Pharmacological properties
Ribavirin triphosphate (RTF) is a potent inhibitor of inosine monophosphate dehydrogenase (IMP dehydrogenase), viral RNA polymerase and viral mRNA-guanylyltransferase. Inhibition of the latter stops capping mRNA, which leads to a significant depletion of intracellular reserves of guanosine triphosphate and inhibition of the synthesis of viral RNA and protein. Ribavirin is also inserted into the viral genome, causing lethal mutations, with a subsequent decrease in the pathogenicity of the virus. Ribavirin inhibits the replication of new virions, which reduces viral load, selectively inhibits the synthesis of viral RNA, without suppressing the synthesis of RNA in normally functioning cells. DNA viruses most sensitive to ribavirin are herpes simplex virus, adenoviruses, CMV, smallpox viruses, Marek's disease; RNA viruses - influenza A, B viruses, paramyxoviruses (parainfluenza, mumps, peptochemic syndrome, pepticitis); (hemorrhagic fever virus with renal or pulmonary syndrome), oncogenic RNA viruses.
Ribavirin-insensitive DNA viruses: Varicella zoster, pseudo-rabies virus, cowpox; RNA viruses: enteroviruses, rhinoviruses, encephalitis virus of the forest Semliki.
Pharmacokinetics
When applied to the skin and locally, systemic absorption is minimal and practically insignificant.
Indications for use
The cream can be used both as a monotherapy and in combination with other dosage forms of ribavirin, intended for parenteral and oral administration in the following diseases:
1. Infections of the skin and mucous membranes caused by Herpes simplex type 1 and 2 viruses of various localization, including in the genital area.
2. Herpes zoster (as part of complex therapy).
Contraindications for Devirs
Hypersensitivity to ribavirin or other components of Devirs, pregnancy, lactation, childhood and adolescence (up to 18 years).
Carefully
Renal failure. Use with caution in women and men of reproductive age.
Application in pregnancy and lactation
The use of cream during pregnancy and lactation is contraindicated.
Dosing and Administration
Externally, apply to the affected skin and mucous membranes (previously washed and dried) in a uniform thin layer, slightly rubbing.
Apply 5 times a day until the symptoms disappear completely.
In the absence of improvement within 8 days of Devirs use, stop taking it and consult a doctor.
To obtain the best effect, treatment should be started as early as possible at the first appearance of the symptoms of the disease.
If Devirs cream is prescribed in combination with one of the dosage forms of ribavirin for oral or intravenous administration, the dose of the cream should be reduced.
Side effects of Devirs
Allergic reactions (skin rash, urticaria, rash) are possible.
Other: hair loss, changes in the content of thyroid-stimulating hormone (TSH), symptoms of autoimmune diseases (extremely rare).
The observed effects are usually reversible, do not affect the effectiveness of treatment and stop after the end of treatment.

Special instructions
Do not use to treat ophthalmoherpes, do not apply in the eye area.
Devirs should be used with extreme caution in patients with renal insufficiency.
