pharmachologic effect

For all forms of Artrosilene, analgesic, antipyretic and anti-inflammatory effects are characteristic.

Pharmacodynamics and pharmacokinetics

The manifestation of anti-inflammatory, analgesic and antipyretic effect is ensured by the ability of ketoprofen to inhibit COX-1 and -2, inhibit the synthesis of PG. In addition, the drug is characterized by anti-bradykinin activity, stabilization of lysosomal membranes and inhibition of their release from the composition of enzymes that contribute to the destruction of tissues in cases of chronic inflammation. The release of cytokines decreases, the activity of neutrophils slows down.
The use of Artrosilene allows you to reduce the morning stiffness, swelling of the joints, increase the volume of movements.
The lysine salt, which is part of the ketoprofen preparation, is a rapidly dissolving molecule with a neutral pH. Therefore, Artrosilene has practically no irritating effect on the digestive tract.
External use of the spray or gel has a local anti-inflammatory, antiexudative and analgesic effect. This high local therapeutic effect on the affected joints, tendons, ligaments, muscles. Treatment with the drug with articular syndrome allows you to ease pain in the joints, both in a calm state and during movement, the morning stiffness and swelling of the joints decrease. It has been established that Artrosilene does not exert a destructive effect on articular cartilage.
When taking capsules, the active substance is fully absorbed from the digestive tract, with bioavailability above 80%. The maximum concentration is reached after 4-10 hours from the moment of taking, depending on the dosage. In this case, the therapeutic effect persists for 4-24 hours. Eating can reduce the concentration of the substance in the body. Inside the body, a complete connection of ketoprofen with plasma proteins was noted. The substance is capable of penetrating the histohematological barriers into the tissue and synovial fluid, where its distribution takes place. Metabolism of ketoprofen occurs in the liver, undergoing glucuronidation with the production of esters and glucuronic acid. Excretion of metabolites is carried out in the urine and an insignificant part with caloric masses.
The effect of the injection solution is manifested after 20-30 minutes and persists for 18-20 hours. The active substance is fully absorbed by binding to plasma proteins. The drug penetrates through the histohematological barriers in the tissues and physiological fluids, evenly distributed in the body. Metabolism occurs with the participation of liver enzymes, which are mainly excreted by the kidneys, for 24 hours.
When using the drug in the form of rectal suppositories, its effect is manifested within 45-60 minutes. The maximum concentration of the substance is directly dependent on the dosage taken. In the body, ketoprofen is fully absorbed, binding to plasma proteins. The penetration of the substance through the histohematetic barriers was noted, being distributed in tissues and important organs. Metabolism occurs in the liver, with the production of esters and glucuronic acid. Excretion of metabolites occurs in the urine.
The use of external means of gel and aerosol is noted by slow absorption. In this case, the effect of the substance appears for a short time and persists for 5-8 hours. Bioavailability is approximately 5%.

Interaction

Simultaneous use of microsomal oxidation in the liver with phenytoin, barbiturates, flumecinol, rifampicin, phenylbutazone, tricyclic antidepressants increases the production of hydroxylated active metabolites.
It has been established that Artrosilene reduces the effectiveness of certain uricosuric agents, enhances the effectiveness of anticoagulants, fibrinolytic agents, antiaggregants, side effects of mineralocorticoids, estrogens, and hypotensive and diuretic drugs.
Joint use with NSAIDs, GCS, corticotropin, ethanol can cause the development of ulcers and bleeding of the gastrointestinal tract, increase the likelihood of deviations in renal function.
When used simultaneously with oral anticoagulants, thrombolytic agents, heparin, cefoperazone, cefamandol antiplatelet agents and cefotetan, the risk of bleeding increases.
In addition, this drug enhances the hypoglycemic effect of insulin and hypoglycemic agents, so with a simultaneous appointment requires a recalculation of the dose. The combination with valproate results in disturbances of platelet aggregation, with verapamil, nifedipine, lithium, and methotrexate, which increases their concentration in the plasma. Combination with santacid and colestyramine can reduce the absorption of Artrosilene.
special instructions
When treating with capsules, injection solution and suppositories, it is required to carry out control of peripheral blood parameters, as well as the state of liver and kidney functions.
When it is necessary to determine 17-ketosteroids, the drug is canceled before the test for 48 hours.
The use of ketoprofen often masks the symptoms of an infectious disease.
Violations of kidney and liver function require lowering of dosage and careful observation of the condition of patients.
Patients suffering from bronchial asthma may experience an attack of bronchial asthma.
When planning pregnancy, it is recommended to refrain from the use of drugs, since it is possible to lower the probability of implantation of the egg.
Aerosol and gel Artrosilene should be applied to the skin without damage. It is important that the drug does not get into the eyes and mucous membranes. Avoid the development of increased sensitivity or photosensitivity if you protect the skin from sunlight during the entire treatment.

Storage conditions

To store the drug is intended a dark place, protected from children, at temperatures up to 25 ° C.
Shelf life - 3 years.

Alcohol and Artrosilene

The simultaneous use of this drug and alcohol strengthens its effect and increases the risk of developing bleeding.

Reviews about Artrosilene

As the reviews on gel Artrosilene show, its effectiveness does not appear in all cases. Messages on the forums show that this drug is often prescribed in the treatment of gout. However, the simultaneous use of capsules and gel bring relief only when the therapy is performed at the initial stage of the disease.
Nevertheless, this drug is very helpful in the treatment of traumatic pains, for example, with bruises. There are reports of how with the help of Artrosilene, patients have eliminated inflammation of the joints. Of course, if pain and other unwanted symptoms persist for a long time, you need to see a doctor instead of trying to choose the medication yourself.
