug: 10 and 20 mg of the active substance. The blister can contain 7 or 10 tablets. Number of tablets in a cardboard bundle: 7, 14, 28, 35, 42, 50, 56, 60, 90, 98 or 100 pcs.

Tablets have a pronounced hypotensive effect. The mechanism of action is based on the selective blocking of calcium receptors. Lercanidipine is able to slow the flow of calcium ions in the smooth muscle tissue and cardiomyocytes.
Lerkamen 20 reduces the overall peripheral vascular resistance and gradually widens the lumen of the vessels, preventing the development of reflex tachycardia and collapse. The drug does not have a negative inotropic effect.

Pharmacodynamics and pharmacokinetics

Lerkamen 10 is well absorbed in the digestive system. The maximum plasma concentration is recorded after 1.5-3 hours. Up to 98% of the active substance binds to plasma proteins.
Biological availability of the drug increases after eating, which is why it is recommended to take tablets on an empty stomach. With increasing doses, increased bioavailability is noted. Initially, the drug is metabolized in the hepatic system, which explains the low absolute bioavailability - 10%.
The hypotensive effect is observed during the day. The active component is excreted through the renal system; the elimination half-life is 10 hours.

Indications for use

Lercanidipine is prescribed to patients suffering from an essential form of arterial hypertension (severity: mild and moderate).

Contraindications

    heart failure;
    obstruction of vessels emerging from the left ventricle;
    individual hypersensitivity to lercanidipine;
    lactase insufficiency;
    pregnancy;
    an unstable form of angina pectoris;
    acute myocardial infarction;
    syndrome of weakness of the sinus node (in patients without a pacemaker);
    lactation;
    the age limit is 18 years.

Side effects

Tablets are intended for oral administration. Do not grind the preparation, damage the protective shell. The effectiveness of the drug increases with the use of lercanidipine on an empty stomach. The duration of therapy is determined by the doctor.
Instructions for use Lerkamen 10: The tablet is taken 1 time every 24 hours. With insufficient reduction in blood pressure after 14 days, you can go to a single dose of 20 mg.
Instructions for Lerkamen 20: tablets washed down with water, on an empty stomach. With an increase in the dose of the drug (more than 20 mg per day), an increase in the hypotensive effect is not observed, but an increase in the frequency and severity of unwanted side reactions is recorded.

Overdose

There is a drop in blood pressure, drowsiness, ischemic changes in the tissues of the myocardium, nausea, development of cardiogenic shock. Recommended induction of vomiting, the appointment of laxatives, enterosorbents. In severe intoxication appoint dopamine, catecholamines, diuretics. Interaction

Absorption of the medication is enhanced by the treatment with midazolam. Inductors CYP3A4, ethanol, cyclosporine and grapefruit juice are categorically contraindicated when taking Lerkamen.
Caution is required when taking high doses of cimetidine (more than 800 mg).
Bioavailability of the active substance increases simultaneously with an increase in the negative inotropic effect during therapy with beta-blockers.
Required correction of the dose of digoxin after the initiation of lercanidipine therapy is required.
To reduce the risk of negative interaction, it is necessary to maintain a 10-hour window when taking simvastatin and lercanidipine.
