The preparation of Onglyza in the composition has the active substance saxagliptin, which is a very strong selective competitive reversible inhibitor of dipeptidyl peptidase-4.
If the drug is taken by patients who suffer from diabetes, they are inhibited for 24 hours in the activity of the enzyme DPP-4.
After the patient took glucose inside, due to inhibition of DPP-4, a 2-3-fold increase in the concentration of a glucose-dependent insulinotropic polypeptide, a glucagon-like peptide-1, occurs. The concentration of glucagon decreases and the response of the glucose-dependent beta-cell response increases. As a result, the body increases the concentration of insulin and C-peptide.
Owing to the release of insulin by beta cells of the pancreas and the decrease in the release of glucagon from pancreatic alpha cells, fasting glycemia decreases, and postprandial glycemia decreases.
In the process of taking saxagliptin in patients, weight gain is not observed.

Pharmacokinetics and pharmacodynamics

In the body, saksagliptin is absorbed rapidly after administration before meals. After oral administration, about 75% of the dose is absorbed. With blood proteins, saxagliptin and its metabolite are not significantly associated.
The maximum concentration of saxagliptin and its main metabolite is noted in the plasma, respectively, for 2 hours and 4 hours.

    for starting combined treatment with Metformin;
    as an addition to monotherapy with thiazolidinediones, metformin, sulfonylurea derivatives, if there is no adequate glycemic control in such treatment.

Contraindications

Overdose

Interaction

According to the research data, there is a relatively small risk of clinically significant interaction of saxagliptin with other drugs.
With simultaneous application of CYP 3A4 / 5 isozymes inductors (dexamethasone, carbamazepine, phenobarbital, rifampicin, phenytoin), the concentration of the main metabolite of saxagliptin can decrease.
Since sulfonylureas can provoke hypoglycemia to reduce risk, it may be necessary to reduce the dose of sulfonylureas with simultaneous treatment with Onglyza.
