Totema is a combined drug that represents the pharmacological group of anti-anemic drugs. It is used to treat anemia, mainly associated with an insufficient level of iron in the patient's body.

Description of dosage form, composition

Tot'hema preparation is released in a dosage form solution for oral administration (oral administration). It has a dark brown color and a characteristic smell. A small precipitate is allowed in the solution. The main active ingredients of the preparation are soya mineral salts, their content in 1 ml of the solution is:
    Iron - 50 mg.
    Copper is 700 μg.
    Manganese - 1.33 mg.
Also, the composition of the solution for ingestion of Tot'hma includes auxiliary components, including polysorbate 80, sodium citrate dihydrate, dextrose, citric acid, glycerol, sucrose, caramel dye, flavor, purified water. Totema solution is contained in glass sealed ampoules of 10 ml, which are placed in cardboard pallets of 10 pieces. The cardboard pack contains 2 pallets (20 ampoules) and instruction on the use of the drug.

Therapeutic effects, pharmacokinetics

The solution for oral administration of Totema has an anti-anemic therapeutic effect. It contains iron, which is the main component of the heme (part of the hemoglobin molecule and myoglobin in the muscles, responsible for the binding and transport of oxygen and carbon dioxide). Copper and manganese are necessary for the normal functional state of most enzymatic systems, including those responsible for the synthesis of hemoglobin. After taking the Totema solution, the active ingredients of the drug are absorbed into the blood, mainly from the duodenum. They are evenly distributed in tissues, where they are included in metabolic processes.

Indications for use

The reception of the Totema solution is indicated for the treatment of anemia (anemia) caused by iron deficiency (iron deficiency anemia), particularly after bleeding, in pregnant women against a background of increased need for iron for the developing fetus, inadequate intake of iron with food or impaired digestion from the intestine.

Contraindications to use

Admission Totema solution is contraindicated in several pathological conditions of the patient's body, which include:
Thalassemia is a pathological change in the shape of erythrocytes, caused by the synthesis of altered hemoglobin.
A peptic ulcer with localization of a defect in the wall of the stomach or duodenum in the stage of exacerbation of the pathological process.
Poisoning (intoxication) with salts of lead, copper, manganese.
Anemia that is not associated with iron deficiency.
Disease Wilson-Konovalov, characterized by a violation of copper metabolism.
Hemochromatosis, hemosiderosis - a violation of the metabolism of hemoglobin in the body.
Disorders of digestion or absorption of some carbohydrates in the intestine (glucose-galactose malabsorption, lactase insufficiency, individual lactose intolerance).
Hypersensitivity to any of the components of the drug.
The patient's age is up to 3 months.
The drug with caution is used with concomitant diabetes. Before the appointment of the Totema solution, the doctor excludes the presence of possible contraindications.

Application rules, dosage

Tot'hema solution is intended for oral administration. To do this, using the pallet carton, the tips of the glass ampoule on both sides break off, and the contents pour into a glass with pure or sweetened water, which is then drunk before eating. Dosage for the treatment of anemia in adults is 2-4 ampoules per day in 2-4 doses (children over 3 years of age, the dosage is calculated based on the 5-7 mg of iron per kg of body weight of the child). For the prevention of iron deficiency anemia in pregnant women (no earlier than 4 months after the onset of pregnancy), the drug is prescribed in a dosage of 1 ampoule a day. The doctor determines the duration of the course of therapy, the dosage and the regimen for the use of this medication individually. Usually the course of therapy lasts no less than a period of time from 3 to 6 months, which is necessary for a sufficient accumulation of iron reserves in the patient's body.

Negative reactions

Against the background of taking the Totema solution, it is possible to develop negative reactions from the digestive system, including heartburn, nausea accompanied by vomiting, stool (constipation or diarrhea), stools dark, darkening of the enamel of the teeth, pain in the upper abdomen. Also the preparation can lead to allergic reactions, more often in the form of a rash on the skin and its itching.

Features of use

Before using the Totema solution, you should carefully read the instructions and pay attention to several features of the correct use of the drug:
Excessive consumption of tea suppresses the absorption of iron ions from the intestine into the blood.
It is advisable to monitor the effectiveness of the therapy not earlier than in 3 months.
To reduce the likelihood of darkening the enamel of the teeth, you should try to minimize the amount of time to keep the Totema solution in the mouth.
The preparation contains carbohydrates (in 1 ampoule contains 3 g of sucrose), which make up 1/4 XE (bread units), which must be taken into account in patients with concomitant diabetes.
During the course of therapy with this drug, the stool can acquire a dark color, which is not a pathological reaction and does not require medical intervention.
Against the background of taking a solution of Totema, it is possible to obtain a false positive result of the analysis of feces for occult blood.
The active components of this drug do not directly affect the functional state of the nervous system, the speed of psychomotor reactions, the ability to concentrate.
In the pharmacy network, the Totema solution is dispensed by prescription. It is not recommended to use it independently.

Excess dose

Exceeding the recommended dosage of the Totema solution is accompanied by an increase in temperature (hyperthermia), weakness, abdominal pain, diarrhea. In this case, symptomatic and pathogenetic therapy with deferoxamine is carried out.

Storage Rules

The shelf life of the Totema solution is 2 years. It is important to store it in a dark, dry place inaccessible to children at an air temperature of no higher than 25 ° C.
