pharmachologic effect
On the human body Pektrol has a vasodilating, vasodilating effect.
Pharmacodynamics and pharmacokinetics
Pektrol is a peripheral vasodilator with a predominant effect on venous vessels. By stimulating the formation of endothelium in the vascular endothelium, the relaxing factor of the endothelium, triggers the activation of the intracellular transmembrane protein-guanylate cyclase, which increases the concentration of the mediator of vasodilation-cyclic guanosine monophosphate (cGMP).
Vasodilatation reduces the venous return to the heart - preload, as well as the burden on the heart, which reduces the need for myocardium in oxygen. The total peripheral resistance (postload) is reduced by the dilatation effect of nitrates.
In addition, the drug has:
coronary dilatation and promotes redistribution of the blood flow of the heart in a region with reduced blood supply;
when swelling of the lungs reduces the flow of blood to the right atrium, which leads to regression of symptoms;
increases tolerance to physical exertion in patients with IHD and angina pectoris;
inhibits aggregation and reduces the synthesis of platelet thromboxane;
Since Pektrol is nitrate, its reception leads to the development of cross tolerance. The abolition of the drug or a break in treatment quickly restores sensitivity.
Indications for use of Pektrol
Most often appoint Pektrol for the prevention of angina attacks. In addition, indications for the use of Pektrol are some forms of pulmonary hypertension, the pathology of the right heart, called the pulmonary heart, chronic heart failure, where the drug is used as a component of combined therapy.
Contraindications
vascular collapse;
cardiogenic shock;
acute circulatory disorders;
severe arterial hypotension (with systolic blood pressure less than 90 mm Hg and diastolic blood pressure less than 60 mm Hg);
reception of PDE inhibitor Sildenafil;
hypersensitivity to the components of Pektrol or nitrates.
Side effects
Regarding different organs and systems, Pektrol can cause the following adverse reactions:
Cardiovascular system: in the first days of treatment, there may be a "nitrate" headache, which usually disappears in the following days of therapy, and arterial hypotension and tachycardia, dizziness, sensation of weakness are also possible. In rare cases with severe arterial hypotension as a result of the paradoxical influence of nitrates, there was an increase in symptoms of angina pectoris, up to collapse, bradyarrhythmia and syncope.
Digestive system: rarely there is nausea and / or vomiting.
Skin: there may be a transient redness of the skin, exfoliative dermatitis.
Nervous system: slowing down of psychomotor reactions.
Instructions for use Pektrol (Method and dosage)
Tablets should be taken orally one hour before or two hours after eating. To swallow it is necessary entirely, not chewing and washing down with water.
For the prevention of angina attacks: begin the course with a standard dose of 40 mg (1 tab.) Once a day in the morning. If necessary, increase the dose to 60 mg, which should also be taken for 1 time. If patients develop nocturnal attacks of angina, the medication is recommended in the evening. Taking into account the clinical picture from 3-5 days of the course of admission, it is possible to increase the single dose to 60-80 mg, in some cases it can reach 80 mg per day, and the regimen is: 1 tablet. dosages of 40 mg twice a day.
Instructions for use Pektrol in the treatment of CHF is individual and should be selected taking into account the therapeutic effect and the resulting adverse reactions. Treatment in this case is carried out in a hospital.
Attention! The dose of the drug and the regimen should be determined by the attending physician individually, based on the severity of the disease and the condition of the patient.
Overdose
It is accompanied by such symptoms as: the onset of syncope, headache, dizziness, visual disturbances, hyperthermia, seizures, skin hyperemia, bradycardia, increased sweating, nausea, vomiting, diarrhea, paralysis, collapse, increased intracranial pressure, methemoglobinemia (cyanosis, anoxia - most often with a chronic overdose), hyperpnoea, dyspnea, right up to the confluence of the patient into a coma.
As a treatment, gastric lavage is used, with methemoglobinemia, 1 g of ascorbic acid is prescribed, iv methylthioninium chloride 1-2 mg per kg of body weight (1% solution, up to 50 ml). In addition, hemodialysis, oxygen therapy, and blood transfusion are effective. With a marked decrease in blood pressure, symptomatic therapy is required-the management of intravenous phenylephrine, with epinephrine and other related compounds being ineffective.
Interaction
In parallel with the medicines listed below, these tablets can cause:
With adsorbents, astringent, enveloping agents, a decrease in the absorption of isosorbide mononitrate is observed.
With anticholinergic substances - memory and attention violation in elderly patients.
Vasodilators, antihypertensive drugs, calcium channel blockers and beta-blockers, tricyclic antidepressants, antipsychotics, PDE inhibitors, ethanol-containing drugs can cause, in combination with Pektrol, an increase in antihypertensive effect.
Simultaneous reception with dihydroergotamine causes potentiation of their effects, whereas with noradrenaline - a decrease in the therapeutic effect of the latter.
With Sildenafil, the risk of severe arterial hypotension or myocardial infarction is high.
