Composition

Form of issue

Cream Elidel - hormonal or not?

This is a non-hormonal drug. The main mechanism of action is oppression of production and release of inflammatory mediators, cytokines in mast cells and T-lymphocytes. This provides anti-inflammatory effect.
Pimecrolimus inhibits calcineurin (calcium-dependent phosphatase), which leads to inhibition of proliferation of T-lymphocytes and a slowdown in the production of interleukin-2, tumor necrosis factor-α, γ-interferon, and other cytokines.
The drug reduces the secondary immune response in skin T cells, which were taken from patients suffering from atopic dermatitis.
The active substance of the drug does not affect the growth of endothelium, keratinocytes and fibroblasts. It acts selectively on immune cells, does not disrupt the function of cells of monocytic origin and Langerhans cells, does not affect the differentiation of T cells, which distinguishes pimecrolimus from corticosteroids.
The drug showed a high effect in reducing inflammation with systemic and local admission.
With allergic contact dermatitis, the cream significantly reduces inflammation reactions under the influence of skin irritants, equal in effectiveness to clobetasol-17-propionate and fluticasone, one of the most active corticosteroids. In this case, there is no skin atrophy or a change in its structure. The cream reduces the inflammatory process on the skin, itching, reduces the manifestations of histopathology, while acting on the skin selectively, - shows less penetration than glucocorticosteroids or Tacrolimus.
The main advantage of Elidel is a local (on the skin) selective anti-inflammatory effect combined with minimal systemic action on the immune response.
The course of treatment of 6 weeks for children of different ages (starting from three months) makes it possible to significantly reduce the signs of skin inflammation - redness, excoriation, infiltration, lichenization and itching. Application during the year allows to reduce the number of exacerbations, prevent the development of atrophies and increase the hypersensitivity of the skin.
The cream of Elidel is not phototoxic and does not cause photosensitization.

Pharmacodynamics and pharmacokinetics

Pimecrolimus does not have a systemic effect and its concentration in the blood is so low that it is impossible to evaluate the metabolic rate.

Indications for use

Treatment of eczema (atopic dermatitis) in adults and children from 3 months.

Contraindications

    individual intolerance;
    children up to 3 months;
    damage to the skin at the site of acute infectious infection of various origins (viral, bacterial, fungal).

It is not recommended to use:

    with systemic erythroderma and the syndrome of Netherton because of the danger of increasing the systemic absorption of the drug;
    with a decrease in immunity, since there is no experience in this category of patients;
    on areas of the skin with suspected dysplastic changes or malignant lesions.

Side effects

With the greatest frequency local reactions are observed in the form of burning, irritation, redness and itching on the skin; can be folliculitis (pustular infection).
Sometimes there are dermatitis, herpetic eczema, paresthesia, molluscum contagiosum, deterioration of the disease course.
Rarely - angioedema, hives, anaphylactic reactions; alcohol intolerance, manifested in itching, rash, face redness.
Also, in rare cases, there may be a violation of skin pigmentation.

Instructions for the use of Elidel (Method and dosage)

To obtain the best effect, treatment begins at the first signs of the disease in order to prevent progression.
The cream is applied to the skin on a thin layer and rubbed until completely absorbed. Apply ointment 2 times a day until recovery. If the symptomatology persists for more than 6 weeks, the diagnosis of atopic dermatitis needs to be clarified.
If the treatment was discontinued, but the symptoms of the disease appeared again, the application should be resumed.

Interaction

Given the low systemic absorption of the cream, the risk of interaction with other agents for systemic use is unlikely.
