Pharmacodynamics and pharmacokinetics
Pharmacodynamics
Hormonal preparation Climen is used for cyclic intermittent hormonal therapy in women in perimenopause and postmenopausal women of the early age at the age of 55 years and for permanent replacement therapy during menopause. It is also indicated for women of reproductive age, if necessary, to regulate the menstrual cycle.
This two-phase preparation contains estrogens and estrogens in combination with progestogens - this repeats a two-phase menstrual cycle. In its composition, estrogen - estradiol valerate, and as a progestogen acts cyproterone acetate, which has a pronounced antiandrogenic effect.
Thanks to the cyclical intake scheme, a menstrual cycle is established in women. At the same time, ovulation is not suppressed and the production of hormones in the body does not change. Estradiol during climax provides treatment for vegetative and psychoemotional symptoms ("hot flashes", sleep disturbance, sweating, irritability, nervous excitability, cardialgia, dizziness, and joint pain). Effectively prevents pathological changes in the mucosa of the urogenital tract and loss of bone mass (suppresses the function of osteoclasts).
Long-term use reduces the risk of fractures. Beneficially affects the level of collagen in the skin, the density of the skin, slows the formation of wrinkles. Ciproterone acetate has an effect on androgen-dependent diseases: seborrhea, acne, androgenic alopecia. The effect of the drug is noticeable in women who have pronounced atherogenic changes in the lipid spectrum - the level of total cholesterol, LDL is lower and the level of HDL is increased, the ratio between them is increased.
Pharmacokinetics
Estradiol valerate is rapidly absorbed. The liver is split into valeric acid and estradiol. The latter is further metabolized to estriol, estrone and catechol estrogens. They have significantly less estrogenic activity. The maximum concentration in the serum is determined after 4-9 hours. It binds to albumin and globulin. In the blood is present mainly in a bound form (30-40%). After discontinuation of the drug, estradiol and estrone return to the baseline within 2-3 days.
Cyproterone acetate is also fully absorbed. Bioavailability reaches 89%. The maximum concentration in the blood is determined after 1-2 hours. It binds to serum albumin. In the process of metabolism, a 15β-hydroxyl derivative is formed. Metabolites from serum are excreted for 1.7 days. A small part of the dose is excreted with bile, and the main part in the form of metabolites with bile and urine.

Climen, instructions for use (Method and dosage)
Tablets are taken orally, wholly, with a small amount of water. It is advisable to take at the same time. Take one tablet, from the 5th day of the cycle. At the conclusion of tablets from 1 package, it is necessary to make a weekly break, during which menstrual bleeding should be expected. After a break take the pill from the next package.
Overdose
At a random dose exceeding daily, no serious side effects were found. Perhaps the appearance of nausea, vomiting and vaginal bleeding. Symptomatic treatment is performed.
Interaction
Before beginning treatment stop using hormonal contraceptives. Long-term use of anticonvulsant drugs or antimicrobial agents increases the excretion of sex hormones, therefore, their effectiveness decreases. This is especially pronounced in the appointment of barbiturates, carbamazepine, primidon, rifampicin, oxcarbazepine, felbamate, topiramate, griseofulvin. Hormone replacement therapy is prescribed no earlier than 3-4 weeks after taking these drugs.
In some cases, the need for antidiabetics is changing.
Excessive consumption of alcohol leads to an increase in the concentration of estradiol in the blood.
Storage conditions
Temperature up to 20 ° С.
Shelf life - 5 years.
Reviews about Climen
Against the background of a decrease in ovarian production of estrogens, a complex of neuroendocrine, vegetative-vascular and neuropsychiatric symptoms appears first in women. Somewhat later, there are urogenital disorders and changes in appearance. After this, ischemic heart disease, osteoporosis, Alzheimer's disease appears.
Replacement hormone therapy is used to improve the general condition, prevent premature aging and various diseases that occur with age. With pathological symptoms in premenopause for the normalization of the cycle, women are recommended drugs in a cyclic mode that mimic the natural menstrual cycle - these are Estrogel, Klimonorm, Cyclone-Proginova, Dyufaston, Femoston and Climen, women's reviews about which are mostly positive.
There are undoubted advantages of substitution therapy. Women note a rapid regression of autonomic symptoms, a complex positive effect on the entire body, as well as preventive action against such complications as osteoporosis. However, there are also negative aspects of treatment. Among the shortcomings of this therapy - an impressive list of contraindications, side effects and the need for a gradual decrease in estrogen in the event of drug discontinuation, as well as a fairly high price. Often, the drug was prescribed to women of young age in cases of hyperandrogenism.
"... I was prescribed with a high level of androgens. Saw it for 3 months: androgens decreased, the skin became normal, new hair did not appear. "
"... It was necessary to reduce the level of androgens, so this drug was prescribed. I see nothing bad in hormones, if they are prescribed according to the indications after the examination. Treatment was scheduled for 4 months. There were no terrible side effects, only a little more weight was added. "
"... Appointed Climen, instruction on the use of which determines a large list of contraindications and as many adverse reactions. I even at first was afraid to take it. But, fortunately, everything was fine and the treatment was well for six months, my condition was normal. "
"... The hair on the chin began to grow. The cosmetician has sent to the gynecologist for inspection. Male hormones are off scale. Have appointed this agent for 5 months. The treatment was well tolerated. "
"... Propensity to acne and strongly falling out of hair. After the appointment of this drug, everything was normalized. "
"... Assigned with menopause. I am well tolerated and the condition has improved significantly. "
"... Took it 7 years, but began to worry about swelling, headache and increased blood pressure. Transfer to Femoston and side effects less. I carry it well. "
"... As for the early menopause, I have been taking hormones for 5 years now, including this one. I better move Angelique. "
It should be noted that this medication can only be prescribed by a doctor and you need to undergo a thorough examination before the treatment begins - gynecological and general medical.
