pharmachologic effect
Urological tool.
Pharmacodynamics and pharmacokinetics
The active component is tolterodin. The drug improves urodynamics. The active substance is a blocker of muscarinic receptors, and acts specifically for the muscles of the bladder. The medication reduces the average volume of urine, eliminates urinary incontinence and decreases the frequency of urination. The active ingredient does not affect the Q-T interval.
The pronounced effect of the drug Urotol is manifested by the end of the 4th week of therapy. Tolterodine is well absorbed in the digestive system, the peak concentration in plasma is recorded after 1-3 hours. The active component is metabolized to the main metabolite and several inactive substances that are excreted by the kidneys. In individuals with severe pathology of the hepatic system, an increase in the level of unchanged tolterodine in plasma is observed. With kidney disease, the half-life of tolterodine is increased.
Indications for use
Urotol is prescribed to patients who suffer from multiple imperative urges to urinate.
The drug is used in highly active bladder, with urinary incontinence.
Contraindications for Urotol
Urotol is not prescribed for intolerance of tolterodine, with urinary retention, severe course of ulcerative colitis, angle-closure glaucoma, toxic megacolon.
In pediatric practice do not prescribe the drug Urotol. The instructions for use recommend caution to use tolterodine for neuropathy, for diseases of the renal system and liver, for reduced intestinal motility, as well as for patients with a high risk of developing myasthenia gravis ("muscular weakness").
When treating patients who receive antiarrhythmics, special caution is required when registering an extended Q-T interval. It is unacceptable during the period of treatment to drive vehicles and other dangerous machinery.
Side effects
Frequent undesirable reactions: reduced tearing, dyspeptic disorders, dry mouth. Less frequent fatigue and memory impairment.
Central nervous system: accommodation change, severe disorientation, vertigo, confusion, impaired visual perception, increased drowsiness, headaches, hallucinations.
Digestive system: increased flatulence, pain in the epigastric region, constipation (due to decreased intestinal motility), gastroesophageal reflux, dry mouth. Rarely recorded weight gain and the development of diarrhea syndrome.
Changes in the heart and vascular system: feeling of palpitations, redness of the upper body and face skin, tachycardia.
Some patients have allergic responses in the form of dermatitis, urticaria. Rarely does angioedema and anaphylactoid reactions develop.
Rarely exacerbated symptoms of dementia, recorded chest pain, pain during urination, dry skin, exacerbation of chronic bronchitis, swelling, urinary retention.
Urotol, instructions for use (method and dosage)
The drug is taken entirely inside. Do not select the dose by dividing the pill. The drug can be taken regardless of the meal. The duration of therapy is determined only by the attending physician.
For urinary incontinence, highly active bladder prescribed 2 mg twice a day. With poor tolerability of tolterodine, the scheme can be changed: 1 mg twice a day. In diseases of the kidneys and liver system, while taking strong inhibitors of CYP-3A4, the daily dose is reduced to 2 mg.
The drug Urotol is recommended to be taken within 6 months. If necessary, receive tolterodina can be continued after six months.
Overdose
With a one-time intake of 12.8 mg of tolterodine, the pain of the urge to urinate is recorded, and accommodation is disturbed.
Signs of overdose: mydriasis, respiratory failure, tachycardia, convulsions, nervous and emotional arousal, urinary retention, hallucinations.
Antidote not developed. Recommended intake of enterosorbents, gastric lavage, symptomatic treatment.
With the development of hallucinations, overexcitation and other central anticholinergic effects, physostigmine is used.
In seizure syndrome, benzodiazepines are administered.
When tachycardias and respiratory changes are carried out measures to restore breathing and use beta-blockers.
With the development of mydriasis in the background of receiving large doses of the drug Urotol, drops for the eyes with pilocarpine are prescribed.
With prolonged urinary retention, catheterization is performed.
Interaction
Medicines that have anticholinergic effects enhance the effect of urotol and increase the risk of unwanted and adverse reactions.
Tolterodine reduces the effectiveness of prokinetics.
Muscarinic receptor agonists slow down the effect of the drug.
Simultaneous treatment with antifungal agents (itraconazole, ketoconazole, miconazole) and macrolide antibiotics (clarithromycin, erythromycin) is unacceptable.
Clinically significant pharmacological interaction with simultaneous therapy with oral contraceptives and warfarin is not registered.
Terms of sale
The prescription is not required to buy Urotol.

special instructions
Before the appointment of the medication, it is necessary to exclude organic changes that can lead to an imperative urge to urinate.
Women are recommended to use reliable methods of contraception for the entire period of therapy with the drug Urotol.
The drug can disrupt the accommodation and speed of psychomotor reactions, which limits its use in the management of complex mechanisms, vehicles.
