Pharmacodynamics

Ovestin, which is an analog of the female sex hormone, normalizes the state of the vaginal epithelium in the menopausal and postmenopausal period, contributes to the restoration of the physiological pH and microflora of the vagina, increases resistance to infections. At the same time, under the influence of an active ingredient, blood cholesterol levels decrease, the volume of plasma increases slightly, the circulation time of corticosteroids increases.
Estriol, in contrast to other hormonal agents, has a short period of action, retaining in the nuclei of endometrial cells for a short time. With the use of this drug there are no bleeding cancellations, the mammographic density does not increase and the proliferation of the endometrium does not develop.

Pharmacokinetics

With topical application, the active ingredient of Ovestina is quickly and almost completely absorbed into the blood, reaching a maximum plasma concentration 12 hours after intravaginal use. 90% bound to albumin blood, almost does not bind to globulins. Within 18 hours it is excreted by kidneys in the form of metabolites, about 2% is excreted by the intestine, along with feces.

Indications for use

    Age-related changes in the vaginal mucosa resulting from estrogen deficiency (discomfort, itching, dryness, painful sensations during intercourse);
    Prevention of inflammatory pathologies of the genito-urinary tract;
    Urinary incontinence;
    Soreness, increased frequency of urination;
    Cervical infertility;
    Condition after surgery with overvaginal access;
    For diagnostic purposes (with suspicion of the development of the tumor process against the background of atrophic changes in the vaginal epithelium).
    Individual intolerance to the components of the drug;
    Pregnancy;
    Endometrial cancer and other estrogen-dependent malignant tumors;
    Mammary cancer;
    Arterial or venous thromboembolism;
    Acute hepatic pathology;
    Vaginal bleeding of unclear etiology;
    Porphyria.

Contraindications

The drug should be used with extreme caution and under constant supervision of the doctor in the following situations:
    the presence of a risk factor for estrogen-dependent tumors;
    endometriosis;
    uterine leiomyoma;
    benign liver tumors;
    diabetes;
    liver failure;
    jaundice;
    arterial hypertension;
    severe headaches, migraines;
    epilepsy;
    systemic lupus erythematosus;
    bronchial asthma;
    pancreatitis;
    hereditary hyperlipoproteinemia;
    otosclerosis.

Dosing and Administration

When atrophic changes in the vaginal mucosa, vaginal suppositories Ovestin is recommended to be used once a day, at bedtime. With the improvement of clinical symptoms, they switch to the administration of medicines 2 times a week.
In preparation for an operation with overvaginal access, a daily administration of 1 suppository is indicated 2 weeks before the proposed date of surgery. Then, for 14 days after the operation - 1 candle 2 times a week.
For unclear results of cytological examination (for diagnostic purposes), the drug is given every other day for a week (until the next smear is taken).

Drug Interactions

Ovestin in the form of vaginal suppositories is not recommended to be used simultaneously with anticonvulsants, barbiturates and antibacterial drugs.
Estriol is able to enhance the pharmacological effect of theophylline and corticosteroids.

Side effects

    Itching, irritation of the vaginal mucosa;
    Nausea;
    Soreness, discomfort in the mammary glands;
    Headache;
    Increased blood pressure (rarely).

Overdose

In case of an overdose of the drug, nausea, vomiting, and possibly the development of vaginal bleeding. There is no specific antidote. Treatment is symptomatic.

special instructions

When starting the use of the hormone replacement therapy it is necessary to establish an individual and family history, and to conduct a complete clinical examination of the pelvic organs and mammary glands.
If vaginal infections are present, concomitant specific treatment should be performed.
Ovestin does not adversely affect the ability to drive vehicles or perform work that requires a high concentration of attention and speed of psychomotor reactions.

Storage conditions

Store in a dry, protected from light, out of reach of children, at a temperature of 2-25 C. The shelf life of the drug is 3 years. At the end of this period, the use of the medicinal product is prohibited.
