Composition

The composition of the Enam tablets varies depending on the content of the main drug component of enalapril maleate. The preparation may contain 2.5, 5, 10, 20 mg of this drug compound.
In addition, the following auxiliary compounds are present in the drug: zinc stearate, maleic acid and anhydrous lactose (for Enam 5 mg / 2.5 mg) and the same components, except for maleic acid, in a preparation containing 10 mg and 20 mg maleate enalapril.

Form of issue

Enam is produced in the form of tablets with chamfered edges of white color, marked with EMT marking on one side, and on the other - risk dividing and digits 2,5, 5, 10, 20, which corresponds to the dosage of the active ingredient in milligrams.
Tablets are packaged in specialized aluminum strips in each of 10 pieces, and then packed into cardboard boxes of 2 strips each.

As noted earlier, Enam belongs to the group of antihypertensive or so-called antihypertensive drugs, the action of which is aimed at inhibiting the angiotensin-converting enzyme. Thus, this drug reduces the activity of the ACE enzyme and stimulates a decrease in the production of angiotensin II.
Pharmacists refer Enam to "prodrugs", since after hydrolysis of the drug in the human body, enalaprilate (i.e., an active drug component) is formed, which has an inhibitory effect on the ACE enzyme.
In addition, the drug has diuretic properties and helps not only reduce blood pressure, but also significantly reduce both pre- and postnagruzka with heart failure on the myocardium.
The drug stimulates the circulation of the small circle and respiratory function of the body, and also helps to normalize blood circulation in the vessels of the kidneys. The drug is rapidly absorbed, its bioavailability reaches a maximum of 74%. The following dosages of the drug are recommended:
    for patients who do not take diuretics - 5 mg per day, gradually increasing the amount of the drug to 10-40 mg per day (for 1-2 doses);
    with arterial hypotension for patients taking diuretics - an initial dose of 2.5 mg provided that it is not possible to cancel diuretic therapy;
    with heart failure of a chronic type - 2.5 mg per day, then 2.5 mg twice daily for a maximum of 4 days, gradually increasing the dosage to 10 mg per day;
    for patients suffering from diabetes and, as a consequence, nephropathy and hypertension - from 2.5 to 5 mg per day, provided that blood pressure is normal, when symptoms of hypertension appear, up to a maximum of 40 mg of the drug per day;
    for patients suffering from kidney failure - a maximum of 2.5 mg per day with a creatine level below 30 ml / min.

Overdose

In case of an overdose of Enam, symptoms of arterial hypotension are observed in patients.

Interaction

With the simultaneous administration of the drug with NSAIDs (non-steroidal anti-inflammatory drugs), its hypotensive effect is reduced. Hyperkalemia may occur with Enam and potassium-sparing diuretics such as Amyloride, Spironolactone, and Triamteren.
When the drug is prescribed together with drugs containing lithium in its chemical composition, the excretion of the latter is slowed down, which requires constant monitoring of the level of lithium in the patient's blood.
Since alcohol increases the antihypertensive properties of the drug during treatment it is forbidden to drink alcohol, the same effect is exerted by: diuretics, calcium channel blockers, beta-blockers, prazosin, nitrates, hydralazine.
The effectiveness of the Enam is reduced when it is used together with painkillers or antipyretic drugs. The enalapril contained in the drug reduces the effectiveness of theophylline.
