Composition

The preparation contains the main component: fluticasone propionate.
Additional components: dextrose, carboxymethylcellulose and microcrystalline cellulose, phenylethyl alcohol, benzalkonium chloride solution, hydrochloric acid, polysorbate 80 and purified water.

Form of issue

It is issued Flixonase in the form of nasal spray, packaged in 60 and 120-dose vials, in a carton.

pharmachologic effect

The spray has anti-allergic, anti-edematous and anti-inflammatory effect.

Pharmacodynamics and pharmacokinetics

This drug exhibits anti-inflammatory effect due to interaction with the GCS receptors. In this case, suppression of proliferation of mast cells, lymphocytes, eosinophils, macrophages and neutrophils is noted. Due to fluticasone propionate, there is a decrease in the production of inflammatory mediators and other biologically active components, for example: histamine, cytokines and leukotrienes, at an early and later stage of allergic reactions. As a result, the patient's reaction to bronchodilators is restored, allowing to reduce the frequency of their use. The frequency of sneezing, itching in the nose decreases, the runny nose and nasal congestion, discomfort in the paranasal sinuses and a feeling of pressure on the nose and eyes are eliminated. In addition, eye symptoms related to allergic rhinitis are alleviated.
The application of the spray has a quick anti-inflammatory effect on the nasal mucosa after 2-4 hours from the time of the first injection.
Reduction in the severity of symptoms, mainly nasal congestion, may persist for at least 24 hours after the administration of a single dose.
The use of the drug in compliance with the recommended dosages does not cause the severity of its systemic activity and does not contribute to the suppression of the hypothalamic-pituitary-adrenal system.
Intranasal spray administration does not lead to detection of the drug in the blood plasma. This means is characterized by low absorption, since its main part is swallowed. Absorption of their gastrointestinal tract into the blood is only about 1% of the dose. However, fluticasone propionate undergoes rapid distribution and a significant ability to bind to plasma proteins. Removal of the drug from the body occurs quickly enough, mainly in the form of metabolites in the bile.

As a rule, this does not require special treatment - it is enough to adjust the dosage and control the level of cortisol in the plasma.

Interaction

It was found that with the use of Flixonase, low concentrations of its active substance fluticasone propionate in the blood plasma are formed. Therefore, the emergence of meaningful interactions is unlikely.
However, the combination with ritonavir and other inhibitors of the CYP3A4 isoenzyme enzyme system of cytochrome P450 can increase the concentration of this drug.

special instructions

This drug can be used only intranasally, with a duration of treatment up to 6 months. If treatment lasts longer than this period, then control of the adrenal cortex is required.
A full-fledged effect of the spray appears only after a few days of use, so a regular therapeutic regimen should be followed.
