Pharmacodynamics

Selectively stimulates imidazoline I1 receptors of the medulla oblongata, which leads to a decrease in the tone of sympathetic innervation and, as a result, a decrease in OPSS, heart rate and heart rate (to a lesser extent). At the same time, the volume of cardiac output practically does not change. Effectively lowers blood pressure, reduces the level of aldosterone and norepinephrine, angiotensin II and renin. Prevents development of myocardial hypertrophy. Has a hypoglycemic effect, which is associated with the excitation of imidazoline receptors of the pancreas.
The cardioprotective action was established hypolipidemic and experimentally proved. Stimulates the production of growth hormone. The hypotensive effect lasts 12 hours. Addiction and withdrawal symptoms are not noted.

Pharmacokinetics

The drug is taken internally. Treatment begins with a dose of 200 mcg in one session, in the morning. If the effect is insufficient, after 3 weeks the dose is increased to 400 mcg in one dose in the morning, or divided into two doses (morning-evening). The maximum SD is 600 μg divided into 2 doses.
Elderly people with preserved kidney function do not adjust the dose. With renal failure, a single dose of 200 μg, and a daily dose of 400 μg.
The use of Tenzotran under the tongue is practiced with hypertensive crises. For this, the 0.4 mg tablet must be ground, the powder poured out under the tongue and after 30 -50 minutes the blood pressure gradually decreases by 15% -20%.

Overdose

It is manifested by headache, drowsiness, dizziness, excessive sedation, pronounced decrease in blood pressure, bradycardia, dryness of the oral mucosa, vomiting.
There is no specific antidote. The stomach is washed, sorbents and laxatives are taken. It is recommended the administration of dopamine with a sharp decrease in blood pressure and atropine with bradycardia.

Interaction

Moxonidine is prescribed with calcium channel blockers and thiazide diuretics - their combined use increases the hypotensive effect.
Strengthens the effect of tricyclic antidepressants, sedatives and hypnotics, tranquilizers. Potentiates the effect of ethanol. Tricyclic antidepressants, in turn, reduce the effect of central antihypertensive agents.
Improves cognitive function in persons taking lorazepam.
Since moxonidine is excreted by the kidneys, it is possible for it to interact with drugs released in the same way.
