Antifungal agent.

Pharmacodynamics and pharmacokinetics

Itrazole - synthetic antimicotic preparation for systemic use, has a wide antifungal spectrum, is a derivative of triazole, the active ingredient is Itraconazole.
Itraconazole inhibits ergosterol synthesis by cytoplasmic membrane of fungus cell. Effective against members of the genus dermatophytes (Microsporum, Trichophyton, Epidermophyton floccosum), has a fungistatic effect against fungi of the genus Candida (including Candida glabrata, etc.), shown against fungi (Histoplasma, Cryptococcus neoformans, Aspergillus, Blastomyces dermatidis, Paracoccidioides, brazonia, Sporothrix schenckii, Cladosporium).
Bioavailability is high, increases in the case of the drug Itrazole immediately following the main meal. High concentration of Itrazole (exceeding the concentration in blood plasma) is observed in the liver, musculoskeletal system, spleen and stomach.
Maximum tropnostyu drug has in relation to the skin tissues, including nail plates and hair. Effective for long-term use, with the defeat of the nail bed effect makes sense to evaluate after 6-9 months of therapy.
Metabolized in the liver, the metabolites of Itraconazole have antifungal activity.
Half-life of Itrazole from blood plasma - up to one and a half days. A significant part of the drug is excreted in the urine, the remaining-with feces. It should be remembered that part of Itrazole accumulates in the skin and nail tissues, and the removal of substances from these tissues is much slower, which allows them to reach high concentrations of Itraconazole.

Indications for use

Itrazole is indicated in dermatology in the treatment of dermatomycosis, fungal keratitis, onychomycosis, which is caused by fungi of the genus dermatophytes or Candida genus.
The indication for the appointment of Itrazole is systemic mycosis, candidiasis and systemic aspergillosis. Also, the drug is effective in the treatment of cryptococcosis (including cryptococcal meningitis), histoplasmosis, sporotrichosis, paracoccidioidomycosis, blastomycosis and other systemic or tropical mycoses.
Treatable with Trazolam kandidomikozy in which there is a loss of mucosal (including vulvovaginal candidiasis) and skin.
Other indications for the appointment of Itrazole include bran lichen and deep visceral candidiasis.

Contraindications

Itrazole should not be prescribed for children unless there is evidence of life. This is true for patients with heart failure of the third stage, in violation of the liver parenchyma, which is accompanied by severe insufficiency.
For the treatment of pregnant Itrazole can be used only when the predicted benefits of therapy outweigh the hypothetical risk of pathology and fetal deformities. The above is true and relatively use of Itrazole under breast-feeding.

Side effect

Digestive tract: morbidity in the area of epigastrium, nausea, there may be disturbances in the intestine, which will be manifested by constipation or diarrhea.
Nervous system: headache, cerebellar or vestibular symptoms, dizziness, ringing in the ears. In rare cases, there may be violations of the visual analyzer, tearing.
Allergic manifestations of the skin: rash, hyperemia of the skin, urticaria, itching and burning. In extremely severe cases, polymorphic exudative erythema and even necrosis of individual sections of the skin develop.
Rare complications of therapy with Itrazole include baldness, dysmenorrhea, menstrual irregularities, development of heart failure with stagnant phenomena in a small circle of blood circulation.
It is possible to defeat the liver, up to toxic hepatitis with the addition of signs of liver failure (hyperbilirubinemia, increased activity of transaminases).

Instructions for use Itrazole (Method and dosage)

For best absorption, Itrazole (in the form of capsules containing 100 milligrams of the drug) should be taken after meals. Capsules should be swallowed whole.
Depending on the nature of the disease, the use of the following schemes is possible: for the treatment of onychomycosis, the so-called pulse therapy is used: it consists in taking Itrazole daily for two capsules twice a day for one week.
In the treatment of fungal nail infections of the hands and feet, there are some differences. So, for the treatment of hand nails recommend two courses of pulse therapy, and for the treatment of mycosis of the nails of the feet need to increase the number of courses to three. The interval between courses when Itrazole is not accepted is three weeks.
Clinical results and efficacy should be evaluated only at the end of a full course of therapy, which is up to four weeks in the treatment of fungal infection of the skin, and up to ten weeks in the treatment of onychomycosis.
According to the instructions for use of Itrazole, except pulse therapy can be used and the scheme of continuous therapy: in this case, take a day once a capsule in a dose of 200 milligrams for three months.

Overdose

Overdose is almost impossible, but with the appearance of pronounced adverse reactions should be performed gastric lavage, and then assign enterosorbents. There is no specific antidote.

Interaction

Itrazole affects the metabolism of drugs that are metabolized in the body in the liver with the participation of cytochrome P450. This applies to terfenadine, astemizole, cisapride, midazolam, methylprednisolone, triazolam, indirect anticoagulants, quinidine, tacrolimus, vincristine and cyclosporine.
As for the metabolism of Itraconazole, those drugs that inhibit the activity of cytochrome P450 (ritonavir, clarithromycin, indinavir), increase the bioavailability of Itrazole. Reverse effect have rifampicin, isoniazid and phenytoin-they increase the activity of microsomal oxidation, thereby accelerating the destruction of Itraconazole in the liver.
Drugs that are widely used in the treatment of gastritis and peptic ulcer disease (antacids, cholinoblockers, hydrogen pump blockers, histamine receptor blockers) slow down the absorption of Itraconazole, and therefore should not take Itrazole, if the above drugs are taken less than two hours ago.
In combination with cisapride, Itrazole increases the toxicity of the latter, there is a risk of ventricular arrhythmias.

Terms of sale

To buy Itrazole the prescription is not required.

Storage conditions

In a dark, inaccessible place for children at a temperature of not more than 25 degrees Celsius.
Shelf life is not more than three years.

Special instruction

In patients with a shift in the immune system (AIDS, organ transplantation in the recent past, changes in the morphological composition of the blood) requires an increase in dosage.
If the patient has cirrhosis or renal failure, Itrazole is prescribed under constant control of the concentration of Itraconazole in plasma, and only when the predicted effect of therapy is higher than the risk of possible liver damage.
If you choose a treatment strategy in which the drug is prescribed for more than one month, it is necessary to systematically monitor the activity of liver enzymes and check other laboratory parameters of the body (the level of direct and indirect bilirubin, the content of blood clotting factors, the amount of protein in the blood plasma).
The appearance or progression of signs of disturbance of the liver Itrazole should be abolished.
When neuropathy (peripheral nervous system disorders) also stop treatment.
