Composition

Depending on the form of release in the chemical composition of a single Nipperen tablets (INN Bisoprolol) may contain 2.5 mg. 5 mg. or 10 mg. active bisoprolol fumarate drug.
Povidone, magnesium stearate, carboxymethyl starch, as well as MCC and colloidal silicon dioxide (anhydrous) are part of the Nipperen as adjuvants.
Titanium dioxide, macrogol and talc are contained in the film membrane of Niperten tablets.

Form of issue

The drug is released in the form of biconvex tablets, nominal volume of 2.5 mg., 5 mg. and 10 mg., coated with a coat of light white color, on top of which, on the one hand, there is a risk (on the one hand).

pharmachologic effect

Niperten has antihypertensive, beta-adrenoblokiruyuschim, antianginal, as well as antiarrhythmic pharmacological action.

Pharmacodynamics and pharmacokinetics

In its essence, Niperten is a beta1-adrenoblocker belonging to selective compounds, it does not possess membrane-stabilizing properties without its own sympathomimetic activity. This medicine helps to reduce the level of renin in the blood, and also reduces the need for myocardium in oxygen.
The drug has a pronounced antiarrhythmic, antihypertensive and anti-anginal effect on the body. Due to its distinctive properties, Niperten can affect the intensity of the ionic calcium current at the intracellular level, and also blocks beta 1-adrenergic receptors formed in small doses in the heart, thereby slowing down the formation of cAMP (cyclic adenosine monophosphate) from ATP.
The antiarrhythmic effect of NIPERTEN is due to the ability of the drug to increase the amount of cyclic adenosine monophosphate, as well as to eliminate such arrhythmogenic factors as tachycardia, hypertension, and increased activity of the nervous system. Under the influence of Niperten, the demand for myocardium in oxygen becomes less as a result of reduced contractility of the main cardiac muscle, due to which the antianginal effect of the drug is achieved.
The drug compound achieves an antihypertensive effect due to a reduction in blood volume (per 1 minute), restoration of sensitivity (the result of influence on the central nervous system and lowering blood pressure), and also by stimulation of peripheral blood vessels. Under the influence of active α-adrenoreceptors, as well as with a decrease in stimulation of β2-adrenoreceptors in the first 24 hours after taking the drug, OPSS (total peripheral vascular resistance) increases.
However, after 3 days, the indicators return to normal, and with prolonged therapeutic treatment - go down. It is noted that, in comparison with similar drugs, Niperten has a less pronounced effect on carbohydrate metabolism, as well as such organs containing β2-adrenergic receptors in their tissues, such as: bronchi, pancreas, peripheral arteries, uterus.
The drug compound is absorbed by 80-90%. About half the dose of the drug is metabolized with the involvement of the liver for 12 hours. In the future, the kidneys remove about 98% of the active compound from the body.
