Indications for use

Treatment of inflammatory dermatosis with concomitant bacterial and mycotic infection or with a high probability of attaching a secondary infection (simple and allergic dermatitis, atopic dermatitis (including diffuse neurodermatitis), limited neurodermatitis, eczema, dermatomycosis (dermatophytosis, candidiasis, multiple color lichen)) , especially when localized in the groin and large folds of the skin; simple chronic lichen (limited neurodermatitis).

Active ingredient, group:

Gentamicin + Dexpanthenol + Mometasone + Econazole, Glucocorticosteroid for local use + antibiotic-aminoglycoside + antifungal agent + tissue repair stimulator
Dosage Form - cream for external use

Contraindications

Hypersensitivity to dexpanthenol, econazole, gentamicin, mometasone, or to any of the components that make up Tetraderm;
- skin post-vaccination reactions;
- skin tuberculosis;
- skin manifestations of syphilis;
- chickenpox;
- herpes simplex;
- application in the field of open wounds;
- rosacea;
- perioral dermatitis;
- children's age up to 18 years (efficiency and safety are not established).

Use during pregnancy and during breastfeeding

Controlled studies on the use of tetraderm during pregnancy have not been conducted. Use during pregnancy is possible only in cases where the intended benefit to the mother outweighs the potential risk to the fetus. During pregnancy, Tetraderm should not be used on large areas of the skin or for a long period of time.
GCS is excreted in breast milk. In this regard, you should stop breastfeeding or use Tetraderm nursing mother.

Dosage and administration (instructions)

Outwardly.
The cream is applied to the affected skin with a fake layer, gently rubbing, 2 times a day to achieve a positive clinical result.
The duration of treatment is individual, depends on the size, location of the lesion and the severity of the disease and is usually 1-2 weeks. In case of ringworm, the average duration of treatment is 2-4 weeks. Tetraderm is not recommended for more than 4 weeks.
If there is no improvement after treatment or new symptoms appear, it is necessary to consult a doctor. Use Tetraderm only according to the method of use and in those doses that are indicated in the instructions. If necessary, please consult a physician before using the drug.

Pharmachologic effect of Tetraderm

Combined preparation for external use.
It has anti-inflammatory, antipruritic, antiexudative, antibacterial, antifungal (fungicidal) and regenerating effects. The activity of Tetraderm due to the pharmacological properties of the components included in its composition.

Gentamicin

A broad-spectrum antibiotic from the group of aminoglycosides. Bactericidal effect and provides highly effective external treatment of primary and secondary bacterial infections of the skin. Active against gram-negative microorganisms: Pseudomonas aeruginosa, Aerobacter aerogenes, Escherichia coli. Proteus vulgaris, Klebsiella pneumoniae; Gram-positive microorganisms: Staphylococcus aureus (coagulase-positive, coagulase-negative and some strains producing penicillinase).

Dexpantepol

Dexpantepol is a derivative of pantothenic acid. Pantothenic acid is a water-soluble vitamin B (vitamin B5) - is an integral part of coenzyme A. Stimulates skin regeneration, normalizes cellular metabolism, accelerates mitosis and increases the strength of collagen fibers. An increase in the demand for pantothenic acid is observed when the skin or tissues are damaged, and its deficiency can be compensated for by external use of dexpangenol. It penetrates all layers of the skin. It has a regenerating, weak anti-inflammatory effect.

Mometasone

Synthetic glucocorticosteroid (GCS), has a local anti-inflammatory, antipruritic and antiexudative action. GCS induces the release of proteins that inhibit phospholipase A2 and are commonly known as lipocortins, which control the biosynthesis of inflammatory mediators such as prostaglandipa and leukotrienes by inhibiting the release of their common precursor arachidonic acid.

Econazole

Econazole is a synthetic imidazole derivative. It has antifungal and antibacterial action. It inhibits the biosynthesis of ergosterol, which regulates the permeability of the cell wall of microorganisms. It is easily dissolved in lipids and penetrates well into tissues. Active against dermatophytes Trichophyton, Microsporum, Epidermophyton, yeast fungi of the genus Candida, Corynebacterium minutissimum, and Malassezia furfur (Pityrosporum orbiculare), causing pityriasis versicolor, and some gram-positive bacteria (Streptococcus, Staphylococcus).

Side effects

From the skin:
rarely - skin irritation, dry skin, burning sensation of the skin, itching, acne, skin atrophy (thinning and loss of elasticity), hypertrichosis (excessive hair growth), hypopigmentation, perioral dermatitis (rash or irritation around the mouth), allergic contact dermatitis, maceration skin (softening of the upper layers of the skin), striae (stretch marks), prickly heat (skin irritation that develops due to increased perspiration), the formation of papules and / or pustules (rash); frequency is unknown - skin hyperemia (redness), urticaria (blistering).
On the part of the system: the frequency is unknown - paresthesia (tingling and numbness).
Other: rarely - folliculitis (inflammation of the hair follicle), secondary infection.
When using external forms of corticosteroids for a long time and / or when used on large areas of the skin, or using occlusive dressings, side effects may occur that are characteristic of systemic corticosteroids, including adrenal insufficiency and Itsenko-Cushing syndrome.

Overdose

Symptoms With prolonged use of local corticosteroids in high doses, it is possible to suppress the function of the adrenal glands with the development of secondary adrenal insufficiency and symptoms of hypercorticism, including Itsenko-Cushing syndrome.
A single overdose of gentamicin is not accompanied by the appearance of any symptoms. Prolonged use or use in doses exceeding the recommended, can lead to a significant increase in insensitive microflora, including fungal, in the lesion.
Overdose of econazole when applied topically does not lead to the appearance of any symptoms.
With an overdose of dexpanthenol, when externally administered, no symptoms are expected.
Treatment. Symptomatic. The acute symptoms of hypercorticism are usually reversible. If necessary, correction of electrolyte imbalance is shown.
If uncontrolled growth of insensitive microorganisms or the development of a fungal infection is uncontrolled, treatment should be stopped and appropriate therapy should be selected.

Special instructions for Tetraderm

Not intended for use in ophthalmology. The drug should not be allowed to enter the eyes and the periorbital area.
Tetraderm cream is not recommended for use on the face and scalp. It is not recommended to use Tetraderm under occlusive dressings, except when necessary.
It should not be used to treat varicose trophic ulcers of the leg and open wounds. Some areas of the body (inguinal folds, armpits and the perianal region) are more at risk for striae, so the duration of the drug in these areas of the body should be limited.
If irritation or signs of hypersensitivity are noted when using Tetraderm, treatment should be stopped and the patient should be treated with another therapy.
Any side effects that occur with the use of systemic corticosteroids, including suppression of the function of the adrenal cortex, can also be observed when external corticosteroids are used. Systemic absorption of local corticosteroids may increase with prolonged use, treatment of large areas of the body, or the use of occlusive dressings. In such cases, the possible development of side effects characteristic of systemic corticosteroids.
It should be borne in mind that GCS can alter the manifestations of certain skin diseases, which may make diagnosis difficult. In addition, the use of corticosteroids may cause delayed wound healing.
With long-term GCS therapy, the sudden cessation of therapy can lead to the development of rebound syndrome, manifested in the form of dermatitis with intense reddening of the skin and a burning sensation. Therefore, after a long course of treatment, the abolition of tetraderm should be made gradually.
Systemic absorption of gentamicin when applied topically may increase if applied to large areas of skin, especially with prolonged treatment or in the presence of skin lesions. With long-term use of Tetraderm on the vast surfaces of the skin with impaired integrity, there is the potential for the absorption of gentamicin and, accordingly, the development of ototoxicity symptoms and other undesirable effects characteristic of gentamicin in its systemic use.
Perhaps the development of cross-allergic reactions to antibiotics from the group of aminoglycosides.
With prolonged external use of gentamicin, growth of insensitive microflora, including fungal, can be observed. In this case, as with the development of irritation, hypersensitivity reactions and superinfection, the treatment must be stopped and appropriate therapy prescribed.

Interaction

Studies of the interaction of tetraderm with other drugs have not been conducted, but it is not recommended to be used simultaneously with other drugs for external use.
