Composition
The composition of Omez D includes two active substances: domperidone and omeprazole (in the form of pellets, which covers the shell, soluble in the intestine). Auxiliary ingredients are magnesium stearate, MCC, carboxymethyl starch sodium, silicon colloidal dioxide, sodium lauryl sulfate, talc.
Form of issue
Produced in the form of hard capsules made of gelatin. Capsules have a white body and a lid of light purple. On the capsule there is an inscription "Omez D" and the logo "DR.REDDY'S". Inside the capsule is an off-white powder.
Capsules are placed in strips of 10 pieces, which fit into cardboard packs.
pharmachologic effect
The Omez D combination drug contains omeprazole and domperidone, which is a blocker of central action of dopamine receptors.
Under the influence of domperidone, the peristalsis of the duodenum and antrum of the stomach increases. When the process of gastric emptying slows down, the action of this substance makes it possible to activate the process. Domperidone stimulates the tone of the lower esophageal sphincter, prevents vomiting and nausea. Also, the substance stimulates prolactin production by the pituitary gland.
Omeprazole blocks the last stage of hydrochloric acid secretion. As a consequence, basal and stimulated secretion is reduced.
In combination, omeprazole reduces the secretion of hydrochloric acid, and domperidone increases the tone of the esophageal sphincter and activates the emptying of the stomach. As a consequence, the aggressiveness of gastric juice decreases and the contents of the stomach are thrown into the esophagus.
Pharmacodynamics and pharmacokinetics
Domperidone through the BBB penetrates poorly, therefore when using domperidone extrapyramidal side effects develop rarely. The substance is absorbed quickly after ingestion. Bioavailability is approximately 15%. The greatest concentration in the plasma - after 1 hour. It is distributed in all tissues, with plasma proteins binds about 90%. Metabolism occurs in the liver and in the wall of the intestine. It is excreted through the intestine and kidneys.
Omeprazole after oral administration begins to act after 1 hour, after 2 hours the maximum effect is achieved. The exposure lasts 24 hours. There is a rapid adsorption of the substance from the gastrointestinal tract, the greatest concentration in the plasma - after about 1 hour. Bioavailability is 30-40%. With plasma proteins, the bond is approximately 90%. Almost completely omeprazole is metabolized in the liver. The main part is excreted through the kidneys, another 30% - with bile.

Omeprazole can reduce the absorption of esters, iron salts, itraconazole and ketoconazole, ampicillin. Also this substance can reduce the excretion of anticoagulants of indirect effects, phenytoin, diazepam. Sometimes it is necessary to reduce the doses of these drugs.
