pharmachologic effect
Develops antihypertensive, antiarrhythmic, antianginal and beta 1-adrenoblocking stimulation. Causes rapid reduction of contractions in the heart muscle.
In the case of sinus tachycardia against a background of hyperthyroidism and functional problems with the heart, as well as atrial fibrillation and supraventricular tachycardia, the drug is able to significantly slow the heart rate down to the restoration of the sinus rhythm.
In contrast to nonselective beta-blockers, the effect of metoprolol on carbohydrate metabolism and insulin production is less significant.
Pharmacodynamics and pharmacokinetics
The drug has a high rate of digestion in the digestive tract. Within 1.5-2 hours after admission, Cmax is reached in the blood plasma. Under the influence of the active substance, the increased activity of the sympathetic system with respect to the heart is suppressed. From what tablets Egilok with regular use cause a decrease in the level of cholesterol in the serum. Bioavailability of the drug is increased by 30-40%, if you take metoprolol with food.
On the removal and assimilation of the active substance, the impaired renal and hepatic function has virtually no effect. However, with pronounced impairment of liver function (cirrhosis, superimposed portocaval shunt), bioavailability increases significantly, and in chronic renal failure, the risk of undesirable side effects increases. In the elderly, the pharmacokinetics of the drug does not respond to significant changes.
After use, the medicine passes a full degree of absorbability. Egilok has a weak binding to proteins in the blood plasma (no more than 10%). The drug is excreted from the body mainly in the form of metabolites, only 5% is excreted by the kidneys.
Indications for the use of Egilok
preventive prevention of migraine attacks;
high blood pressure;
impaired functional cardiac activity;
angina pectoris;
impaired cardiac rhythm (supraventricular tachycardia and bradycardia with ventricular extrasystoles and atrial fibrillation);
myocardial infarction.
Indications for the use of tablets also apply to people over 60 years of age.
Contraindications
SSSU;
cardiogenic shock;
pronounced bradycardia (less than 50 beats per minute);
lactation period;
simultaneous administration of MAO inhibitors;
age to 18 years;
hypersensitivity to the components of the drug in particular, or to beta-blockers in general;
sinoatrial blockade;
severely impaired peripheral circulation;
bronchial asthma in severe form;
AB - blockade of 2 or 3 degrees.
Adverse Events
In relation to the central nervous system: increased fatigue threshold (very often), headaches and dizziness (often); infrequently - convulsions, weakened attention, depressive state, increased heart failure, nightmares; rarely - nervous excitability, anxiety, sexual dysfunction, hallucinations, memory impairment.
In relation to the senses (rarely): deterioration of vision.
In relation to the digestive system (rarely): abdominal pain, diarrhea, constipation, dryness in the mucous membrane in the oral cavity.
In relation to the respiratory system: dyspnoea with physical exertion (often), rhinitis (rarely).
In relation to skin integuments (not often): a rash, increased sweating.

Instructions for Egilok C: take 1 time per day, in the morning.
How to take (the final size of the dose and the number of receptions) the medicine is determined by the doctor individually. The maximum dose is 200 mg. In case of impaired renal function and elderly age, the redistribution of the amount of medication consumed is not required.
Heart failure with compensation: 25 mg per day.
Hyperthyroidism: 50-200 mg per day.
Arrhythmia: 50-200 mg per day.
Angina pectoris: 50 mg per day.
Migraine attacks (prevention): 100-200 mg per day.
Tachycardia: 50-200 mg per day.
Myocardial infarction (secondary prophylaxis): 200 mg per day.
Overdose
Excessive and uncoordinated use of the drug leads to overdose, the most obvious symptom of which is the response from the cardiovascular system: slowing of the pulse, heart failure. In some cases, when using drugs by persons under 18 years of age, a negative reaction from the CNS is also possible: increased fatigue, the appearance of seizures, excessive sweating, the onset of fatigue.
Common symptoms: apnea, impaired consciousness, tremor, bronchospasm, vomiting, hyperkalemia or hyperglycemia, impaired renal activity, asystole, marked decrease in blood pressure.
In case of an overdose for 20-120 minutes, one or more of the above symptoms occurs depending on the characteristics of the organism. High concentration of metoprolol in the body, depending on the nature of the symptoms, is eliminated by gastric lavage, symptomatic therapy, the appointment of adsorbents, atropine sulfate, gluconate, dopamine, norepinephrine.
The use of Egilok medicines with other medications
The list of prohibited preparations for simultaneous use with Egilokom is wide. Therefore, it is necessary with special care to combine this drug with third-party medicines.
When mixed with verapamil it can cause cardiac arrest.
When mixed with beta-adrenoblockers (estrogens, theophylline, indomethacin), the hypotensive property of metoprolol decreases.
When mixed with ethanol, the pressure on the CNS is increased.
When mixed with oral hypoglycemic drugs and insulin, the likelihood of hypoglycemia increases.
When mixed with barbiturates (pentobarbital), the metabolism of metoprolol is accelerated by the induction of enzymes.

Concor or Egilok - which is better?
The exact answer can be given only with an individual examination. However, in general, Concor has fewer side effects than Egilok, and its use at a low pulse is more permissible. Egilok has a stronger drug effect compared to Concor.
Egilok and alcohol
Interaction of the drug with alcohol-containing substances leads to a sharp decrease in blood pressure, which in turn can lead to hypoxia of the brain. So, are possible: weakness, dizziness, loss of consciousness. In the case of failure to provide medical care and with strong concentrations in the body of metoprolol and alcohol, the energy resources in the brain are depleted, which can lead to tragic consequences.
Egilok during pregnancy and lactation
In connection with the lack of comprehensive information about the medical studies carried out concerning the impact of metoprolol on the embryo, it is allowed to include the drug during the treatment period in only one case, when the benefit for the pregnant patient is considered higher than the risk of harm to the fetus.
Nevertheless, the drug is often used in pregnancy, but in no case in the first trimester and not during lactation and breastfeeding of the child. The size of the dose and frequency of acceptance are determined strictly individually. Among those who tested the drug, there are reviews during pregnancy. As a rule, those women who followed the recommendations of the doctor, during the course of taking medication, did not experience discomfort, but on the contrary, their cardiovascular status improved.
Reviews on Egilok
Given that the drug is one of the most popular drugs in its group, there are many responses, such as the doctors' reviews of Egilok Retard, as well as feedback from ordinary users who publish their opinions on the Internet. In order to learn about the reviews of tablets, it is enough to visit the forum on pharmaceuticals. In the opinion of the majority, the drug can often cause temporary fatigue in the early stages of admission, but at the same time it fights well with the rapid pulse, quickly slowing it down. In addition, during the course of slowing the rate of motor reactions is noted, and therefore driving the vehicle and managing potentially dangerous mechanisms should be cautious.
