Pharmacological group
Antifungal drug for local use.
Pharmacological properties
Terbinafine is the active substance Fungoterbine cream, is an allylamine and has a wide spectrum of antifungal action.
Terbinafine disrupts the early stages of ergosterol biosynthesis in fungi. This leads to a deficiency of ergosterol and to the intracellular accumulation of squalene, which causes the death of the fungal cell. The action of terbinafine is carried out by inhibiting the enzyme squalene epoxidase, located on the cell membrane of the fungus. This enzyme does not belong to the cytochrome P450 system, therefore, terbinafine does not affect the metabolism of hormones or other drugs.
Terbinafine has a fungicidal effect against dermatophytes, molds and some dimorphic fungi. Activity against yeast-like fungi, depending on their species, may be fungicidal or fungistatic.
Urea is the second active component of Fungoterbine cream, has a softening, exfoliating and moisturizing effect, is able to increase dermatopenetrantness (penetration into the skin) and create high concentrations of terbinafine in the skin. Urea helps to bind water and soften the stratum corneum.
Pharmacokinetics
With topical application of the cream less than 5% of the dose is absorbed, thus the systemic effect of Fungoterbine cream is minimal. When applied once on a skin of 30 cm2, 3.5% of the dose is detected in the urine. The drug quickly penetrates the skin and accumulates in the sebaceous glands.
Indications for use
Treatment and prevention of fungal diseases:
Fungal infections of the skin caused by dermatophytes such as Trychophyton (for example, T.rubrum, T.mentagrophytes, T.verrucosum, T.violaceum), Microsporum canis, and Epidermophyton floccosum.
Skin infections caused by yeast-like fungi of the genus Candida (for example, Candida albicans).
Pityrosporum versicolor, caused by Pityrosporum orbiculare (also known as Malassezia furfur).
Contraindications
Hypersensitivity to the components of Fungoterbine cream, children's age (up to 2 years).
Pregnancy and breastfeeding
In experimental studies, the teratogenic properties of terbinafine have not been identified. Use of the drug during pregnancy is possible in cases where the potential benefit to the mother outweighs the risk to the fetus.
If necessary, the use of the drug during lactation is necessary to solve the issue of termination of breastfeeding.
Dosage and administration
The drug is applied to the skin once or twice a day. Before applying the cream must be cleaned and dried the affected areas. The cream is applied with a thin layer on the affected skin and lightly rubbed. To improve the effectiveness of treatment may use occlusive dressings. For infections accompanied by diaper rash (under the mammary glands, in the interdigital spaces, between the buttocks, in the groin area), the places of application of the cream can be covered with gauze, especially at night.
The average duration of treatment:
Ringworm of trunk, legs: 1 week 1 time per day.
Ringworm stop: 1 week 1 time per day.
Skin candidiasis: 1 or 2 times a day for 1-2 weeks.
Multi-colored lichen: 1 or 2 times a day for 2 weeks.
Side effects of Fungoterbine cream
In rare cases, there was redness, itching or burning sensation at the site of application of the cream, however, these phenomena rarely led to the need to discontinue treatment.
The above symptoms should be distinguished from allergic reactions, for example, urticaria, which occur rarely, however, when they appear, it is necessary to stop treatment.
Overdose with Fungoterbine cream
No cases of drug overdose have been reported. In case of accidental ingestion, one can expect the development of the same side effects as overdose of Fungoterbine tablets, including: headache, nausea, dizziness, gastralgia, frequent urination, rash.
Treatment: activated carbon, if necessary, carry out symptomatic therapy.
Phenomena of urea overdose when applied externally are not known.
Interaction with other drugs and dietary supplements
Any drug interactions when using terbinafine is not known externally.
