Additional substances: sodium croccarmellose, magnesium stearate, microcrystalline cellulose, magnesium oxide, hydroxypropyl cellulose, silicon dioxide, lactose monohydrate.
The shell consists of macrogol, hypromellose, titanium dioxide and talc.

Form of issue

pharmachologic effect

It belongs to the group of statins and has a hypolipidemic effect. Inhibits selectively and competently the enzyme involved in the synthesis of cholesterol.
Triglycerides and cholesterol become constituent parts of atherogenic lipoproteins in the liver, after which they are transferred to the periphery by blood. By interacting with low-density lipoprotein receptors, they are converted into these lipoproteins.
Due to the inhibition of HMG-CoA reductase, the level of lipoproteins and cholesterol in the blood decreases. The synthesis of LDL is reduced and the activity of their receptors is increased.
The drug is able to reduce the amount of LDL in homozygous hypercholesterolemia of a hereditary nature, when other drugs have no effect.
The drug reduces the level of cholesterol by 30-46%, atherogenic lipoproteins by 41-61%, triglycerides by 14-33% and increases the content of lipoproteins that have anti-atherogenic properties.

Pharmacodynamics and pharmacokinetics

In the blood, the maximum concentration of the drug occurs within 60-120 minutes. Eating reduces the duration of absorption, but lowering cholesterol is comparable to that without eating. In case of application in the evening, the concentration of the drug is lower than when taken in the morning.
With blood proteins binds to 98%. It is metabolized in the liver with the formation of active metabolites.
It is excreted with bile, the half-life is 14 hours. The effectiveness of the drug is maintained by active metabolites up to 30 hours. When hemodialysis is not withdrawn.

    treatment of patients who have increased triglycerides in the blood (Fredrickson type IV) and type III Fredrickson (dysetalipoproteinemia), if the diet does not work;
    decrease in the amount of cholesterol and LDL with homozygous family-type hypercholesterolemia;
treatment of heart disease and blood vessels in the presence of increased factors for the onset of coronary heart disease (arterial hypertension, patients older than 55 years, a history of stroke, albuminuria, left ventricular hypertrophy, smoking, peripheral vascular disease, IHD in the family, diabetes mellitus).

Instructions for use of Torvacard (Method and dosage)

Please reed this instruction before buy Torvacard.
In the process of treatment the patient must observe a lipid-lowering diet.
Therapy is started with 10 mg per day, subsequently increased to 20 mg. Daily therapeutic dose - from 10 to 80 mg. The dose is selected taking into account laboratory indicators and individual characteristics.
The drug is taken regardless of food.
Before the reception and, if necessary, dose adjustments, laboratory control of lipid levels is performed.
The effect of the application comes after 14 days.
To treat patients with homozygous hypercholesterolemia, one of the few drugs that give effect is Torvacard, the instructions for use clearly determine the daily dose, which is 80 mg.

Overdose

When symptoms of an overdose occur symptomatic therapy.

Interaction

Use of drugs that inhibit metabolism, mediated by the enzyme CYP450, erythromycin, antifungal and immunosuppressive drugs, fibrates, cyclosporine, clarithromycin, nicotinamide, nicotinic acid, the concentration of Torvacard in the blood increases. This increases the likelihood of myopathy, so you need to monitor the level of CK in the blood.
The joint administration of funds with aluminum hydroxide or magnesium reduces the concentration of Torvacard, but this does not affect the effectiveness.
Combination with colestipol reduces the concentration of atorvastatin, but their combined lipid-lowering effect is superior to each separately.
Cimetidine, Spironolactone, ketoconazole increases the probability of a decrease in the level of steroid hormones.
Taking oral contraceptives and a daily dose of Torvacard 80 mg increases the content of ethinyl estradiol in the blood.
Use in combination with digoxin reduces the concentration of the latter by 20%.

Storage conditions

In a place that is protected from children.
Shelf life - 4 years.

special instructions

Before starting treatment, you should try to lower cholesterol level by diet, treatment of obesity and concomitant diseases, increase in physical activity.
In the process of treatment, it is necessary to control the level of AST and ALT. For the first time the control is performed before, 6 weeks and 3 months after the start of therapy, as well as after dose adjustment and once every six months. If the level of enzymes increases more than 3 times, the drug is canceled.
Taking Torvacard can cause weakness and pain in the muscles (myopathy) and increase in CK in the blood. If muscle pain or weakness occurs in combination with fever, you should consult your doctor.
The drug is canceled at the risk of renal failure due to rhabdomyolysis. This can be trauma, extensive surgery, metabolic and electrolyte balance disorders, arterial hypotension, severe infection, convulsions.
Taking Torvacard can lead to the development of diabetes in patients who have an increased risk. But it is worth remembering that the benefits of taking statins are higher than the risk of diabetes, so there is no need to cancel the drug, and patients at risk should be constantly under the supervision of a doctor.
