Form of issue

Pharmacodynamics and pharmacokinetics

Loperamid is a synthetic derivative of piperidine. It affects the peripheral opioid receptors, reducing the production of acetylcholine and prostaglandins. Thus, there is a decrease in the propulsive peristalsis of the small intestine, defecation occurs with a lower frequency, and the feces acquire a more firm consistency.
Indications for the use of the drug are frequent bouts of diarrhea. This is due to the ability of the means to increase the tone of the anal sphincter, to reduce the frequency of defecation.
After taking the pills, they quickly enter the bloodstream, after being absorbed into the digestive tract. After 4-5 hours after administration, loperamide reaches its maximum concentration in the blood plasma and, in a rather small amount, can penetrate the blood-brain barrier.
It undergoes metabolic reactions in the liver, is excreted after 9-14 hours with colonic masses, a small amount - through the kidneys.
Due to the fact that the drug is metabolized in the liver, it is necessary to consult a doctor and adjust the dose for patients with liver diseases.

Contraindications

Lopedium, instructions for use (Method and dosage)

Capsules take whole, do not bite, wash down with water.
According to instructions for the use of Lopedium, it can be prescribed to adults and children, starting at six years of age.
For adults, with chronic or acute diarrhea, the initial dosage is 4 mg, after each trip to the toilet (act of defecation). The dosage can then be reduced to 2 mg. The maximum drug taken for a day is 16 mg.
For children, initially prescribed 2 mg, a maximum of 6 mg per day.
The course of treatment ends when the state is reached, when the time between two acts of defecation is at least 12 hours.

Overdose

Symptoms of overdose is a strong suppression of the central nervous system, including urine retention, intestinal obstruction, severe CNS depression, stupor, miosis, convulsions. It should be remembered that children are more sensitive to the drug than adults.
Naloxone is a specific antidote to the drug. You may need to re-administer the antidote. Do not exclude and symptomatic therapy, gastric lavage, reception of enterosorbents.

Interaction

Do not combine medicine with other drugs that depress the central nervous system.
Take with caution with P-glycoprotein inhibitors (quinidine, ritonavir), inhibitors of CYP3A4 (Gemfibrozil), Itraconazole, Desmopressin, ketoconazole.
