Pharmacodynamics and pharmacokinetics
Pharmacodynamics
The drug is a quinazoline derivative and belongs to the group of α1-adrenoblockers. The mechanism of action is the selective effect on postsynaptic adrenoreceptors that are located in the urethra (prostatic part) and the bottom of the bladder. Alfuzozin affects the tissues of the prostate gland (smooth muscle) and thereby reduces the resistance of urine outflow from the bladder and normalizes the parameters of urination (reducing the tone of the urethra, improves the emptying of the bladder). In this case, the volume of urine released and the decrease in its residual volume increase.
Pharmacokinetics
When receiving Dalfaz SR, which provides a sustained release of the active ingredient of alfuzosin, the average bioavailability of the drug as compared with the form of the immediate release preparation increases to 104%. Binding to blood proteins is about 90%. Metabolised in the liver. T1 / 2 is 9 hours. It is excreted through the kidneys, mostly unchanged and through the intestines in the form of pharmacologically inactive metabolites.
Indications for use
Dalfaz Retard and Dalfaz SR - symptomatic treatment of benign prostatic hyperplasia;
Dalfaz SR - as an auxiliary drug for acute retention of urine with prostatic hyperplasia during catheterization.
Contraindications
High sensitivity to the drug, liver disease, kidney, hypotension, intestinal obstruction due to the presence of castor oil in the preparation.
Side effects
Drowsiness, dizziness, asthenia, headache, hypotension, tachycardia, attacks of angina in the presence of the patient's IHD, nausea, chest pain, dry mouth, abdominal pain, itching, rash, diarrhea, rhinitis.
Dalfaz, instructions for use (Method and dosage)

Interaction

with drugs-blockers of α1-adrenergic receptors (minoxidil, prazosin, urapidil) because of the high risk of developing severe postural hypotension;
with inhibitors of CYP3A4 (itraconazole, ketoconazole, ritonavir) because of the risk of increasing the concentration of alfuzosin in the blood;
with drugs that have hypotensive effect because of the risk of increased hypotensive effect.
