Usually is issued in ampoules for 5 ml, placed on the contour plastic pallet 5 or 10 ampoules, in the carton packing there are 1 or 2 of them. The drug can be equipped with a scarifier. Ampoules of brown color, made of glass of the first hydrolytic class, have a break point.

pharmachologic effect

Nootropic action and improvement of the blood flow of the brain.

Pharmacodynamics and pharmacokinetics

Vinpotropile is a combined preparation that can improve cerebral blood flow due to the active substance - vinpocetine and have a nootropic effect through the active ingredient - piracetam.
Vinpocetine is able to improve brain metabolism by increasing glucose and oxygen consumption, norepinephrine and serotonin metabolism, neuronal resistance to hypoxia, and has an antioxidant effect. Intensification of glucose transport occurs through the BBB, while the decay of glucose molecules becomes more energy-efficient: the Ca2 + -dependent PDE is selectively blocked by the aerobic pathway and the level of AMP, catecholamines, and cyclic GMP and ATP of the brain increases. Regarding the blood system - reduces the viscosity and aggregation of platelets, increases the plasticity of oxygen carriers - erythrocytes, blocks the utilization of adenosine and activates the return of oxygen. Enhanced cerebral blood flow does not cause significant changes in systemic circulation. It is established that the substance can penetrate through the placental and histohematological barriers.
Piracetam is included as an active substance in Vinpotropile to optimize the integrative activity of the brain: by promoting memory consolidation and the speed of spreading excitement, hemispheric coupling makes the learning process more effective. In addition, sedative and psychostimulating effect is not observed. Under the influence of drugs gradually improves microcirculation and suppressed aggregation of activated platelets, which has a cerebroprotective effect in hypoxia, intoxication, exposure to electric shock. The EEG shows that α- and β-activities are enhanced, δ-activity decreases, and the severity of the vestibular nystagmus is reduced. At the initial stage of cerebral vascular disease in the elderly and senile age has a pronounced effect. Penetrating through the GE and placental barriers, it is effectively removed by hemodialysis.

Pharmacokinetics of the concentrate (parenteral administration)

Vinpocetine reaches a therapeutic plasma concentration of 10-20 ng / ml, with a volume distribution of 5.3 liters per kilogram, binds to proteins by 66 percent with a clearance of 66.7 l / h, has an extrahepatic metabolism. In breast milk within an hour is 0.25%, half-life is 220-367 min. Elimination by the kidneys and through the intestine (ratio 3: 2).
The half-life of pyracetam from plasma is 4.5 hours, of the brain - 7.7 hours, of cerebrospinal fluid - 8.5 hours (with chronic and terminal renal failure may extend to 2.5 days). Virtually without biotransformation, two-thirds of the substance is excreted by the kidneys in 30 hours, with a total clearance of 80-90 ml. Studies in the vivariums showed that the substance is selectively cumulated in the tissues of the GM cortex, mostly basal cores, in the frontal lobes, parietal lobes, occipital lobes and in the cerebellum.

Pharmacokinetics of tablets and capsules

The maximum concentration of vinpocetine is reached after 2-4 hours, and the absorption process occurs mainly in the initial sections of the gastrointestinal tract. Subsequent inductions induce linear kinetics. The half-life is approximately 290 minutes. Whereas the bioavailability of piracetam is approximately 100%, the maximum concentration of 84 μg per ml after an hour, multiple-dose increases it to 115 μg per ml.

Indications for use Vinpotropile

Instructions for use Vinpotropile (Method and dosage)

To prepare a solution for infusions, take 2-3 amp. of the preparation, 500 ml of a 0.9% solution of sodium chloride and a dextrose-containing solution. Enter should be intravenously at 50-100 ml once a day at a speed of not more than 80 drops per minute. Carry out infusion no later than three hours before bedtime. The standard course of treatment is from 10 days to 2 weeks, which, if necessary, can be repeated after 6-8 weeks. If there is a marked improvement, you can switch to another method of intake - inward.
Instructions for use Vinpotropile in tablets: take inside 1 tablet 2 or 3 times during the day, but no later than 4 hours before sleep, and does not matter the mode of food intake. You need to drink plenty of clean water.
Capsules are also taken orally, however, before meals, 1 table. 2 or 3 times during the day, but no later than 4 hours before bedtime. As maintenance therapy, you should take 1 capsule 3 times during the day.
The length of the course of treatment with tablets and capsules ranges from 2-3 weeks to 2 months and even half a year. The withdrawal of the drug requires a gradual dose reduction or the use of another dosage form, for example, capsules. Duration of treatment and dosage should be checked with your doctor.

Overdose

An overdose of vinpocetine is accompanied by an increase in the severity of dose-dependent adverse reactions. For treatment, gastric lavage, activated charcoal and symptomatic therapy are prescribed.
Overdose of pyracetam invokes abdominal pain, diarrhea with blood. Treat symptomatically, hemodialysis in this case is effective at 50-60%. A specific antidote was not found.

Acenocoumarol - reduced platelet aggregation, viscosity, fibrinogen content and von Willebrand factor.
Methyldopa is the potentiation of the hypotensive effect.
Anticonvulsants - a decrease in the effectiveness of drugs.
Use with caution with substances that have a central or antiarrhythmic effect.
