Nise instruction for use

Nise is a medicinal preparation in the form of a gel, which has analgesic and anti-inflammatory effect, which is used for various types of arthritis, spondylitis, osteoarthritis, osteochondrosis, myalgia and pain syndrome.
Nimesulide, which is an active ingredient in the Nise drug, belongs to the class of sulfonanilides. According to the classification, it belongs to the group of non-narcotic analgesics, including non-steroidal anti-inflammatory drugs:
    Salicylic acid and its derivatives - mesalazine, salicylamide, choline salicylate and others.
    Pyrazolones - metamizole sodium, propiphenazone.
    Butylpyrazolidines are clofezone, phenylbutazone.
    Derivatives of acetic acid - diclofenac, indomethacin, ketorolac, etodolac and others.
    Oksikam - lornoxicam, meloxicam and others.
    Propionic acid derivatives are ibuprofen, dexketoprofen, phenoprofen and others.
    Phenamates - mefenamic acid, ethofenamate.
    Coxiba - parecoxib, rofecoxib, celecoxib, etorikoksib.
    Anilides are paracetamol, propacetamol.
    Other drugs to which nimesulide belongs.
It has a pronounced analgesic, anti-inflammatory and antipyretic effect. The mechanism of activity is the selective inhibition of the enzyme - cyclooxygenase type 2, which takes part in the synthesis of one of the classes of inflammatory mediators - prostaglandins. Due to the inhibition of this enzyme, the production of mediators and the severity of the inflammatory reaction decrease.
Nise in the form of a gel refers to the OTC group of drugs and is dispensed without a prescription.

Form of issue and composition

The dosage form is represented by a gel for external use by 20 and 50 g.
The production belongs to the Indian company Dr. Reddy's Lab. Ltd.

Indications for use

    rheumatoid or psoriatic joint damage;
    articular cider with gout;
    Bechterew's disease;
    osteochondrosis accompanied by radiculopathy;
    deforming osteoarthritis;
    pain in the muscles of rheumatic and other origin;
    tendinitis, tendosynovitis, bursitis and other inflammatory processes in soft tissues;
    pain in the rehabilitation period after surgery, with trauma, in the joints, with the root syndrome.

Contraindications

    hypersensitivity to gel components;
    a combination of bronchial asthma or polyps of the nose with intolerance to acetylsalicylates;
    erosion or ulcers of the digestive tract, bleeding of any origin;
    exacerbation of granulomatous enteritis, ulcerative colitis and other intestinal diseases;
    pathologies associated with impaired coagulation;
    uncompensated heart failure;
    severe liver dysfunction;
    the propensity to develop toxic effects on the liver when using nimesulide;
    simultaneous administration of drugs with hepatotoxic effects;
    alcoholism and drug addiction;
    severe renal dysfunction or other kidney pathologies;
    hyperkalemia;
    rehabilitation period after coronary bypass;
    pregnancy and lactation;
    age up to 12 years.

Observance of precautions is necessary in such cases:

    ischemia of the heart;
    cerebrovascular disease;
    congestive heart failure;
    dyslipidemic or hyperlipidemic syndrome;
    diabetes;
    pathology of peripheral vessels;
    smoking;
    renal dysfunction;
    ulcers of the digestive tract;
    availability Helicobacter pylori;
    elderly age;
    taking non-steroid drugs for a long period;
    alcohol abuse;
    other serious diseases;
    simultaneous use of anticoagulant and antiplatelet agents, oral glucocorticosteroids, antidepressants from the group of inhibitors of the capture of serotonin.

Dosing and Administration

Nise is applied externally, to open areas of the body. Do not use it under a bandage or when wearing tight clothing. Before and after applying the gel, you should thoroughly wash your hands. After use, the tube must be closed tightly.
The gel is applied in a thin layer, a strip about 3 cm above the inflamed area, 3-4 times a day. The duration of treatment is individual, an average of 1-2 weeks.

Side effects

When applying the gel locally, there may be various skin irritations, redness, rash, desquamation, itching.
Systemic exposure can develop as a result of long-term use or use on vast areas of the body:
    the digestive tract: heartburn, nausea, vomiting, stool disorders, stomach pain, gastrointestinal ulcers;
    nervous system: headache, dizziness;
    excretory system: fluid retention, hematuria;
    allergic manifestations: skin rashes, anaphylaxis;
    laboratory indicators: decrease in the number of platelets, leukocytes, anemia, agranulocytosis, increased activity of enzymatic systems of the liver, prolongation of bleeding time.
When these or other adverse events appear, the drug should be discontinued and appropriate treatment initiated.

Special instructions and drug interactions

    the drug should be applied to intact skin, avoiding getting the gel on the wound surface, as well as in the eyes and mucous membranes;
    the drug is applied externally, to open areas of the body. Apply it under occlusive dressings;
    every time, before and after using the gel, you should thoroughly wash your hands;
    tightly close the tube cover after applying the drug;
    possible interaction with drugs that are competitors in relation to binding to blood proteins;
    Nimesulide is able to enhance the action of drugs that reduce blood coagulability;
    Nimesulide may reduce the therapeutic effect of furosemide and antihypertensive agents;
    under the influence of nimesulide may increase the undesirable effects of methotrexate;
    Nimesulide with simultaneous application can increase the level of lithium in the blood;
    when treated with hydantoin and sulfanilamide preparations in combination with nimesulide, patients are carefully monitored by a specialist and regularly examined;
    the substance can cause increased effects of cyclosporin on the kidneys;
    simultaneous reception of glucocorticosteroids or antidepressants from the group of inhibitors of the capture of serotonin increases the likelihood of developing gastrointestinal bleeding.

Terms and conditions of storage

Shelf life - 2 years. Store the medicine at room temperature, in a dry, dark place, out of reach of children.
