Form of release, composition and packaging

Spray nasal in the form of a clear, colorless or slightly colored solution, practically odorless.
1 ml
ipratropium bromide monohydrate 0.6 mg
xylometazoline hydrochloride 0.5 mg
Excipients: glycerol (85%) - 27.9 mg, disodium edetate dihydrate - 0.5 mg, hydrochloric acid concentrated to pH 4.5, sodium hydroxide to pH 4.5, water to 1 ml.
10 ml - vials from PVP (1) with pump dispensing device - packs cardboard.

pharmachologic effect

Xylometazoline belongs to the group of local vasoconstrictors (decongestants) with alpha-adrenomimetic action, causes narrowing of the blood vessels of the nasal mucosa, thereby eliminating edema and hyperemia of the nasopharyngeal mucosa. Relieves nasal congestion, facilitating nasal breathing in rhinitis.
Ipratropium bromide has an anticholinergic effect. With intranasal application reduces nasal secretion, stopping flow from the nose due to competitive inhibition of cholinergic receptors located in the epithelium of the nasal cavity.
In therapeutic concentrations, it does not irritate the mucous membrane, does not cause its hyperemia.
Otrivin complex begins to act after 5-10 minutes and has a lasting effect for 6-8 hours.

Pharmacokinetics

In the intranasal application, ipratropium bromide and xylometazoline hydrochloride are little absorbed and present in the blood plasma in small amounts.

Indications for Otrivin complex

- symptomatic treatment of edema and hyperemia of the nasal cavity;
- Acute respiratory diseases with phenomena of rhinitis (cold), accompanied by stuffiness;
Acute allergic rhinitis;
- Pollinosis;
- Sinusitis.

Contraindications

- hypersensitivity to the components of Otrivin complex;
- Hypersensitivity to atropine or similar compounds (hyoscyamine, scopolamine);
- glaucoma;
- surgical interventions on the membranes of the brain (in the anamnesis);
- I trimester of pregnancy;
- Atrophic rhinitis;
- age to 18 years.
With caution: diabetes, hyperthyroidism, pheochromocytoma, angina pectoris III-IV FK, obstruction of the bladder neck, prostatic hyperplasia; patients predisposed to epistaxis, dizziness, tremor of skeletal muscles, sleep disorders, arrhythmias, increased blood pressure; paralytic ileus; cystic fibrosis.

Dosage

Intranasally.
For patients over the age of 18: 1 injection in each nasal passage 3 times / day.
The drug is used no more than 7 days without consulting a doctor.
Long-term use of xylometazoline can cause swelling of the nasal mucosa and increased secretion, which is due to the development of increased sensitivity of cells to the active substances of Otrivin complex, the so-called reverse effect.
1 Injection of Otrivin complex contains about 70 μg of xylometazoline hydrochloride and 84 μg of ipratropium bromide.

Side effects of Otrivin complex

Determination of the frequency of adverse reactions: very often (≥10); often (≥1 / 100, <1/10); infrequently (≥1 / 1000, <1/100); rarely (≥1 / 10,000, <1/1000); very rarely (<1/10 000).
From the nervous system: often - headache; rarely - dizziness; very rarely - insomnia.
From the digestive system: often - dry mouth; infrequently - dyspepsia, nausea.
From the senses: rarely - increased intraocular pressure, mydriasis, pain in the eyes, worsening of the condition with a closed angle glaucoma; very rarely - a violation of the clarity of visual perception.
From the urinary system: rarely - difficulty urinating.
On the part of the respiratory system: often - nosebleeds, irritation and / or dryness of the mucous membrane of the nasopharynx, burning sensation, tingling, sneezing, nasal hypersecretion; infrequent - nasal congestion (with frequent and / or prolonged use of the drug).

Overdose

With nasal application of the drug Otrivin Complex acute overdose is unlikely, t. absorption of Otrivin complex is very low.
Symptoms: in case of overdose of xylometazoline, the clinical picture is characterized by nausea, sweating, lowering of body temperature, headache, bradycardia, accommodation disorder, hypertension, respiratory depression, coma. Arterial hypertension can be replaced by hypotension.
In case of excessive use of ipratropium bromide, an overdose is unlikely due to very little absorption of the substance into the blood, but dry mouth, difficulty in accommodation, tachycardia can develop.
Treatment: symptomatic therapy is performed under the supervision of a physician. A significant overdose can cause symptoms associated with the anticholinergic effect of ipratropium bromide from the side of the central nervous system, including hallucinations, for the elimination of which cholinesterase inhibitors are prescribed.

Drug Interactions

The drug is incompatible with MAO inhibitors, tri- and tetracyclic antidepressants. it is possible to enhance the sympathomimetic effect of xylometazoline.
Sympathomimetic drugs cause the release of catecholamines, incl. norepinephrine, which has a vasoconstrictive effect, which increases blood pressure. With a significant increase in blood pressure treatment with Otrivin Complex should be abolished, symptomatic treatment should be performed.
