Pharmacokinetics

The drug contains a combination of B vitamins: thiamine hydrochloride (B1), pyridoxine hydrochloride (B6) and cyanocobalamin (B12).
Soluble in water vitamins are fully absorbed in the body after intramuscular injection.
Thiamine hydrochloride (vitamin B1). Part of the assimilated thiamine hydrochloride takes part in enterohepatic circulation. The main products of excretion of thiamine hydrochloride are thiaminecarboxylic acid and pyramine (2,5-dimethyl-4-aminopyrimidine). Together with this, a small amount of thiamine hydrochloride is released unchanged.
Pyridoxine hydrochloride (vitamin B6). In the body of pyridoxine hydrochloride is oxidized to pyridoxal or aminated to pyridoxamine. The condition of its functioning as coenzyme is the phosphorylation on the CH2OH-group in the 5-th position, that is, the formation of pyridoxal-5-phosphate (PALF). In the blood, approximately 80% of PALF is bound to proteins. Pyridoxine hydrochloride is predominantly accumulated in the muscles in the form of PALF. The main product of excretion is 4-pyridoxic acid.
Cyanocobalamin (vitamin B12). After absorption, vitamin B12 binds in the serum to specific B12-binding beta (trans-balanamine) and B12-binding alpha1-globulin. The accumulation of vitamin B12 occurs mainly in the liver. The half-life of serum is approximately 5 days, and from the liver - about 1 year.

Pharmacodynamics

Neuromultivit contains combinations of B vitamins that are especially needed as coenzymes in the intermediate metabolism of the central and peripheral nervous system. Like all vitamins, they are an integral part of nutrition and can not be produced by the body on their own. The addition of these vitamins to diet in therapeutic doses helps to balance any existing shortage, thus providing the necessary amount of coenzymes. Co-administration in large doses is considered to be more effective than using components alone, especially when it is necessary to quickly correct an acute vitamin deficiency.
The therapeutic use of these vitamins in disorders of the nervous system prevents the appearance of concomitant deficiency conditions (sometimes caused by an increased demand due to the disease) on the one hand, and also stimulates the natural mechanism of debugging the other side. At the same time, the analgesic effect of the B complex of vitamins enhances the healing effect.
The active form of vitamin B1 (thiamine) is thiamine diphosphate. During the carbohydrate metabolism, it acts as a coenzyme in the decarboxylation of alpha-keto acids, such as pyruvate and alpha-ketoglutarate. Thiamin is present in the membrane nervous tissue affects the functions of the nerves through the biosynthesis of neurotransmitters.
Vitamin B6 (pyridoxine) is a coenzyme necessary for the metabolism of amino acids, acting as a prosthetic group of important enzymes in the nervous tissue. Thus, Pyridoxine affects the biosynthesis and concentration of several neurotransmitters, such as dopamine, noradrenolin, 5 hydroxytryptamine, histamine and GABA. Pyridoxine performs an important function in the conversion of homocysteine ​​to cysteine, as well as the resynthesis of homocysteine ​​into methionine.
Vitamin B12 (cyanocobalamin) affects the synthesis of nucleic acids and the composition of fatty acids in neuronal cerebrosides and phospholipids. As
it is a coenzyme, it performs an important function in cell growth and during replication, for example, also in the resynthesis of homocysteine ​​into methionine.

Dosing and Administration

The drug is administered only intramuscularly (in the upper outer quadrant of the gluteus muscle).
In severe, acute conditions: 1 injection a day before the relief of the syndrome.
At mild conditions: 1 injection 2-3 times a week.

Drug Interactions

Thiamine solution is incompatible with solutions of dopamine, methoxantrone, pyridoxal phosphate.
With the simultaneous administration of L-dopa, Vitamin B6 can reduce its effect. Pyridoxine weakens the antitumor effect of altretamine.
Pyridoxine enhances the hypoglycemic effect of glimepiride, insulin and a combination of glibenclamide + metformin.
Cyanocobalamin is incompatible with ascorbic acid, heavy metal salts and pyridoxal phosphate.
Lansoprazole slows the absorption of cyanocobalamin, mesalazine slows its absorption.
Aminoglycosides, salicylates, antiepileptic medicines, colchicine, potassium preparations - reduce the absorption of vitamin B12.
The combination of thiamine + pyridoxine + cyanocobalamin is incompatible with ascorbic acid, benzylpenicillin, dextrose, riboflavin, phenobarbital.
When combined with epinephrine and norepinephrine, it is possible to intensify their side effects on the heart.

special instructions

Neuromultivit, solution for injection can not be mixed with other drugs.
Do not administer for acute thromboembolic disease. Care should be taken in persons prone to thrombosis with angina pectoris. It is necessary to monitor blood coagulability.
With caution apply for peptic ulcer of stomach and duodenal ulcer, ischemic heart disease.
With severe liver damage - can cause deterioration of its function.
Features of the effect of the drug on the ability to drive a vehicle or potentially dangerous mechanisms
Neuromultivit has an effect on the ability to drive vehicles. Care should be taken when managing transport and working with potentially dangerous mechanisms.

Overdose

Overdosing can occur only after the administration of extremely large doses over a long period.
Symptoms: neuropathy with ataxia, impaired sensory functions, seizures with EEG changes, hypochromic anemia, dermatitis.
Treatment: symptomatic therapy.

Form of production and packaging

2 ml of the drug is dispensed into ampoules of dark glass (hydrolytic class I) with a capacity of 2 ml.
The label is self-adhesive on the ampoule.
5 ampoules are placed in a contour mesh package made of a polyvinyl chloride film.
