pharmachologic effect
The preparation of iron replenishing its deficiency in the body.
Pharmacodynamics and pharmacokinetics
Iron - a very important and necessary for the body element. It is a part of hemoglobin and myoglobin, it performs a number of important functions in the metabolism process. Deficiency of iron leads to anemia.
Due to the fact that the polymetal complex is more stable and practically does not enter the oxidation reaction, iron is not released into the digestive tract as Fe3 + ions. Its structure is most similar to the natural structural element - ferritin. Due to this, the frequency and the number of possible side effects from taking the drug is significantly reduced.
Reduces the degree of susceptibility to oxidation of low and very low density lipoproteins.
Getting into the systemic bloodstream, by sucking into the intestine (in the duodenum or small intestine) or by direct injection (injection solution), iron passes through the reticuloendothelial system and accumulates in the liver. And then, in the bone marrow, it participates in the processes of formation of hemoglobin, myoglobin and other elements.
Studies were carried out using the technique of double isotopes, which showed that the degree of assimilation of iron is selective. If a sufficient amount of substance is present in the body, then the degree of absorption will be extremely low, in comparison with the state of iron deficiency. The stronger the body lacks iron, the more it absorbs.
The product has a low toxicity. Unused iron is excreted with feces, dead cells of the epithelium of the gastrointestinal tract and skin, during breathing, with bile and urine. It should be remembered that in women there is a large loss of iron during menstruation.
If it is not possible to use the drug in tablets or another form, an injectable solution is used. When intravenously administered one day after the injection, the drug reaches the maximum concentration in the plasma.
Indications for use
The remedy is used:
for the treatment and prevention of iron deficiency anemia, clearly not showing a lack of iron;
to prevent people on a strict diet, the elderly, pregnant women, children and adolescents during periods of active growth.
Injection of the drug can be used only after confirming the lack of iron by examining the blood composition (hemoglobin level in the blood, red blood cells, hematocrit, serum ferritin).
Contraindications
The drug is contraindicated:
in the presence of hypersensitivity reactions;
bone marrow hypoplasia;
with chronic polyarthritis;
hemochromatosis, hemosiderosis;
the Osler-Randu-Weber Syndrome;
when erythropoiesis is disturbed;
with sideroahrestic anemia, lead anemia, porphyria of the skin, thalassemia;
with uncompensated liver cirrhosis and kidney disease in acute, late stages;
with stenosis or other diseases of the digestive tract;
with regular blood transfusions.
In the presence of diseases of the gastrointestinal tract, a solution for intramuscular injections is often used. It can not be used for intravenous administration, in the first trimester of pregnancy, for children up to 4 months. Otherwise, contraindications remain the same as for oral forms.
Side effects
Adverse reactions for oral forms:
pain in the epigastric region, nausea, diarrhea, constipation, vomiting;
urticaria, eczema, anaphylactic reactions;
rarely, the enamel stains black.
Solution for injection, adverse reactions:
arthralgia;
fever and swollen lymph nodes;
dyspepsia;
allergic reactions;
headache.
If the injection has been done incorrectly, the skin may change color, inflammation and pain occur at the site of administration of the remedy.
Instructions for Maltofer (Method and Dosage)

for children from one year to 12 years - from 20 to 40 drops per day;
adults, pregnant women and children over 12 take 40-120 drops a day.

Children up to a year are prescribed 6 to 10 drops per day;
children from one year to 12 years - 10-20 drops;
adults and pregnant women from 20 to 40 drops per day.
Instructions for syrup

According to the instructions for tablets, Maltofer Viefort, daily dosage for adults with iron deficiency anemia - 1-3 pieces per day. For the prevention or treatment of latent iron deficiency, appoint one tablet a day. Pregnant women take 2-3 tablets a day as part of the treatment.
Solution for oral administration, instruction
In order to normalize the level of hemoglobin in the blood (for adults) one bottle of the drug is drunk 1 or 3 times a day. The course is from 3 to 5 months, then the dosage is reduced to 1 bottle. For prophylaxis take 1 bottle a few months. For pregnant women, the dosage will be slightly higher.
Instruction for injection
The daily dosage and duration of treatment should be determined by the physician. They are calculated according to the weight of the patient according to special tables (according to the formula).
Before the first dose, a sensitivity test should be performed. Intramuscularly injected half or a quarter of a single dose. In the absence of any side effects, the remaining liquid in the syringe is injected.
Typically, for adults, the daily dosage is one ampoule of the solution. The maximum amount of medication administered during the day:
for children weighing up to 5 kg - the fourth part of the ampoule;
for children weighing from 5 to 10 kg - half the ampoule;
10-50 kg - one ampoule;
for adults - two ampoules.
If after 2 weeks of application there is no improvement in hemodynamic parameters, it is necessary to correct the treatment and revise the diagnosis.
Instead of the usual method of drug administration, it is recommended that the ventro-gluteal technique. The needle should be thin, not more than 6 cm in length. After the injection, it is recommended to walk a little or move your foot to disperse blood (with medicine) through the body.
Overdose
The likelihood of an overdose is extremely small, especially when taken orally. No cases of overdose have been reported. Clinical studies have confirmed that even with the intake of 2000 mg of iron per kg of human weight, a lethal outcome is unlikely.
The likelihood of an overdose increases with intramuscular injection of the drug. There can be an acute overload with iron, symptoms of hemosiderosis will appear.
Therapy - according to the manifested symptoms, the reception of chelates (Deferoxamine intravenously).
Interaction
Antacids, vitamin E, eggs, milk, coffee and black tea, cheeses and some cereals can complicate the process of iron absorption in the intestine. Citric acid and vitamin C, on the contrary, strengthen the process.
Plasma concentrations of penicillamine, tetracycline and sulfasalazine are reduced by the action of iron salts.
Storage conditions
Keep out of direct sunlight, keep away from light and from children. Temperature regime: up to 25 degrees.
Shelf life - 5 years. Ampoules should be used immediately after opening.
special instructions
The drug (especially the form for oral use) can stain the feces black.

Reviews about Maltofer
Reviews about tablets
The drug helps to raise low hemoglobin in a relatively short time. The medicine has, basically, positive reviews. Side effects occur rarely, most often it is diarrhea or constipation.
