Release form

Round-shaped biconvex white tablets with the inscription "ALN 70" on one side.
    2 tablets in a blister - 1, 3, 2, 4 or 6 blisters in a pack of cardboard;

Pharmacodynamics

A specific non-hormonal bone osteoclastic resorption inhibitor, which inhibits osteoclast activity. Forosa stimulates osteogenesis, restores the right balance between destruction and restoration of bone tissue, increases bone density (normalizes phosphorus-calcium metabolism), facilitates the growth of bone tissue with a physiological histological structure.

Pharmacokinetics

Oral bioavailability (on an empty stomach) ranges from 0.64 to 0.59%. When used alendronate with food and within 120 minutes after a meal, bioavailability is negligible. When taken simultaneously with orange juice or coffee, it is also reduced by 60%. The content of Forosa in the blood is low. Plasma protein binding is 78%.
Data confirming the biotransformation of alendronate in the human body are not available. The half-life is close to 72 hours. About half of the administered dose is excreted in the urine and a small amount in the feces.

Indications for use

    treatment of osteoporosis with the prevention of fractures in men;
    treatment of osteoporosis in postmenopausal women, as well as for the prevention of compression fractures of the vertebrae and fractures of the femoral neck in this group of patients;
    therapy of osteoporosis caused by prolonged intake of glucocorticosteroids.

Contraindications for Forosa

    allergic to the components of the drug;
    strictures or impaired esophagus;
    the inability of the patient to stand or sit for half an hour;
    pronounced metabolic disorders of minerals;
    hypocalcemia;
    severe kidney failure;
    pregnancy and lactation;
    age under 18 years old.
Forosa tablets should be used with caution to persons with diseases of the digestive system (gastritis, dysphagia, after surgery on the organs of the upper digestive system, exacerbation of peptic ulcer, duodenitis, gastrointestinal bleeding, hypovitaminosis D).

Side effects of Forosa

    Digestive reactions: melena, dyspepsia, esophagitis, abdominal pain, heartburn, dysphagia, nausea, vomiting, gastritis, bleeding and ulceration of the mucous membranes of the upper digestive system, stricture of the esophagus, esophageal perforation.
    Reactions from the nervous activity: headaches, taste perversion, dizziness, irritability.
    Reactions from view: scleritis, uveitis, inflammation of the connective tissue of the episcleral region.
    Skin reactions: angioedema, hyperemia of the skin, alopecia, Stevens-Johnson syndrome, urticaria, photosensitivity, epidermal toxic necrolysis.

Forosa, instructions for use (method and dosage)

The drug is taken orally.
In order to ensure a better absorption of Forosa, it should be consumed in the morning on an empty stomach, with a glass of water and not earlier than half an hour before the first dose of food, drinks or other medicines. Other beverages (including mineral water) can reduce the absorption of the active substance.
To reduce irritation of the mucous membranes of the esophagus, Forosa tablets should be taken:
    only after the final awakening from sleep and getting out of bed;
    swallowing whole (not chewing, not dissolving or dissolving them in the mouth due to the risk of ulceration;
    it is forbidden to take a lying position before the first meal, which follows no sooner than half an hour;
    It is forbidden to take medicine before going to bed or after sleeping before getting out of bed.
The standard dosage of the drug Forosa is 70 mg once a week.
The dosage is not subject to correction in the elderly and patients with diseases of the liver and kidneys. It is not recommended to prescribe a drug for people with severe kidney dysfunction.

Overdose

Signs of overdose: dyspepsia, hypocalcemia, gastritis, abdominal pain, heartburn, dysphagia, esophagitis, hypophosphatemia.
Treatment of overdose: symptomatic; taking milk and antacids to bind alendronate. Due to the high risk of ulceration of the esophagus, it is forbidden to induce vomiting, the patient must be in an upright position.

Interaction

Co-administration of calcium or antacids suppresses alendronate absorption. Therefore, it is recommended to take other drugs at least half an hour after taking Forosa.
Anti-inflammatory nonsteroidal agents (including Aspirin) may enhance the undesirable effects of Forosa on the part of the digestive system.
The use of alendronate with a large number of the most used drugs was not accompanied by the appearance of significant clinical interactions.

The use of alendronate at bedtime or in a prone position increases the likelihood of esophagitis.
Before starting treatment with a drug, correction of hypocalcemia and other metabolic disorders (for example, vitamin D deficiency) must be made. Due to the increase in bone density during treatment with alendronate, a slight asymptomatic decrease in the content of calcium and phosphate in the blood is not excluded. Therefore, it is necessary to ensure the supply of a sufficient dose of calcium and vitamin D, especially in individuals receiving glucocorticosteroids.
Patients should be warned that when you skip taking another pill, you need to take it in the morning of the next day (it is forbidden to take 2 tablets per day). Subsequently, it is recommended to continue taking Forosa on the day of the week selected at the beginning of the treatment.
Rarely, when using other bisphosphonates, osteonecrosis of the jaw was detected. Risk factors that trigger osteonecrosis of the jaw: malignant tumor, concomitant treatment (radiation therapy, chemotherapy, corticosteroids), gum disease, infection, coagulopathy, anemia. Most of these cases are registered with intravenous bisphosphonates.
Dental surgery during treatment with bisphosphonates can aggravate osteonecrosis of the jaw.

During pregnancy and lactation
