Composition

One dose of the metered aerosol for inhalation includes 124 mg of salbutamol sulfate, which corresponds to 100 μg of pure substance. As auxiliary substances, ethanol and hydrofluoroalkane are used.
One tablet can contain 2 or 4 mg of active substance, in prolonged-action tablets (retard) - 4 mg (for children) and 8 mg (for adults).
In a single dose of powder for inhalation, 200 or 400 μg of salbutamol is contained, in oral syrup - 0.4 mg / ml, in a solution for inhalation application - 1.25 mg / ml (the solution goes to pharmacies in 2 ml ampoules, 20 ampoules in one package), in the injection solution - 0.1 mg / ml.

Form of issue

Salbutamol has the following dosage forms:
    dosed aerosol for inhalation 100 mg / dose (available in aluminum cans under pressure, 200 doses in each inhaler, sprayed onto the glass surface, the contents of the balloon leave a white spot);
    dosage powder for inhalation 200 or 400 mg / dose;
    tablets in a film coat of 2 and 4 mg.
The preparations of salbutamol produced under other trademarks can also take the form:
    retard tablets;
    solution for inhalation;
    injection solution;
    syrup;
    concentrate for the preparation of infusion solution;
    capsules filled with powder for inhalation use.

pharmachologic effect

Salbutamol is a bronchodilator, selective β2-adrenomimetic (selective β2-adrenergic receptor antagonist). Pharmacological action of the drug: tocolytic and bronchodilating.

Pharmacodynamics and pharmacokinetics

When used in a therapeutic dose, it has a pronounced bronchodilator effect, prevents and repairs bronchospastic syndrome, increases the vital capacity of the lungs, impedes the release of histamine, chemotaxis, slowly reacting substance and a number of other highly active substances into the extracellular space.
Against the background of the use of salbutamol, there are insignificant positive foreign and chronotropic effects on the cardiac muscle.
The drug promotes the expansion of the coronary arteries of the heart, the suppression of early and late bronchial reactivity, the reduction of resistance in the respiratory tract, as well as the tone and contractile activity of the myometrium, the improvement of the function of the ciliary epithelium of the bronchi, stimulates sputum and mucus production.
In this case, the agent has practically no effect on β1-adrenergic receptors, it does not provoke a decrease in blood pressure and, in comparison with other similar mechanisms of action with drugs, has a much less effect on the heart.
Salbutamol also has metabolic effects: it helps to decrease the concentration of potassium in the blood plasma, affects the process of glycogen digestion to glucose (glycogenolysis) and insulin secretion.
Individual patients (especially patients with bronchial asthma) are able to exert hyperglycemic and lipolytic effects, thereby increasing the risk of developing lactatacidotic coma.
After the administration of the inhalation forms of the drug, the effect begins to develop after 5 minutes and reaches a maximum within 30-90 minutes (about 75% of the maximum effect is achieved within 5 minutes).
From 10 to 20% of the dose received by the patient falls into the respiratory tract, the remaining 80-90% remain in the device and settle in the oropharynx, after which it is swallowed. The substance remaining in the respiratory tract is partially absorbed into the lungs and, not biotransforming in them, enters the bloodstream.
Part of the substance that has got into the digestive tract is absorbed and actively metabolized on the first passage through the liver to form phenolic sulfate.
Both the conjugate and the unmodified substance are excreted mainly in the urine.
Most of the injected into the vein, taken orally or by inhalation salbutamol, is eliminated from the body within 72 hours. The half-life period is from 3.7 to 5 hours.

Indications for use

Indications for use of Salbutamol are:
    marked with all forms of bronchial asthma bronchospasm (the drug can be used both to stop the bronchospasm syndrome, and to prevent it);
    emphysema of the lungs;
    Chronical bronchitis;
    asthmatic bronchitis;
    proceeding without complications premature birth.
In pediatrics, Salbutamol is also used to treat bronchial obstructive syndrome.

Contraindications

The use of salbutamol is contraindicated in:
    hypersensitivity to the components of the drug;
    heart rhythm disturbances (eg, in polymorphic ventricular extrasystole or paroxysmal tachycardia);
    heart defects;
    myocarditis;
    tachyarrhythmias;
    stenosis of the aortic aorta;
    ischemic heart disease;
    thyrotoxicosis;
    decompensated diabetes mellitus;
    epileptic seizures;
    glaucoma;
    pyloroduodenal narrowing;
    insufficiency of liver and / or kidney function;
    pregnancy.
The drug is not prescribed in combination with non-selective β-adrenoblockers, as well as for the treatment of children under 2 years.
With caution, it should be used for hyperthyroidism, pheochromocytoma, severe form of chronic heart failure, arterial hypertension.
Intravenous use of Salbutamol is contraindicated in:
    intrauterine fetal death;
    infections of the birth canal;
    bleeding associated with placenta previa;
    premature placental abruption;
    threatening spontaneous abortion in 1 or 2 trimesters of pregnancy;
    late toxicosis (toxicosis of the 3rd trimester of pregnancy).

Side effects

Salbutamol, released in the form of an aerosol, when used in therapeutic doses does not cause side effects.
Oral administration, while following the dosing regimen described in the manual, is accompanied by undesirable side effects rarely.
When exceeding a single or daily dose of Salbutamol, as well as in patients with increased sensitivity to stimulants of β2-adrenergic receptors, the most significant side effects may be tremor of fingers or hands, internal shaking, increased tension.
In the case of a significant excess of the therapeutic dose or a special sensitivity of the patient, transient dilatation of peripheral vessels, dizziness, headache, mild tachycardia, nausea, muscle cramps, vomiting can be noted.
Very rarely arise angioedema, manifested as a skin rash allergic reactions, urticaria, collapse, hypotension, bronchospasm.

Instructions for use Salbutamol

For patients older than 12 years, the daily dose of Salbutamol in the oral dosage forms is 6 to 16 mg. It is recommended to divide it for 3-4 reception. In cases when this is necessary, it is allowed to increase the daily dose to 32 mg with a multiplicity of applications 4 times a day.
Children from 6 to 12 years are shown Salbutamol 2 mg 3 or 4 times a day, for patients from 2 to 6 years, the optimal dose is 1-2 mg with a multiplicity of applications 3 times a day.
When inhaled, the dose depends on the prescribed dosage form. In accordance with the instruction for use, the aerosol Salbutamol is prescribed for relief of bronchospastic syndrome or an attack of bronchial asthma. Adult spray is usually recommended to enter 0.1-0.2 mg, children - 0.1 mg.
Multiplicity of procedures depends on the characteristics of the clinical situation and indications.
In order to prevent a bronchial asthma associated with physical stress before physical exertion, Salbutamol aerosol is administered in a dose of 0.1 mg, for adults - 0.2 mg. The highest daily dose is 0.8 mg (corresponding to 8 inhalations).
The drug, produced in the form of a powder, should be used in a similar way, however with a corresponding twofold increase in the dose.
The solution for inhalations is used in a dose equal to 2.5 mg 3 or 4 times a day. If it is advisable, it is permissible to increase the dose to 5 mg with the previous multiplicity of applications.

Instructions for the use of an inhaler Salbutamol

Before using the inhaler, you should check it for proper operation. This procedure is also recommended if the patient has not used it for some time.
    It is necessary to remove the cap from the inhaler and make sure that the outlet tube is not clogged with dust or dirt.
    Holding the can in the vertical position, you should place the thumb under the bottom, and the index finger on its upper part, after which it is necessary several times to shake the canister intensively up and down.
    After a deep breath (it is recommended to inhale without tension), you should throw your head up and tightly hold your mouth with the mouthpiece of the inhaler.
    Holding the tube with your lips, you should take a slow deep breath, while pressing the index finger (in the first third of the inspiration) on the valve of the inhaler and releasing the dose of the medicine. The air continues to inhale slowly.
    Taking out the tube of the canister from the mouth, it is necessary to hold your breath for 10 seconds (or as long as it is possible to do without tension), then slowly exhale through the nose.
    If it is necessary to take more than one dose of Salbutamol, after the first inhalation is held for about a minute and then repeat all the actions described above (starting with the second paragraph). After the procedure, the can should be closed with a cap.
At the time of release, do not rush the drug, to achieve the maximum effect, inhale air should be as slow as possible. Optimal before the procedure a little practice before the mirror.
If steam escapes from the top of the inhaler or from the corners of the mouth during inhalation, it is necessary to start inhalation again from the second point.

How to clean the inhaler?

Cleaning the inhaler should be done at least once a week. The can is removed from the plastic case, after which the case and the cap are washed with warm water (not hot!).
The washed parts should be thoroughly dried without the use of heating devices. After this, the can is again placed in a case and covered with a cap.
Do not douse the metal can into the water.

Overdose

Symptoms of Salbutamol overdose are:
    expansion of peripheral vessels;
    nausea and vomiting;
    hallucinations;
    increased excitability;
    flutter of the ventricles;
    tachycardia;
    decrease in blood pressure indicators;
    acidosis;
    muscular tremor;
    hypokalemia;
    hypoxemia;
    headache;
    general malaise;
    replacing hypoglycemia hyperglycemia.
Treatment implies withdrawal of the drug, appointment of cardioselective β-adrenoblockers and symptomatic therapy to the patient.
If there is a suspicion of an overdose of Salbutamol, the serum potassium concentration should be kept under constant control.

Interaction

When using Salbutamol in combination with noncardioselective β-blockers, mutual inhibition of therapeutic effects is possible. In combination with theophylline, the risk of arrhythmia and tachycardia (in particular, supraventricular extrasystole) may increase.
In combination with diuretics, glucocorticosteroids and xanthine derivatives, the risk of hypokalemia increases.

Terms of sale

Storage conditions

Store Salbutamol is recommended in a place protected from direct sunlight. The temperature should not exceed 30 ° C. Do not freeze, protect from children.
A balloon with an aerosol must be protected from bumps and falls.
Shelf life - 36 months from the date of issue.

special instructions

Increase the dose or increase the number of applications should be under the supervision of a doctor. The reduction of the interval is allowed only in extreme cases, and it must be strictly justified.
Against the background of the use of the drug, there is a risk of developing hypokalemia, therefore, in patients with severe forms of bronchial asthma, it is recommended to constantly monitor the serum potassium concentration. The risk of hypokalemia increases with hypoxia.
The World Anti-Doping Agency (WADA) prohibits the use of Salbutamol in sports, since this drug has the ability to artificially enhance endurance and physical activity during sports competitions.
The use of tablet formulations of the drug is generally recommended when inhaled drug administration is not possible (for example, in young children), since exceeding the dosage in this dosage form can trigger a cardiostimulatory effect.

Salbutamol in pregnancy

In pregnancy, Salbutamol is contraindicated. During the period of breastfeeding, the drug can be prescribed if the benefit to the mother exceeds any possible risks to the child. This is due to its ability to penetrate into breast milk.

Reviews about Salbutamol

Salbutamol is described as a powerful and effective remedy. In particular, a lot of positive feedback was left on preparations manufactured by Russian companies. Moshimfarmpreparaty im. ON. Semashko and Altayvitamins.
For many people who suffer from bronchial asthma, these funds are a real salvation, as they quickly improve the patient's condition even with severe bronchospasm.
Another plus of domestic drugs is their low price compared to foreign analogues, which is a very important criterion of choice for a person forced to use the drug for a long time.
Somewhat less optimistic are the reviews on Salbutamol-Teva. Almost all patients who have tried this drug note the aesthetic appearance and inconvenience of the inhaler. The main drawback of the device indicates a short mouthpiece, which makes the use of the drug not very comfortable.
