Pharmacodynamics and pharmacokinetics

Pharmacodynamics

Naphthyfine is a topical antimycotic that belongs to the class of allylamines. The mechanism of its action is associated with the ability to suppress the activity of the enzyme squalene-2,3-epoxidase, and thus inhibit the biosynthesis of ergosterol, which is part of the cell membrane of the fungus.
Activity of the drug is manifested in relation to:
    dermatophytes (microsporum, epidermophyton, trichophyton);
    yeast fungi (Pityrosporum, Candida spp.)
    molds (Aspergillus spp.)
    other fungi (eg Sporothrix schenckii).
As for aspergillus and dermatophytes, naphthyfine acts fungicidally, with respect to yeast fungi, depending on the strain of the microorganism, it can exhibit both fungicidal and fungistatic activity.
It has antibacterial activity against Gram (+) and Gram (-) bacteria, which can cause secondary infection, as well as anti-inflammatory action (which allows quickly eliminating inflammation symptoms, especially itching).

Pharmacokinetics

After application to the skin / nail penetrates well into their various layers and creates stable antifungal concentrations in them. A similar effect is observed when using the Systemic Absorption preparation with external application of naphthyfine undergoes no more than 6% of the substance.
The absorbed quantity is partially metabolized and excreted in the urine and intestinal contents. T1 / 2 - 2-3 days.

Indications for use

The drug is intended for the treatment of skin mycoses and skin folds (including between the fingers of the feet and brushes), nails, pityriasis, skin candidiasis, dermatomycosis (with or without accompanying itching).
Exoderil solution is effective in the treatment of fungal lesions in hair growth zones, as well as skin mycoses in areas with hyperkeratosis.

The main contraindication to the use of Exoderil is the intolerance of any of the components (active or auxiliary) entering the composition of the cream or solution.
In addition, the drug is not allowed to be applied to wound surfaces.

What is better - a cream or a solution?

A feature of the drug Exoderil is that, in addition to the antimycotic effect, it also has antibacterial activity: the agent acts on pathogenic microorganisms, often combined with fungal lesions.
The solution penetrates well into the nail, evenly spreading in it and contributing to the destruction of the fungus, removing inflammation and eliminating itching.
In some diseases (in particular, with foot mycoses, when both the nail plate and the skin are simultaneously affected), both dosage forms are often used in combination: in the morning, the nail is treated with a solution, and in the evening a cream is applied to the skin.
In severe cases, in addition to topical therapy, antimycotics of systemic action (tablets, solution for infusions, powder for the preparation of oral suspension) can be prescribed.

For children

Experience with clinical use in children is not available, and therefore it is not recommended to prescribe drops / cream of this category of patients.

Application in pregnancy

If the product is used in accordance with the recommendations given in the instructions, its effect on the fetus / newborn is unlikely.
The results of studies of teratogenicity are evidence that the risk of any embryotoxic effect is absent.
During pregnancy and during breastfeeding, the medicine can be used only after a careful evaluation by the doctor of the ratio of the expected effect of therapy / risk.
Nursing women should avoid getting the solution / cream on the skin and in the child's digestive tract.

Reviews of the Exoderil

Reviews about the cream Exoderil, as well as feedback on the solution Exoderil, on the forums you can meet different: some people this drug helps restore the health of nails and skin, someone, on the contrary, even after 2-3 months of using the drug, no improvement notices.
Experts say that you can get rid of nail / skin fungus only if the treatment is done correctly.
Since with foot fungal lesions, a pathological process spreads to nails in about half the cases, it is advisable to combine ointment and drops: for example, the solution is applied in the morning with nail fungus, and the skin and affected nail plates are treated with cream for the night.
Any external means (drops, cream or varnish) from the nail fungus should be applied regularly and long course.
It is very important (especially during the treatment) to take care of the nails / skin properly (for example, once a week to arrange nails for baths with soda and laundry soap), do not forget about personal hygiene and the need to disinfect all surfaces that come into contact with the fungus.
In severe forms of fungal infection, topical antimycotic agents (Exoderil or its analogs) are combined with systemic drugs to achieve a positive effect.
