pharmachologic effect

Relpax - a drug with antimigrenous action. The active ingredient eletriptan demonstrates pronounced selectivity relative to serotonin receptors, which are located in the carotid arteries.
The antimigraine activity of the drug is related to the ability of eletriptan to narrow the blood vessels inside the skull and its inhibitory effect on neurogenic inflammation.

Pharmacokinetics and pharmacodynamics

After oral administration, eletriptan is actively absorbed in the digestive tract, absorption is 81%. The bioavailability level for internal admission is approximately 50%.
The highest concentration in the blood is observed 1.5 hours after internal administration. The maximum concentration of active substance increased by 20-30% if the medicine was taken after a fat meal. With blood proteins, eletriptan binds to 85%.
The half-life is approximately 4-5 hours. Metabolism occurs in the liver, after which the excretion occurs through the kidneys and the gastrointestinal tract.

Instructions for use Relpax (Method and dosage)

Instructions for use Relpax uses the drug inside. In this case, the tablets should be washed down with liquid.
If the patient is just beginning to develop migraine headache, you must take the pill as early as possible. But the expressed efficiency of its action is noted also at reception at the developed attack of a migraine.
Adult patients aged 18 to 65 years are recommended to take an initial dose of 40 mg.
When the headache recur during the day, you can re-take the remedy in a similar dose. If you need to take the pill again, then you can do it no earlier than 2 hours after taking the first pill.
If, after taking the first dose of Relpax, the headache does not decrease within 2 hours, then the second dose of the drug should not be taken.
But if you can not stop the attack, then an effective clinical response may appear in the next migraine attack. If a dose of 40 mg does not allow to obtain the desired effect, the next attack can take Relpax in a dose of 80 mg.
The total daily dose of the drug should not be more than 160 mg.
People suffering from mild or moderate impairment of liver function do not need to adjust the dose.

Overdose

In case of an overdose, the patient may develop arterial hypertension, as well as other cardiovascular system disorders. It is necessary to immediately wash the stomach, symptomatic treatment.
Since the half-life of the active substance is approximately 4 hours, with an overdose it is necessary to observe the patient for at least 20 hours until all the symptoms disappear.
There is no data on the effect of peritoneal dialysis and hemodialysis and on the concentration in the blood of eletriptan.

Interaction

The pharmacokinetics of eletriptan can be influenced by the simultaneous use of other drugs. If Erythromycin and Ketoconazole are simultaneously administered, Cmax eletriptan increased 2 and 2.7 times, respectively. The half-life of eletriptan also increased.
Therefore, Relpax is not prescribed concomitantly with Itraconazole, Ketoconazole, Clarithromycin, Erythromycin, Josamycin, as well as with protease inhibitors.
There was no interaction between Relpax and tricyclic antidepressants, β-blockers, SSRIs and flunarizine.
If you simultaneously receive Relpax, Propranolol and Verapamil or Fluconazole, then the maximum concentration of eletriptan increases. However, such changes do not have clinical significance.
If, after taking Relpax in 1-2 hours, caffeine or ergotamine is taken, this leads to a small but additive increase in blood pressure. Therefore, funds that contain ergotamine or ergotamine-like drugs should not be administered within 24 hours after Relpax's medication. Similarly, Relpax can be taken only 24 hours after taking such medications.
With simultaneous reception of eletriptan with drugs that demonstrate serotonergic activity, the risk of manifestation of serotonin syndrome increases. With this combination, patients need to be closely monitored.
