pharmachologic effect

Wikipedia indicates that Novocaine (INN: Procaine) is a remedy for local anesthesia that shows moderate anesthetic activity. The name in Latin is Novocainum. The formula of the active substance is C13H20N2O2. Qualitative reactions to novocaine are described in the textbooks on pharmaceutics. Has a great breadth of therapeutic influence. The active substance blocks Na + channels, preventing the generation of pulses and their conduction along nerve fibers.
Under the influence of procaine, the action potential in the membranes of nerve cells changes, and there is no pronounced effect on the resting potential. The agent suppresses pain impulses, and impulses of another modality in the body.
When administered directly into the bloodstream and during absorption, it lowers the level of excitability of peripheral cholinergic systems, reduces the production and release of acetylcholine from preganglionic endings.
As evidenced by the pharmacopeia, Novocain relieves spasm of smooth muscles, reduces the level of excitability of the myocardium and motor cortical areas of the brain. Under its influence, the inhibition of polysynaptic reflexes is suppressed, the descending inhibitory effects of the reticular formation of the brain stem are eliminated. When taking large doses of the drug, the patient may develop seizures.
Short anesthetic activity of the drug is noted. The duration of infiltration anesthesia is 0.5 to 1 hour.

Pharmacokinetics and pharmacodynamics

In the body, complete systemic absorption of the active substance is noted.
The level of absorption depends on the route of administration, the site of administration, and also on the dose of the drug. The substance in the body quickly hydrolyses, resulting in the formation of two major metabolites, pharmacologically active. This is diethylaminoethanol, which produces a mild vasodilating effect, and para-aminobenzoic acid (a competitive antagonist of sulfonamide drugs that attenuates their antimicrobial effect). Half-life is 30-50 s, the half-life in the neonatal period is 54-114 s. The body is mainly excreted through the kidneys, about 2% is excreted unchanged. Through the mucous membranes is absorbed poorly.

Indications for use

Novocaine is used for infiltration, intraosseous, epidural, conductive, spinal anesthesia. It is also used for anesthesia of the mucous membranes in the treatment of ENT diseases. It is also used for paranephric, vagosympathetic cervical, paravertebral and circular blockade.
Novocain is used to potentiate the effect of the main drugs used for anesthesia; is also administered intravenously with the aim of arresting the pain syndrome of different origin.
Intramuscularly used to dissolve penicillin to prolong its duration. It is also noted that such a drug as an auxiliary drug in the following diseases:
    atherosclerosis;
    endarteritis;
    spasms of cerebral vessels and coronary vessels;
    arterial hypertension;
    diseases of the joints of infectious and rheumatic origin.
Rectally used candles with Novocaine in hemorrhoids and anal fissures, in case of spasms of smooth muscles of the intestine.

Contraindications

There are some contraindications to the use of medication. Do not use Novocain IV and intramuscularly with high sensitivity to the agent, as well as other local anesthetic esters and paraaminobenzoic acid. Do not prescribe the drug to children under 12 years.
For local anesthesia, the remedy is not used if there are severe fibrotic changes in the tissues.

Caution Novocaine is used for:

    emergency operations that accompany acute blood loss;
    Conditions that are characterized by a decrease in hepatic blood flow;
    cardiovascular insufficiency progressing;
    a lack of pseudocholinesterase;
    Inflammatory diseases or infection of the injection site;
    renal failure;
    under the age of 18 and after age 65.

Side effects

In the process of application such side effects can be noted:
    central and peripheral NA: headaches, dizziness, drowsiness, trism, weakness;
    hemopoiesis: methemoglobinemia;
    cardiovascular system: increase or decrease of blood pressure, bradycardia, peripheral vasodilation, arrhythmias, collapse, pain in the chest;
    allergy symptoms: a rash on the skin, itching, other anaphylactic manifestations, hives.
In case of development of the above-described negative manifestations or other side effects, you should immediately inform the treating specialist about it.

Instructions for use Novocaine (Method and dosage)

Novocaine 0.5% is used for infiltration anesthesia in a dose of 350-600 mg. Adults at the beginning of surgery are given a dose of not more than 0.75 g (150 ml), then, during each hour of surgery - no more than 2 g (400 ml) of the solution.
The implementation of the paranephalic blockade involves the introduction of 50-80 ml of the solution into the peritoneal cellulose.
Carrying out the circular and paravertebral blockade involves the administration of 5-10 ml of solution intradermally. In the case of vagosympathetic blockade, 30-40 ml should be administered.
In order to reduce absorption and lengthen the effect during local anesthesia, an additional solution of epinephrine hydrochloride 0.1% is added at a rate of 1 drop to 2-5-10 ml of the procaine solution.
When used in adolescents after 12 years, the highest dose is 15 mg per 1 kg of body weight.

Overdose

When the drug is overdosed, the patient may display pallor of the mucous membranes and skin, nausea, dizziness, vomiting, the appearance of "cold" sweat, tachycardia, increased respiration, lowering blood pressure down to collapse, methemoglobinemia, apnea. LS affects the nervous system, which is manifested by a sense of fear, convulsions, hallucinations, motor excitement.
In case of an overdose it is necessary to maintain adequate pulmonary ventilation, to carry out symptomatic and detoxification treatment.

Interaction

Novocain potentiates the effect on the CNS of drugs used for general anesthesia, sedatives and hypnotics, tranquilizers and narcotic analgesics.
With the simultaneous administration of anticoagulants, the likelihood of bleeding increases.
If the injection site has been treated with disinfection solutions containing heavy metals, the risk of edema and tenderness as a local reaction increases.
The use of Novocaine simultaneously with monoamine oxidase inhibitors increases the likelihood of a sharp drop in blood pressure.
Novocain prolongs and increases the effect of muscle relaxant drugs.
Local anesthetic effect is prolonged by vasoconstrictors (phenylephrine, epinephrine, methoxamine).
Under the influence of procaine, the antimiasthenic effect of the agents decreases. Therefore, an additional correction of myasthenia gravis therapy may be required.
Paraaminobenzoic acid (procaine metabolite) is an antagonist of sulfonamides.
With the simultaneous administration of cholinesterase inhibitors, the metabolism of local anesthetics is reduced.

Storage conditions

Store Novocaine at room temperature, in a dry and dark place, protect from children.

Shelf life

Can be stored for 3 years, after the expiry date can not be applied.

special instructions

Before using the drug, you need to conduct an individual test for sensitivity to the drug.
In the process of treatment, it is necessary to monitor the functions of the vessels, heart, central nervous system, respiratory system.
10 days before the introduction of anesthetic locally, it is necessary to cancel the intake of monoamine oxidase inhibitors.
It is important to note that when using the same dose of novocaine, the toxicity of procaine is higher if the solution is more concentrated.
Since the procaine penetrates poorly through intact mucous membranes, it is not effective for carrying out superficial anesthesia.
During treatment, care must be taken when driving a vehicle, as well as in other activities that require concentration.
Electrophoresis with Novocaine in osteochondrosis is carried out after diagnosis and under the supervision of the attending physician.
Do not drip Novocain's solution into the eye without coordinating such actions with your doctor.

Children

The medicine is not used for children under 12 years old. Adolescents from 12 to 18 years of age are used with caution.

Novocain in pregnancy and lactation

If there is a need to use Novocain in pregnancy, experts determine the intended benefit and possible risk. In the period of childbirth apply with caution. If you need to use Novocaine during breastfeeding, lactation should be discontinued.

Reviews
