pharmachologic effect

Antifungal drug.

Pharmacodynamics and pharmacokinetics

The active substance is a highly selective inhibitor of the synthesis of sterol derivatives inside fungal cells - fluconazole. Diflucan is effective against yeast fungi, against yeast-like fungi, and against mold fungi. Diflucan is excellently absorbed in the digestive tract and showed high bioavailability when taken orally: the concentration of the drug in the blood when taken in tablet form was more than 91% of the level of the substance in the plasma with intravenous administration. Food intake does not affect the bioavailability of Diflucan in any way, and the maximum concentration in the blood plasma is reached after 30-90 minutes. The half-life is 30 hours, the drug lasts a day. Diflucan well penetrates the skin, organs, tissues, including the spinal fluid, the secret of the mucous membranes and saliva. High concentrations of fluconazole (higher than in blood plasma) are noted in the skin (in the stratum corneum), in the sweat glands. Unchanged from the body is excreted by the kidneys.

Indications for use

The drug is prescribed for fungal lesions of the genitalia in women known as vaginal candidiasis. The drug is effective in all kinds of fungal infections caused by Candida fungi (candidiasis). This pathology can be caused by immunosuppressive therapy, prolonged use of antibiotics, immunodeficiency, including in HIV infection. It is possible to use Diflucan for preventive purposes. Other indications for the prescription of the drug Diflucan are cryptococcosis (including cryptococcal meningitis), mycosis of the skin, fungal infection of the nail plate (onychomycosis). Diflucan is also used for men.

Contraindications

Diflucan is not prescribed with the simultaneous use of terfenadine, cisapride. No tests have been performed and there are no data on the safety of fluconazole during pregnancy and lactation, and although this contraindication should be considered relative, Diflucan should be given to pregnant women with caution. Intolerance to fluconazole is also one of the contraindications for prescribing the drug.

Side effects

In the vast majority of cases, taking Diflucan is not accompanied by side effects, however, violations from the internal organs are possible.
Nervous system: headaches accompanied by vertigo can occur, increased muscular excitability and convulsive syndrome are possible. Skin can respond to the reception of Diflucan by allergic reactions, possibly even the development of complications such as malignant erythema (a complication is called Stephen-Johnson syndrome), which is manifested by the appearance of painful blisters on the mucous membranes and on the skin. Cases of such a formidable complication as Lyell's syndrome are noted: bullous elements appear on the skin, in which necrotic processes develop.
Digestive tract: pain in epigastrium, changes in laboratory parameters of liver function, attachment of toxic hepatitis, digestive disorders, vomiting, attacks of nausea.
From the heart can be changes in conductivity, which are reflected in the cardiogram. In blood, there may be a decrease in platelet count, agranulocytosis and lymphocytopenia. Allergic reactions can reach the maximum degree of severity (anaphylactic shock).

Instructions for the use of Diflucan (Method and dosage)

The medication issued in the form of capsules has a dosage of 150 mg, 100 mg and 50 mg. In solutions for intravenous administration - 100 mg and 200 mg in bottles of 50 ml and 100 ml. The dose of 150 mg is effective daily. It is appropriate to use a suspension for children. To treat the majority of fungal lesions, it is sufficient to take 150 mg of Diflucan once. So for vaginal candidiasis, women only need to take one tablet of the drug once, and for a chronic process, the drug should be taken in the same dosage at a frequency of once a month. To prevent this condition, Diflucan is indicated in the same dosage. The duration of treatment is determined by the doctor depending on the severity of the disease. With a number of fungal infections, the dosage of the drug can be increased to 400 mg, and the course of treatment is prolonged to four weeks.
How to take the drug before or after eating? In general, the use of the drug is not dependent on food intake.
This medicine does not exist in the form of candles.

Instructions for use Diflucan for thrush

How to take Diflucan with thrush? It is enough to take the drug once in order to defeat the disease. Improvement is noted already a day after receiving the remedy. During the week, the symptoms should completely disappear, otherwise you should consult a doctor. When relapsing thrush is advised to use one capsule every month for six months.

Overdose

There were cases of drug overdose, in which there were hallucinations and paranoid behavior. In this situation, treatment with symptoms is applied, and it is also recommended to do a gastric lavage. It should be remembered that the drug is excreted in the urine, so the process of stimulating urination in this case is also a priority.

Interaction

The drug in combination with oral contraceptives increases the content of the latter in the blood. Rifampicin helps to speed up the removal of Diflucan from the blood plasma, which requires correction of the dosage of fluconazole upward. In turn, Diflucan raises the content of Teofillin in the blood, which requires a reduction in the dosage of the latter with simultaneous admission. Terfenadine and Diflucan can affect the electrical activity of the heart, the cardiogram changes, the heart rhythm is disturbed. A lethal outcome is possible, because the simultaneous use of these drugs is possible only when the dosage of Diflucane does not exceed 400 mg. All of the above about Terfenadine is also true of Cisapride. Diflucan can change the blood content of Cyclosporine, therefore, with simultaneous admission, it is recommended to perform a one-time control study of the level of the latter in blood plasma. Diflucan increases the concentration of benzodiazepine, which leads to the appearance of psychomotor reactions. Combined use of Diflucan and drugs that affect the time of blood clotting can be fraught with increased coagulation time and bleeding. Hydrochlorothiazide reduces the excretion of Diflukane and increases its concentration in the blood by 40%.

Storage conditions

In a place inaccessible to children at a temperature not exceeding 30 degrees Celsius.

Shelf life

Not more than 3 years, and no more than 14 days for the prepared suspension.

special instructions

There have been cases of development of severe toxic hepatitis against the background of Diflucan, therefore during therapy it is necessary to carefully monitor the level of hepatic enzymes. This is especially true for patients who have a history of liver disease, and if the function of liver cells is impaired. Such patients are strictly forbidden to engage in self-medication and take Diflucan without prior consultation with a specialist. Toxic hepatitis caused by Diflucane may result in death. Necrotic skin changes (Lyell's syndrome), as a rule, occurred with concomitant severe pathology of the immune system (HIV). Patients with cardiovascular diseases and concomitant electrolyte imbalance should take the drug with caution, since the risk of arrhythmia is high.
The medicine has an official site Diflucan.
It is necessary to distinguish this drug from Dufalac, which is a laxative.

For children

The dose should be prescribed by the doctor depending on the nature of the disease. The daily dose for children should not exceed 400 mg of the active substance. Usually they take medicine once a day.

For newborns

In neonates, the active substance of the drug is slowly excreted. The dose should be chosen by the doctor. However, usually the drug is applied every 72 hours in the first 2 weeks after the birth. At 3-4 weeks of life, the interval between taking the drug should be 48 hours.

Compatibility with alcohol

It is not recommended to drink together with alcohol, it can be dangerous for life. Interaction can not be predicted.

Diflucan in pregnancy

During pregnancy and breastfeeding, Diflucan is prescribed only for life indications, since there is no evidence that the drug is safe and does not have a teratogenic effect on the fetus.
