Pharmacodynamics

Peripheral vasodilator with a predominant effect on the venous vessels. The action of nitroglycerin is associated mainly with a decrease in myocardial oxygen demand due to a decrease in preload (dilation of peripheral veins and a decrease in blood flow to the right atrium) and an afterload (decrease in total peripheral vascular resistance).
It has a central inhibitory effect on the sympathetic vascular tone, inhibits the vascular component of the formation of pain syndrome. Causes expansion of meningeal vessels, which explains the headache in its application. When using the drug under the tongue, an attack of stenocardia is usually stopped after 1.5 minutes, the hemodynamic and antianginal effect lasts from 30 to 60 minutes.

Pharmacokinetics

Dosage and administration

If an angina attack is not stopped within 5 minutes, you must take another 1 tablet of Nitrocor.
In the absence of a therapeutic effect after taking 2 tablets, you must call a doctor.
With frequent attacks of stenocardia, it is advisable to use prolonged nitroglycerin preparations.

Side effect

On the part of the central nervous system: blurred vision, "nitrate" headache (especially at the beginning of the course of treatment, with continued therapy decreases), dizziness and a feeling of weakness, anxiety, psychotic reactions, lethargy, disorientation.
Since the cardiovascular system: lower blood pressure; tachycardia, rarely (especially in case of overdose) - orthostatic collapse, cyanosis, facial flushing.
On the part of the gastrointestinal tract: dry mouth, nausea, vomiting, abdominal pain.
Allergic reactions, hypothermia.

Overdose

Symptoms: decrease in blood pressure (below 90 mm Hg) with orthostatic disregulation, severe dizziness, headache, fainting, shortness of breath, severe weakness, rapid heartbeat, fever, cramps, asthenia, drowsiness. When using high doses (more than 20 mcg / kg) - collapse, cyanosis, methemoglobinemia, dyspnea and tachypnea.

Interaction

The hypotensive effect of the drug might be excessive in the case of uncontrolled simultaneous reception vasodilators, antihypertensives, angiotensin converting enzyme (ACE) inhibitors, phosphodiesterase inhibitors, blockers "slow" calcium channel blockers, tricyclic antidepressants, alcohol, diuretics, beta-blockers, monoamine oxidase (MAO) inhibitors procainamide.
Appointment with dihydroergotamine can lead to an increase in its content in the blood and high blood pressure (due to increased bioavailability of dihydroergotamine).
The simultaneous appointment of Nitrocor and heparin reduces the effectiveness of the latter (after discontinuation of the drug, a significant decrease in blood clotting is possible, which may require lowering the dose of heparin).

Special instructions for Nitrocor

While taking Nitrocor tablets, a significant reduction in blood pressure and the appearance of dizziness may occur with a sudden transition to a vertical position from a sitting or lying position; when drinking alcohol, performing physical exercises and hot weather. To Nitrocor, as well as to other organic nitrates, with frequent use may develop addiction, requiring an increase in dosage.
