Contraindications

- Hypersensitivity to Femoston mini.
- Diagnosed or suspected breast cancer.
- Diagnosed or suspected estrogen-dependent malignant tumors (for example, endometrial cancer).
- Diagnosed or suspected progestogen-dependent neoplasms (eg, meningioma).
- Bleeding from the vagina of unknown etiology.
- Untreated endometrial hyperplasia.
- Thrombosis (arterial and venous) and thromboembolism currently or in history (including thrombosis, deep vein thrombosis; thromboembolism of the pulmonary artery, myocardial infarction, ischemic or hemorrhagic cerebrovascular disorders).
- Acute or chronic liver disease is currently or in history (before normalization of functional liver function tests), including malignant liver tumors.
- Porphyria.
- Pregnancy and breastfeeding period.
- Galactose intolerance, lactase deficiency, glucose-galactose malabsorption syndrome.
- Meningioma.
- Multiple or pronounced arterial or venous thrombosis factors associated with congenital or acquired predisposition: protein C deficiency, protein S deficiency, antithrombin III deficiency, presence of antibodies to phospholipids, angina pectoris, prolonged immobilization, severe forms of obesity, cerebral or coronary artery disease , transient ischemic attacks, complicated lesions of the valvular apparatus of the heart, atrial fibrillation.

Dosage and administration (instructions)

Femoston mini is taken orally daily, in continuous mode, 1 tablet per day (preferably at the same time of day), regardless of the meal.
Patients who make the transition from another continuous sequential or cyclic regimen of the drug should complete the current cycle, and then switch to Femoston mini. Patients who are not receiving drugs for HRT or are switching from continuous use of combination drugs for HRT can start taking Femoston R Mini on any given day.
If the patient misses taking the pill, it must be taken within 12 hours after the usual time of admission; otherwise, the missed pill should not be taken, and the next day it is necessary to take the pill at the usual time. Skipping Femoston mini may increase the likelihood of a "breakthrough" uterine bleeding or spotting.

Pharmachologic effect of Femoston mini

Estradiol compensates for the lack of estrogen in the body in postmenopausal women and reduces the severity of menopausal symptoms.
Didrogesterone is a progestogen that is effective when taken orally and has activity similar to parenterally administered progesterone.

Side effects

Neoplasms: benign, malignant and unspecified: increase in leiomyoma size.
Disorders of the genitals and breast: breast tension / tenderness, menstrual disorders (including "bloody" postmenopausal bleeding, metrorrhagia, menorrhagia, oligo-amenorrhagia, irregular menstruation, dysmenorrhea, abdominal pain, change in vaginal secretion, irregular menstruation, dysmenorrhea), lower abdominal pain, change in vaginal secretion, irregular menstruation, dysmenorrhea, abdominal pain, change in vaginal secretion, irregular menstruation, dysmenorrhea breast enlargement, premenstrual syndrome.
Mental disorders: depression, nervousness, changes in libido.
Violations of the skin and subcutaneous tissues: allergic reactions such as urticaria, skin rash and itching, vascular purpura, angioedema.
Heart disorders: myocardial infarction.
Disorders of the gastrointestinal tract: abdominal pain, nausea, vomiting, flatulence, dyspepsia.
Vascular disorders: venous thromboembolism, arteritis, varicose veins.
Disorders of the liver and biliary ducts: impaired liver function, sometimes in combination with jaundice, asthenia, malaise, gallbladder disease.
Nervous system disorders: headache, migraine, dizziness.
Disorders of the musculoskeletal and connective tissue: pain in the back (lower back).
Immune system disorders: hypersensitivity.
General disorders: asthenic conditions (weakness, malaise, fatigue), peripheral edema.
Infections and invasions: vaginal candidiasis.
Other: weight gain, weight loss.

Special instructions

Femoston mini is prescribed only if there are symptoms that adversely affect the quality of life. Therapy should continue until the benefits of taking the drug outweigh the risk of side effects.
Experience with the use of Femoston mini in women over 65 is limited.

Interaction

Perhaps an increase in plasma concentration to the toxic level of drugs:
tacrolimus and cyclosporin A (CYP 450 3A4, 3A3), fentanyl (CYP 450 3A4) and theophylline (CYP 450 1A2).
Efficiency may decrease in the following combinations:
- The metabolism of estrogens and progestogens can be enhanced by simultaneous use of microsomal enzymes at the same time as the liver enzymes, P450 2B6, 3A4, 3A5, 3A7: anticonvulsants (phenobarbital, carbamazepine, phenytoin) and antimicrobials (rifampicin, anti-microbes (rifampicin, acetaminum, carbamazepine, phenytoin) and antimicrobial drugs (rifampin, anti-microbial).
- Ritonavir and nelfinavir, although known as strong inhibitors of CYP 450 3A4, A5, A7, when used simultaneously with sex hormones can enhance their metabolism.
- Herbal preparations containing Hypericum perforatum (Hypericum perforatum) can increase estrogen and progestogen metabolism through CYP 450 3A4.
- Increased metabolism of estrogen and progestogen clinically may be manifested by a decrease in the effect of Femoston mini and the appearance of bleeding from the vagina.
