pharmachologic effect
Chondroprotective. Alflutop is a medicine that regulates metabolism and helps to restore damaged cartilage tissue. Relieves pain and inflammation.
Pharmacodynamics and pharmacokinetics
Reduces the activity of hyaluronidase, an enzyme that plays a major role in stabilizing hyaluronic acid and collagen of the second type and participating in the destruction of the intercellular matrix.
Reduces the permeability of capillaries, inhibits the destruction of the macromolecular structure of the basic substance of connective tissue, stimulates the regenerative processes in loose fibrous connective tissue and cartilaginous tissue of the joints.
The proteoglycans contained in the extract of marine fish determine the trophic effect and increase the hydrophilicity of the cartilage (and, consequently, its elastic-elastic properties), as well as the uniformity of bone tissue and the height of the cartilage.
Due to these properties of Alflutop in the patient:
decreases the intensity of pain (motion, at rest, while climbing the stairs);
decreased local swelling;
the severity of contracture decreases.
The use of the drug Alflutop helps to alleviate motor activity and increase the volume of movements of the affected joint. Also in patients receiving this drug, the serum glycoprotein content of seromucoids and globulin, fibrinogen, number of leukocytes and C-reactive protein in the synovial fluid decreases.
At the end of the six-year course of treatment, the level of hyaluronic acid in the synovial fluid is doubled. Anti-inflammatory effect is observed after a week of treatment.
Indications for use: from which injections Alflutop?
The injections of Alflutop medication are prescribed for degenerative rheumatic joint diseases (indications for use: osteochondrosis, arthrosis of small joints, gonarthrosis, coxarthrosis), ossification (chondral and endochondral), traumatic dystoses, periodontopathy (the drug is used as a supplement to the main therapy).
Contraindications
Contraindications include pregnancy, lactation, childhood and adolescence, allergy to the components contained in the medicine.
Side effects
Side effects of the drug appear as:
weak and short-term myalgia;
itching dermatitis, sensations of burning, redness at the injection site;
short-term enhancement of joint pain after the introduction of the solution into the joint.
In rare cases, hypersensitivity reactions are possible.
Instructions for use of Alflutop
In Sport-Wiki (Wikipedia of scientific bodybuilding) it is indicated that Alflutop is intended for intramuscular, paravertebral, periarticular, intraarticular administration, and also for injection into painful points (with fibromyalgia).
With polyosteoarthritis and osteochondrosis treatment course is designed for 20 days. The contents of the ampoule (1 ml) is injected deep into the muscle once a day.
If the pathological process has spread to large joints, injections should be done in each affected joint. Single dose 1-2 ml. The course of treatment provides for 5-6 intra-articular injections, which are performed once every 3-4 days.
After the last dose has been introduced into the joint, Alflutop continues to be administered intramuscularly for another 20 days. After six months, the course should be repeated.
With sequestered spinal hernia, Alflutop is prescribed as an adjunct to the main treatment: the use of chondroprotectors in this pathology can prevent further degenerative changes in the discs.
Drug therapy for hernia sequestration is performed using:
NSAIDs;
diuretics (these drugs are prescribed a short course to eliminate excess fluid and reduce pressure on the spine);
analgesics (in a number of cases it is advisable to do a novocaine blockade);
vitamins of group B;
muscle relaxants;
drugs that improve blood supply and nutrition of tissues near the source of inflammation;
anticonvulsants.
Currently, Alflutop is often prescribed for the treatment of complicated by sequestration of the pulpous nucleus of a hernia. The effect of its use is noticeable within a week after the start of the course. Contained in the solution of natural substances activate the synthesis of cartilaginous tissue and have a positive effect on the homogeneity of the bone.
As a result - the patient with sequestered hernia decreases the intensity of pain, the inflammation focuses and the mobility of the vertebrae increases.
If after 2-3 weeks of treatment there is no positive dynamics, a decision is made about the need for a planned operation.
How correctly to prick a preparation in a joint?
After puncturing the joint bag, make sure that the syringe needle is positioned correctly. For this, an attempt should be made to aspirate the synovial fluid.
However, in most cases this is quite problematic, since the filling fluid of the joint due to its high viscosity can block the tip of the needle with dense particles.
In such a situation, it is possible to judge the correct placement of the needle in the joint cavity only by the relative painlessness and ease of administration of the drug.
If there is difficulty with aspiration of the fluid, you do not need to rotate the needle of the syringe, as this can injure the articular bag, intraarticular ligament, cartilage or synovial membrane. As a result, hemarthrosis may develop.
In cases where the evacuation of the synovial fluid does not cause difficulties, it should be removed (the maximum possible number). This reduces the pressure in the joint cavity, reduces pain (including caused by the introduction of an additional volume of injection solution) and facilitates the injection.
With synovial fluid, in addition, the aggressive substances contained in it (in particular, proteases) are removed, the concentration of the active substance of the drug in the joint increases and its leakage through the lymphatic ways slows.
The medicine should be administered slowly, without exerting excessive effort.
If the tip of the needle does not rest on the dense tissues inside the joint, the injection is usually of little or no pain. Too much resistance when injected and / or intense pain can indicate that the needle has not hit the joint cavity. In this case, it should be a little pull on yourself or, conversely, move a little deeper.
Overdose
In predisposed patients with overdose, hypersensitivity reactions, sometimes severe, may develop.
In such situations, the drug is canceled. Further treatment measures are symptomatic.
Interaction
Drug and other types of Alflutop interactions have not been established to date.
Storage conditions
In the dark place at a temperature of 15-25 ° C.
Shelf life - three years.
special instructions
The risk of developing hypersensitivity reactions increases with intolerance to seafood.
What is better - Dona or Alflutop?
The medicine (and its analogs) is the corrector of the metabolism of the cartilaginous and bone tissue. In contrast to Alflutop and Hondrolone, the drug Dona uses glucosamine as the active substance.
Like chondroitin sulfate, glucosamine, possessing tropism for cartilage and a complex mechanism of action, inhibits the destruction of cartilaginous tissue and stimulates its synthesis.
Both of these substances are natural components of cartilage. The first is an integral part of the molecule of proteoglycan chondroitin sulfate and is a high-molecular polysaccharide from the group of proteoglycans. The second is necessary for the synthesis of proteoglycans.
Both glucosamine and chondroitin (in the form of sulfate) have their own analgesic and anti-inflammatory effect, which is realized not by suppressing the synthesis of Pg, but by inhibiting the activity of lysosomal enzymes and superoxide radicals.
Both of these substances have a positive effect on cartilage due to the ability to suppress catabolic and stimulate anabolic processes in it. Nevertheless, it is extremely difficult to finally assess the degree of their influence on the course of osteoarthritis.
The minus of the preparation of Dona can be considered that it is produced in tablet form. That is, there are doubts that his active substance reaches the focus of the lesion in the required concentration. It is difficult to judge the effectiveness of Alflutop due to the fact that the composition of the extract of marine fish contained in it is quantitatively unknown.
For children
To date, there is no evidence base that would confirm the safety and effectiveness of Alflutop in pediatric practice, and therefore the drug is not prescribed to persons under 18 years of age.
Compatibility with alcohol
The drug is designed for damaged cartilage and bone tissue. The injection is done directly in the affected joint. Penetrating into the affected tissue, the active substance inhibits the destruction of the joint and stimulates the formation of a healthy cartilaginous tissue.
In order for the articular cartilage to function normally, it must absorb liquid into the joint cavity and then give it back under load. The process of destruction of cartilaginous tissue is associated with a shortage of water. As a result, the cartilage loses its ability to recover.
One of the properties of alcohol is its ability to remove water from the body, which can negate the effectiveness of the drug.
However, in the manufacturer's instructions there is no definitive prohibition on alcohol consumption during the treatment with Alflutop.
Specialists also recommend remembering that alcoholic beverages can enhance the effect of the drug, thus provoking an overdose to them, and may in conjunction with the drug cause swelling of the respiratory tract.
In pregnancy
The use of medication during pregnancy and breastfeeding is contraindicated.
Reviews about Alflutop
The doctors 'comments about Alflutop, as well as the patients' opinions on the forums, are often contradictory. Someone puts the effectiveness of the drug in doubt, someone, on the contrary, considers it a very effective tool.
As a minus, patients note mainly the high cost of the drug mixture and sometimes not immediately noticeable result from its use.
The experts' feedback allows us to draw the following conclusion: the effectiveness of the drug directly depends on how timely the treatment was started, and what resources were assigned to the patient in addition to Alflutop.
To date, it has been proven that during 3 years, patients who received joints for joints did not have x-ray progression of osteoarthritis, and the effect was preserved for several months.
Other studies conducted in a group of nearly 4,000 patients with osteoarthritis of the hip and knee joint showed that the use of chondroitin does not allow achieving a clinically significant effect. In patients who took chondroitin, compared with patients who received placebo, there was no decrease in joint pain, as well as slowing down the narrowing of the joint space.
Nevertheless, since many people are convinced that the chondroprotectors help them, and also because there are no undesirable effects due to their use, doctors see no harm in continuing the treatment until it seems useful to the patient.
Since chondroprotectors rather inhibit the destruction of cartilage, and do not restore it, these drugs should be used not for the period of exacerbation of the disease, but for preventive purposes (for example, to prevent the development of degenerative changes that occur in the hernia of the spine).
