Composition

The preparation contains the active substance isosorbide-5-mononitrate, as well as several auxiliary elements:
    lactose monohydrate;
    microcrystalline cellulose;
    corn starch;
    talc;
    magnesium stearate.

Form of issue

Tablets are white in color, round in shape and centered.

pharmachologic effect

Monocinque has a predominant effect on venous vessels. The peripheral vasodilator widens the vessels, reduces the level of preload, as it expands the peripheral veins, reduces afterload, as it reduces overall peripheral vascular resistance, dilates coronary vessels, regulates oxygen balance in the myocardium, stimulates blood flow and promotes its redistribution into ischemic areas.
The active substance of the drug reduces the systolic stress of the walls of the left ventricle and reduces the level of its diastolic volume.
In patients with IHD, the drug promotes increased tolerance to physical exertion, in parallel reducing the pressure in a small circle of circulation.
Isosorbite mononitrate promotes relaxation of the musculature of the bronchi, gall bladder, biliary tract, urinary tract, esophagus, thick and small intestine, including sphincters.
The antianginal effect of the drug is observed already half an hour after oral administration and lasts for a maximum of six hours.

Pharmacodynamics and pharmacokinetics

This drug is characterized by a rapid and almost complete absorption with bioavailability of up to 100 percent.
Approximately one and a half hours after ingestion, the maximum concentration of the active substance in the blood plasma is reached. Half an hour after taking the tablets, the therapeutic concentration of isosorbide mononitrate in the blood is 250 ng / ml and by the fourth hour it reaches 414 ng / ml. By the twelfth hour, the therapeutic concentration decreases and is already 199 ng / ml, indicating a very slow drop in concentration.
Metabolization of the active substance is carried out in the liver with the formation of pharmacologically inactive metabolites.
Isosorbide mononitrate is excreted by the kidneys with a half-life of up to five hours. Approximately two percent is displayed unchanged.

Indications for use

Long-term treatment of angina and heart failure.

Contraindications

    high susceptibility to isosorbide mononitrate or any component of the drug;
    high susceptibility to organic nitrates;
    simultaneous use of Monocinque Retard with inhibitors of phosphodiesterase-5 (PDE-5) (Sildenafil, Tadalafil, or Vardenafil);
    angle-closure glaucoma (intraocular pressure may be increased);
    head trauma or cerebral hemorrhage (increased intracranial pressure);
    severe anemia.

Take with caution:

    with severe renal or severe hepatic insufficiency;
    hypothyroidism;
    malnutrition;
    hypothermia;
    hypotension;
    hypertrophic cardiomyopathy;
    breastfeeding.

Overdose

Symptoms of overdose include hypotension, pulsating headache, palpitations, visual impairment, tachycardia, methemoglobinemia, sweating, metabolic acidosis and to whom.
High blood sugar levels or methemoglobinemia can lead to signs or symptoms of hypoxia. In case of symptoms of overdose, symptomatic treatment is prescribed.

Interaction with other drugs

Substrate CYP3A4, (main).
Interaction of Monocinque with CYP3A4 inductors: CYP3A4 inducers reduce the levels / effect of isosorbide mononitrate. Such medications include aminoglutethimide, carbamazepine, nafcillin, nevirapine, phenobarbital, phenytoin, and Rifamycins.
Interaction with CYP3A4 inhibitors: May increase the levels / effect of isosorbide mononitrate. Such inhibitors include: azole imidazole, ciprofloxacin, clarithromycin, diclofenac, doxycycline, erythromycin, imatinib, isoniazid, nefazodone, nicardipine, propofol, protease inhibitors, quinidine and verapamil.
Interaction with Sildenafil, Vardenafil: Significant reduction in systolic and diastolic blood pressure. Specialists do not recommend the use of sildenafil, tadalafil, vardenafil and drugs of this effect within 24 hours after taking Monocinque.
