Composition

In 1 ml of eye drops Dorzopt contains 20 mg of active dorzolamide (in the form of hydrochloride) and auxiliary compounds: hyethellosis, mannitol, monohydrate of citric acid, benzalkonium chloride, sodium hydroxide, dist. water.

Pharmacodynamics and pharmacokinetics

The drug is a local inhibitor of carbonic anhydrase. The mechanism of action is based on oppression of the secretion of the aqueous humor and slowing the formation of bicarbonate, with a subsequent weakening of sodium and water transfer.
The drug does not cause spasm of accommodation, as well as miosis or hemeralopia.

Information on pharmacokinetics

    Absorption occurs through the cornea, and also, to a lesser extent, through the sclera and limbus. The indicators of systemic absorption are low. Getting into the blood stream, quickly penetrates into red blood cells with a significant content of carbonic anhydrase II.
    The binding with plasma proteins occurs by 33%. It is converted to an N-desethylated metabolite, which is less active with respect to carbonic anhydrase II, but is able to block carbonic anhydrase I. Prolonged application leads to accumulation in red blood cells.
    It is excreted through the kidneys. The withdrawal of the drug has two elimination phases: rapid and subsequent slow, caused by the gradual release of the substance from the erythrocytes.
    Half-life: about 4 months.

Indications for use

Treatment of increased intraocular pressure in patients with ocular hypertension, pseudoexfoliation and open-angle glaucoma (including secondary glaucoma).

Contraindications

    increased or hypersensitivity to the components of eye drops;
    severe renal dysfunction;
    hyperchloremic acidosis;
    age group: under 18 years of age;
    lactation period.

Use with caution

    patients with low endothelial cell density of the eye cornea, due to an increased likelihood of corneal edema;
    patients with hepatic insufficiency;
    at a diabetes.

Side effects of Dorzopt

Local
    burning and itching in the eyes, eyelid edema, paresthesia, lacrimation, impaired vision, conjunctivitis, blepharitis, superficial pinpoint keratitis, photophobia;
    rare cases are the occurrence of iridocyclitis, allergic reactions, increased thickness of the cornea, hypotension of the eye, retinal detachment after AGO (antiglaukomatoznyh operations).
System
    a feeling of bitterness in the mouth, nausea, the development of asthenia, nephrourolythiasis, malignant exudative erythema, migraine, toxic epidermal necrolysis, agranulocytosis, aplastic anemia, skin rashes.

Dorzopt, instructions for use (Method and dosage)

Burying is necessary for 1 drop directly into the conjunctival sac of the eye three times a day.
When combined with topical β-adrenoblockers, 1 drop twice a day is sufficient.
Note: if the therapy includes several topical ophthalmic drugs, the interval between their administration should be at least 10 minutes.

Interaction

    Combination with antiglaucoma drugs (β-adrenoblockers, Pilocarpine, Dipivefrin, Carbachol) causes an increase in the action of Dorzolamide.
    With acetazolamide - increased risk of systemic adverse reactions.
    With high doses of Acetylsalicylic acid - probably an increase in toxicity.
