Form of issue

The drug is available in the form of a dosed aerosol for inhalation, solution. Aerosol is a colorless transparent liquid in which there are no visible particles. Produced in 10 ml vials (contains 200 doses), as well as 15 ml (contains 300 doses). In one dose of the drug - 20 μg of active substance. The drug is contained in balloons of steel, equipped with a metering valve and mouthpiece.
Solution for inhalation Atrovent - a colorless transparent liquid, in 1 dose of which contains 250 μg of active substance. Packaged solution in bottles of dark glass to 20 ml.

pharmachologic effect

The active substance of the drug is ipratropium bromide, a blocker of m-cholinergic receptors. The molecule of this substance resembles acetylcholine. When it enters the tracheobronchial tree, it binds to the acetylcholine receptors.
Under the influence of Atrovent there is a blocking of acetylcholine receptors in the muscles of the bronchi. At the same time, there is almost no undesirable anticholinergic effect on the smooth musculature of the urinary tract, digestive tract, etc.
As a consequence, there is a bronchodilator effect, the level of secretion of the glands of the bronchial mucosa decreases. With the use of the device, it is possible to prevent reflex spasms of the bronchi that result from the influence of those factors that provoke the development of bronchospasm (cool air, cigarette smoke, etc.).
Atrovent also prevents the development of bronchospasm, which develops due to the pathological effect of the vagus nerve. In the treatment of these drugs in people suffering from bronchial asthma, significantly improves external breathing. The manifestation of the effect of bronchodilation is noted about 10 minutes after the inhalation and lasts for six hours.

Pharmacodynamics and pharmacokinetics

Atrovent provides a pronounced bronchodilator effect and does not allow manifestation of bronchospasm. Under its influence the secretion of the glands of bronchial mucosa decreases.
If the drug is injected into the body through inhalation, the absorption level is very low. The level of concentration of the active substance in the blood plasma is very low, so it can be determined only if large doses of ipratropium bromide are taken.
It is excreted mainly through the intestine, while 25% is released unchanged, the remains of the substance are released as metabolites.

Instructions for use of Atrovent (Method and dosage)

If the patient applies Atrovent, the instructions for use should be carefully studied and observed.
As a rule, the dosage of the drug is set individually. Instructions for the use of Atrovent for inhalation provides that the drug for adults and children who are already six years old, appoint two inhalations, 4 times a day.

In the complex treatment of bronchial asthma, children under 12 years of age are prescribed 1-2 inhalations every six hours, if necessary.
For the treatment of COPD, Atrovent can be used as a solution for inhalation. The inhaler is filled with 10-20 drops of the drug, inhalations are carried out as needed. In this case, adults should not receive more than 2 mg of the drug per day, children -1 mg of the drug.
The necessary dose of the drug is put into the nebulizer, which was previously diluted with fiz. solution. The result should be 4 ml of solution. The dose should be prepared each time before the inhalation.

Overdose

No specific symptoms were recorded with an overdose. The intensity of side effects can be intensified by applying too much of the dose, in particular, there may be a manifestation of tachycardia, dry mouth, paresis. Symptomatic therapy should be given.

Interaction

If Atrovent is prescribed concomitantly with cromoglycic acid, the effectiveness of both agents may be reduced. With simultaneous treatment with xanthine derivatives, β2-adrenoreceptor agonists, a more pronounced bronchodilator effect is observed.
Anticholinergic effect of Atrovent increases with simultaneous therapy with antiparkinsonian drugs, tricyclic antidepressants, quinidine.
Cisapride reduces the severity of Atrovent's influence. When joint therapy with Atrovent and β2-adrenoreceptor agonists, the condition may worsen in patients with an angle-closure glaucoma.
You can not practice aerosol treatment in parallel with disodium cromoglicate, since two aerosols can interact physically.

A remedy for the treatment of people suffering from occlusive glaucoma and disorders of the process of urination manifested in connection with benign prostatic hyperplasia is used cautiously.
If the solution accidentally gets into the eyes of a patient with an angle-closure glaucoma, he may have an increase in intraocular pressure. If a person observes a feeling of discomfort or pain in the eyes, blurred vision, the appearance of spots before the eyes, this may be signs of narrow-angle glaucoma. In this case, it is necessary to use drops that narrow the pupil, and consult an ophthalmologist without delay.
If the condition worsens during treatment or if there are no signs of improvement, you should consult a doctor and determine a plan for further therapy. With a sudden increase in dyspnea, you should immediately consult a specialist.

Which is better: Atrovent or Berodual?

Both these drugs belong to the same group of drugs with a bronchodilator effect. However, Berodual is a complex agent, which contains the substances ipratropium bromide and fenoterol hydrobromide. Atrovent has a relatively slow therapeutic effect on the body, so it does not apply to the relief of severe attacks. Berodual in view of a combination of two strong components effectively and quickly removes spasms of bronchi. But it should be noted that the drug Berodual has a broader list of side effects and contraindications. Therefore, in the initial therapy, Atrovent is often used.

Children

Atrovent can be used to treat children only after the appointment of a treating specialist and under its strict control. Aerosol is used to treat children from the age of six, the solution from a five-year old. Caution is given to children who suffer from chronic lung diseases, Down's disease, brain damage, and paralysis.

You can not use Atrovent women in the first trimester of pregnancy. At later dates, the drug may be administered if its benefit exceeds its potential risk. During the research, there was no teratogenic and toxic effect on the fetus. When lactation, Atrovent is administered with caution.
