Composition

Eye drops contain 2.5 or 5 mg of Timolol maleate + excipients (sodium phosphate, 2-aqueous, mono-substituted, sodium hydroxide, benzalkonium chloride, disodium 12-hydrous sodium phosphate, water).

Form of issue

The drug is released in vials of 5 ml, in a cardboard pack of 1 vial with a dispenser-dropper. Contents vials - a solution without color and odor, transparent. Oftan Timolol is released in two dosages, 2.5 or 5 ml of the active substance in 1 ml of the agent.

Pharmacodynamics and pharmacokinetics

The active component of Oftan Timolol is the L-isomer of timolol, available as a maleate salt. The substance belongs to the group of non-selective beta-adrenoreceptor blockers. This isomer is similar to the beta-1 and beta-2 receptors. Due to this, the drug is able to reduce the intraocular pressure, due to a decrease in the intensity of production of the liquid.
With topical application, the product penetrates directly into the ciliary tissue, where moisture production takes place. It is still not completely clear whether timolol affects the blood vessels in the anterior segment of the eye, but with reduced intraocular pressure, there are improvements in the blood circulation of the retina.
The effect of taking the medicine lasts a very long time, up to a day. Sympathomimetic activity and membrane-stabilizing effect of the substance does not possess. The drug does not affect the accommodation and the size of the pupil, does not cause dependence (withdrawal syndrome does not arise).
The active substance is readily soluble in fats, so it quickly enters the target tissues through the conjunctival vessels and the nasal mucosa. After 3-4 hours after taking the maximum concentration and effect of the drug is achieved. The action lasts about a day.
A drug is produced with streams of watery moisture flowing out of the eye. The half-life is 8 hours. That part of the substance that has penetrated into the systemic bloodstream (for example, through the stomach or the walls of blood vessels) undergoes metabolic reactions in the liver tissues, inactive metabolites are excreted through the kidneys. The half-life from plasma is 4 hours, the degree of binding to proteins is 60%. The medicine is able to overcome the blood-brain barrier.

Indications for use

Oftan Timolol appoint:
    for the treatment of various types of glaucoma (open-angle, glaucoma on the aphakic eye, other types of secondary glaucoma);
    at a closed angle glaucoma (as part of complex therapy with miotics);
    with increased intraocular pressure;
    with congenital glaucoma, if other therapeutic measures are not effective enough.

Contraindications of Oftan Timolol

The medicine is not prescribed:
    when allergic to the components of the product;
    if the patient has heart failure;
    at a bronchial asthma and other diseases of respiratory ways;
    up to 18 years;
    with sinus bradycardia;
    with atrioventicular blockade of the second or third degree.

Care should be taken:

    with other diseases of the lungs and heart;
    with diabetes mellitus;
    with thyrotoxicosis or hypoglycemia;
    myasthenia gravis;
    when the drug is combined with other drugs of the same group (beta-blockers).

Side effects

The risk of adverse reactions depends on the degree of penetration of the drug into the blood plasma and the presence of chronic diseases in the patient.

The following local reactions were observed:

    discomfort and pain in the eyes, dryness, redness;
    sensation that there is a foreign body in the eye, photophobia, erosion of the cornea of ​​the eye;
    hypotension, decreased corneal sensitivity, keratopathy;
    conjunctivitis (including allergic), blepharoconjunctivitis, hyperemia;
    deterioration of vision clarity, superficial spotted keratitis;
    exfoliation of the choroid, contact dermatitis of the eyelids, blepharoptosis, hemorrhages in the retina, diplopia, ptosis, keratitis.

Reactions of a systemic nature:

    allergy, anaphylactic shock;
    shortness of breath, cessation of breathing, cough, cyanosis, dyspnea, nasal congestion, bronchial asthma;
    bradycardia, lowering blood pressure, arrhythmia, congestive heart failure, cerebral infarction, pain in the sternum, collapse, stroke, cerebral vascular ischemia, extrasystole;

    nausea, diarrhea, indigestion;

The drug is conjunctival.

Overdose

When an overdose can develop symptoms that are characteristic of beta-blockers: headache, nausea, bronchospasm, vomiting, arrhythmia, dizziness.
In case of accidental ingestion, dyspnoea, dizziness, nausea, diarrhea, vomiting, disturbance of the heart rhythm, general weakness can be observed inside.

In case of ingestion:
    stop receiving;
    rinse the stomach;
    apply (depending on the symptoms) dopamine, noradrenaline, glucagon, atropine or isoprenaline hydrochloride;

    therapy with digitalis, diuretics.

Interaction

Due to the joint administration of the drug with insulin or other anti-diabetic drugs hypoglycemia may develop.
The combination with adrenaline can cause a strong dilatation of the pupils.
Simultaneous reception of nicardipine, diltiazem and medication can lead to increased intraocular pressure.
It is not recommended to combine Oftan Timolol with pilocarpine.
When combined with calcium antagonists, reserpine, cardiac glycosides, beta-blockers, there may be a significant decrease in blood pressure, AV blockade and a slowing of the heart rate.
Timolol can enhance the action of muscle relaxants. The gap between the opening of funds should be at least 48 hours.
Enzymes P450, CYP2D6 take part in the metabolism of timolol, therefore it is not recommended to take other agents, in the metabolism of which the same enzymes (quinidine) participate.
When combined with epinephrine, mydriasis may develop.
The interval between taking clonidine and timolol should be at least two days.
Cimetidine is able to enhance the therapeutic effect of the drug.
