Local anti-inflammatory, decongestant, antipruritic.

Pharmacodynamics and pharmacokinetics

Emulsion Locoid Crelo is a synthetic, non-halogenated therapeutic agent from the group of glucocorticoids, used in dermatologists as a local (external) drug.
A feature of the active ingredient of the emulsion - hydrocortisone is the esterification of its molecule with a butanic acid residue (butyrate), which resulted in a therapeutically significant increase in the effectiveness of the drug in comparison with natural hydrocortisone. The use of Locoid Crelo is accompanied by rapid relief of the negative symptoms of many skin diseases, including itching, swelling and inflammation.
The recommended therapeutic doses of the drug do not affect the normal functioning of the hypothalamic-pituitary-adrenal system. The use of elevated dosages of hydrocortisone butyrate for a long time may cause some increase in its serum level, especially when applying emulsion for occlusive dressings. As a rule, such an increase in the plasma concentration of hydrocortisone butyrate is not accompanied by a decrease in the capacity of the pituitary-adrenal system. In the case of cancellation of the drug, the production of its own cortisol quickly returns to normal.
When applying the emulsion, the accumulation of hydrocortisone butyrate in the epidermis is noted, its main concentrations are observed in the granular cutaneous layer. The drug is characterized by a slight systemic absorption with a minimal amount of the active ingredient of the therapeutic agent being absorbed into the systemic blood stream unchanged.
Metabolism of the main part of hydrocortisone butyrate occurs directly in the epidermis and later in the liver, with the release of hydrocortisone and other metabolites.
A certain amount of unchanged hydrocortisone butyrate, like its metabolites, is excreted from the body with the help of the intestines and kidneys.

Instructions for use

The instruction for the Locoid Crelo permits the use of the emulsion exclusively externally.
This drug form of the drug Locoid is designed to treat acute skin inflammation and is especially suitable for application to wet areas of the skin or on the skin with abundant hair (including the scalp).
The dosage therapeutic regimen, depending on the localization and severity of skin lesions, involves applying a thin layer of emulsion to the problem areas of the skin, with a frequency of use from 1 to 3 times a day. With signs of positive dynamics of treatment, you can reduce the frequency of application of the emulsion to 2-3 times a week. Locoid Crelo is best applied using light massaging movements of the hand. If necessary, after the procedure of applying the drug, an occlusive bandage can be applied to the surface of the problem skin with mocclusion. Excess amount of emulsion is removed from the skin surface with warm water. The total weekly dose of the drug is best limited to 30-60 grams.

Overdose

Data on the possibility of an overdose emulsion Locoid Crelo no. Prolonged use of high dosages of the drug may cause the occurrence of side effects inherent in glucocorticoids or the strengthening of already existing negative effects. With the development of manifestations of hypercorticism, the Locoid Crelo should be abolished.

Interaction

There is no reliable information about the interaction of locally applied hydrocortisone butyrate with other medicinal products.

Storage conditions

The emulsion should be stored in a factory polyethylene bottle at an ambient temperature of up to 25 ° C.
Shelf life - 2 years.

special instructions

As with the use of other local glucocorticoids, the duration of use of Locoid Crelo should be limited to the minimum possible time, with the appointment of a therapeutic course sufficient to stop the negative skin process.
It is better to avoid applying emulsion to the periorbital area, as this practice of using the drug can lead to the formation of glaucoma.
Systematic exposure of hydrocortisone butyrate to the conjunctiva can cause increased intraocular pressure.
If it is necessary to treat skin lesions with severe lichenification, hyperkeratosis or dryness, it is preferable to use other medicinal forms of Locoid (ointment or Lipocream).
It is advisable not to use the emulsion of the drug in the presence of atrophic skin changes.
The development of side effects of a local and systemic nature is most often observed when the drug is applied on extensive skin areas or applying it under occlusive dressings. Children also suffer from the formation of various negative effects of glucocorticoid therapy.
If treatment with Locoid Crelo is ineffective for 14 days, the diagnosis or prescribed therapy should be reviewed.

Children

Locoid Crelo in the form of an emulsion can be prescribed to children from 3 months of age.
When using the drug to treat children and applying it to the face or under occlusive dressings, the duration of the therapeutic course should be shortened to the lowest possible.
The area of skin of the child, on which it is permissible to apply emulsion, should be as much as 20% relative to the total area of the body.
When using local glucocorticoids in childhood, the possible oppression of the functionality of the adrenal cortex develops faster. Also, such therapy can lead to a decrease in growth hormone excretion. For this reason, prolonged treatment with Locoid Crelo requires observation of the growth and weight of the child, as well as a periodic check of the serum cortisol level.

In pregnancy and lactation

Appointment of Locoid crelo for pregnant and lactating women requires special care and a careful correlation of the benefits / risks of such therapy.

Reviews

When using the emulsion of this remedy strictly according to the indications and in a minimum time frame, the reviews about Locoid Crelo are practically all positive. Patients using this drug, note the high and rapid effectiveness of its action and ease of use. Occurrence of side effects inherent in Locoid Crelo is most often observed during long-term therapy with high doses of emulsion, therefore, the use of the drug is best limited to the minimum possible time.
