Pharmacodynamics and pharmacokinetics

Pharmacodynamics

Antianginal drug belongs to the group of peripheral vasodilators, affecting mainly the venous vessels. The mechanism of action of nitroglycerin is based on the ability to release nitric oxide, which is an endothelial relaxing factor. Based on the action of nitric oxide (it increases the concentration of cyclic guanosine monophosphatase inside the cells), the penetration of calcium ions into smooth muscle cells is slowed down, which causes relaxation of smooth muscles of the vascular wall and their expansion. As a result, preload is reduced (venous return to the heart) and afterload (resistance in the great circulation), which reduces the need of the heart muscle for oxygen. Reducing the return of venous blood reduces the filling pressure, normalizes the blood supply to the subendocardial layers and pressure in the pulmonary circulation.
As a result of the expansion of the coronary vessels, there is a redistribution of coronary blood flow in areas with insufficient blood supply. Nitrospray (nitroglycerin) lowers the sympathetic tone of the vessels, while the vascular component of the pain syndrome is inhibited. The drug relaxes the smooth muscle cells of the bronchi, gallbladder, urinary tract, esophagus, bile ducts, intestines and their sphincters.

Pharmacokinetics

The drug is rapidly absorbed from the surface of the mucous membranes, reaching a therapeutic concentration in the blood within a few minutes. Communication with blood proteins is about 60%. Excreted in the urine.

Indications for use of Nitrospray

Prevention and relief of attacks in angina pectoris, acute myocardial infarction and in the treatment of acute left ventricular failure.

Contraindications for Nitrospray

Hypotension, shock, hemorrhagic stroke, vascular collapse, hypertrophic cardiomyopathy, acute myocardial infarction (with low filling pressure in the left ventricle), recent head injury, childhood, angle-glaucoma, toxic pulmonary edema, pregnancy, lactation.

Side effects

Headache, weakness, blurred vision, dizziness, restlessness, nausea, vomiting, dry mouth, skin rash, arterial hypotension, dermatitis.

Nitrospray, instructions for use (method and dosage)

Nitrospray is applied sublingually in the form of an aerosol. 1 dose aerosol contains 400 µg nitroglycerin. At the first signs of a stenocardia attack, to relieve it, a remedy is applied under or on the tongue with 1-2 doses of the drug (400-800 mcg) while holding the breath. Before use, shake the can and hold it upright when spraying.
After spraying the spray, it is necessary to close the mouth, but not to swallow immediately. Repeated dosing should be done at intervals of at least 30 seconds. The drug, if necessary, re-enter, but not more than 3 doses over 15 minutes at intervals of 5 minutes.
For the prevention of an attack, 1 dose is administered 5-10 minutes before the intended stress or load. The maximum dosage of nitrospray per day is 6.4 g (16 doses), a single dose of 1.6 g (4 doses). During the period of use of the drug, it is necessary to be careful with a sudden change in body position, hot weather, physical exercise due to the risk of orthostatic reactions. Alcohol is not recommended during this period. The drug must be canceled under the supervision of a physician and gradually.
If, after the injection of 3 doses for 15 minutes, the attack is not stopped, hospitalization of the patient is necessary. If the drug is accompanied by a headache, it is possible to simultaneously take acetylsalicylic acid.

Overdose

Dizziness, cyanosis of the lips, palms, nails, fainting, shortness of breath, severe weakness or fatigue, fever, palpitations, convulsions.

Interaction

The hypotensive effect of Nitrospray is enhanced while taking other antihypertensive drugs, antidepressants, ethanol, PDE inhibitors, while reducing the effectiveness of Nitrospray. At the same time taking nitrospray with heparin, the anticoagulant effect of heparin is reduced.
