Composition

In one Irifrin bottle, 10% contains phenylephrine hydrochloride - 100 mg, and in one Irifrin bottle 2,5% - 25 mg.
Excipients: benzalkonium chloride, edetate disodium, sodium hydroxide, sodium hydrogenphosphate dihydrate, anhydrous sodium dihydrogen phosphate, sodium metabisulphite, citric acid, sodium citrate dihydrate, purified water.
One bottle of Irifrin BK (without preservative), 2.5% contains phenylephrine hydrochloride - 25 mg.

Form of issue

Eye drops Irifrin is a clear, colorless or light yellow solution. 5 ml of such a solution in a dropper vial or a vial of darkened glass, one such dropper or a bottle of darkened glass in a cardboard bundle.

Pharmacodynamics

Phenylephrine is an adrenomimetic with alpha-adrenergic activity. When taken in conventional doses does not produce a stimulating effect on the central nervous system.
With local use in ophthalmology stimulates the outflow of intraocular fluid, narrows the vessels of the conjunctiva and causes the dilatation of the pupil (mydriasis).
Phenylephrine has a strong stimulating effect on alpha-adrenergic receptors in postsynaps, has an insignificant effect on the beta-adrenoreceptors of the heart region, it has almost no positive inotropic and chronotropic effects. The drug has a vasoconstrictive (vasoconstrictive) action, similar to the action of norepinephrine, but in phenylephrine it is weaker and longer.
Vasoconstriction occurs 30-90 s after use, the duration of the action is 2-5 hours.
After the introduction (instillation), phenylephrine helps reduce the dilator of the pupil, causing mydriasis. Dilation of the pupil is achieved within 20-60 min after instillation; continues after instillation of 2.5% solution for 2 hours, 10% solution - 3-5 hours.

Pharmacokinetics

Phenylephrine freely penetrates the tissues of the organ of vision. The maximum concentration in the blood comes in 10-20 minutes after the local application. Preliminary instillation of anesthetics increases absorption in the systemic circulation and prolongs mydriasis. The drug is excreted in the urine in its original form (less than 20%) and in the form of inert metabolites.

Indications for use

    The need for dilated pupils in ophthalmologic diagnostic procedures (control of the posterior segment of the eye).
    Iridocyclitis (prevention of the appearance of posterior synechia and relaxation of exudation in the iris).
    Carrying out a provocative examination in persons with a narrow angle of anterior chamber and possible closed-angle glaucoma.
    In surgical ophthalmology with preoperative training to enlarge the pupil.
    Differential diagnosis of deep and superficial eye injection.
    Therapy of glaucomycyclic crises.
    Performing laser interventions at the bottom of the eye and in vitreoretinal surgery.
    Therapy of the "red eye" syndrome (to reduce hyperemia and sensitivity of the ocular membranes).
    Inside and topically: to reduce edema of the mucous nasopharynx and conjunctiva for allergic and catarrhal diseases (as part of a combination treatment, only for Irifrin BC).
    Parenteral: to increase blood pressure during collapse and hypotension caused by decreased vascular tone (only for Irifrin BC).

Contraindications

    Allergy to the components of the drug.
    Closed-angle or narrow-angle glaucoma.
    Diabetes.
    Severe disorders of the cardiovascular system (arterial hypertension, heart disease, aneurysm, tachycardia).
    Thyrotoxicosis, hyperthyroidism.
    Application together and within 14 days after discontinuation of therapy with MAO inhibitors.
    Expansion of the pupil during surgery in patients with a defect in the integrity of the eye or in violation of tear outflow.
    Simultaneous use with tricyclic antidepressants, antihypertensive drugs (including beta-blockers).
    Congenital deficiency of glucose-6-phosphate hydrogenase.
    Porphyria.
    Newborns with low weight.

Side effects

    From the eyes: conjunctivitis, edema of the periorbital area; possible burning sensation, irritation, blurred vision, lacrimation, a feeling of discomfort, increased intraocular pressure.
    After taking Irifrin, a reactive miosis may appear. With repeated use of the drug at this time, mydriasis is usually less pronounced. This effect is typical for the elderly.
    From the cardiovascular system: a feeling of palpitations, tachycardia, reflex bradycardia, arrhythmia, arterial hypertension, coronary artery occlusion, pulmonary embolism.
    Due to the strong contraction of the dilator pupil caused by the action of phenylephrine, after 35-45 minutes after use in the cavity of the anterior chamber, small pigment grains from the iris leaf may appear. This suspension must be differentiated with anterior uveitis or with the presence of blood cells in the cavity of the anterior chamber.
    From the skin: dermatitis.
    Very rarely, when using Irifrin (drops of 10% for eyes), severe disorders occur, including vascular collapse, myocardial infarction and intracerebral haemorrhage.

The instruction for Irifrin prescribes the use of drops locally.
With ophthalmoscopy, a single use of a 2.5% solution is used. For the appearance of mydriasis, it is sufficient to instill one drop of Irifrin 2.5% into the conjunctival sac. The expansion of the maximal force is achieved in 20-30 minutes and lasts for 1-3 hours. If necessary, it is possible to prolong mydriasis, for which an additional instillation is performed in an hour.
In children from 12 years and adults with weak dilatation of the pupil to provoke diagnostic mydriasis, it is justified to use a 10% solution without dose adjustment.
During the diagnostic, a single use of a 2.5% solution can be used for:
    provocative test in persons with a narrow angle of anterior chamber and possible closed-angle glaucoma;
    differential diagnosis of the type of eye injection (superficial or deep);
    iridocyclite 2.5 or 10% solution is used to prevent development and rupture, the appearance of the posterior synechiae; to reduce exudation in the anterior eye chamber (one drop of Irifrin is instillated into the conjunctival sac 2-3 times a day);
    relief glaucomycyclic crises. Phenylephrine produces an antihypertensive effect in this pathology. 10% solution is used 2-3 times a day;
    preparation of patients for surgical interventions for 40-60 minutes to achieve the dilatation of the pupil is a single injection of 10% solution; after the violation of the integrity of the membranes of the eye, repeated use of the drug is prohibited.
A 10% solution can not be used to impregnate tampons during surgery.

Irifrin BC is also used:

    with hypermetropia with propensity to spasm of accommodation, accompanied by a high load on the eye - the drug is instilled in the evening together with a 1% solution of cyclopentolate. At standard visual load, Irifrin BC is used at bedtime 3 times a week;
    when treating true and false myopia - 1 drop before going to bed every 3 days for one month.

Overdose

Symptoms of overdose: manifestation of the systemic effect of phenylephrine.
Treatment: recommended the use of alpha-adrenergic blocking agents, for example 5-10 mg of phentolamine intravenously.

Interaction

The dilatation of the pupil is enhanced by the combined use of phenylephrine and atropine.
The use of 2.5 or 10% solution of the drug with MAO inhibitors and within three weeks after their withdrawal, should be carried out with caution, since in this case, an uncontrolled increase in blood pressure is possible.
Using Irifrin 10% in combination with beta-blockers can provoke acute arterial hypertension.
Application together with sympathomimetics can enhance the cardiovascular effect of phenylephrine.

Storage conditions

Since the duration of the pupil enlargement can reach 1-3 hours, patients may experience a photophobia, so, before restoring the vision function, you must protect your eyes from strong sunlight. Reading and watching TV programs are forbidden.
