pharmachologic effect

Antiallergic agent. The mechanism of action is associated with stabilization of mast cell membranes and a decrease in the release of histamine, leukotrienes and other biologically active substances from them. Suppresses the accumulation of eosinophils in the respiratory tract caused by the platelet activating factor. Prevents attacks of bronchial asthma, some other manifestations of allergic reactions of immediate type. It blocks histamine H1 receptors.

Pharmacokinetics

After oral administration, it is almost completely absorbed from the digestive tract. Simultaneous food intake does not affect the degree of absorption of ketotifen. Metabolized to 50% with the "first pass" through the liver. Cmax in blood plasma is reached within 2-4 hours. Binding to plasma proteins is 75%.
It is biphasic. T1 / 2 in the initial phase is 3-5 hours, in the final - 21 hours. It is excreted by the kidneys, 60-70% in the form of metabolites, 1% - in unchanged form.

Indications

Prevention of allergic diseases, incl. atopic bronchial asthma, allergic bronchitis, hay fever, allergic rhinitis, allergic dermatitis, hives, allergic conjunctivitis.

Contraindications

Hypersensitivity to ketotifen.

Dosage

Is taken internally. Adults - 1 mg 2 times / day (morning and evening) with meals. If necessary, the daily dose can be increased to 4 mg.
Maximum daily doses: for adults - 4 mg.
For children aged 3 years and older - 1 mg 2 times / day; at the age of 6 months to 3 years - 500 mcg 2 times / day.

Side effects

From the side of the central nervous system: drowsiness, mild dizziness, slowing of mental reactions, usually disappearing a few days after the start of treatment.
On the part of the digestive system: it is possible to increase appetite; rarely - dyspeptic symptoms, dry mouth.
From the hemopoietic system: thrombocytopenia.
From the urinary system: dysuria, cystitis.
From the side of metabolism: increase in body weight.

Drug Interactions

Ketotifen can enhance the effects of sedatives, hypnotics, antihistamines and ethanol.
At reception of a ketotifenum simultaneously with oral hypoglycemic preparations reversible decrease in quantity of thrombocytes was marked.

special instructions

The therapeutic effect of ketotifen develops slowly, within 1-2 months.
The anti-asthma therapy should be continued for at least 2 weeks after the onset of ketotifen.
With the simultaneous use of ketotifen and bronchodilators, the dose of the latter can sometimes be reduced.
Impact on the ability to drive vehicles and manage mechanisms
Patients taking ketotifen should refrain from potentially dangerous activities associated with the need for increased attention and rapid psychomotor reactions.

Pregnancy and lactemia

In pregnancy, especially in the first trimester, use is possible only if the potential benefit to the mother exceeds the possible risk to the fetus.
Ketotifen penetrates into breast milk, therefore, if it is necessary to use it during lactation, the question of stopping breastfeeding should be resolved.
