Heptral relates to a group of medicinal medicines, the action of which is directed to the regeneration and normalization of liver functions. He also corrects the outflow of bile and most positively affects its composition and structure. Thanks to this, both the general condition of the liver and its work are greatly improved. To a large extent, heptral is able to reduce the degree of damage to liver cells by serious diseases, such as cirrhosis, hepatitis and others. Thanks to a soothing and mild anti-depressant effect, the patient's mental state during the therapy improves significantly, which is also important.

Composition

Active component of the drug is ademethionine. The substance belongs to the coenzyme group - substances that are necessarily present in the enzymes and affect the course of various reactions in the human body. Ademethionine takes a direct part in the transport of molecules of substances of methyl groups throughout the body. When the body functions, this coenzyme is synthesized in many tissues and liquid forms of the body.
Ademetionin was discovered in the 50s of the last century by the Italian chemist Cantoni. The substance obtained as a result of synthesis stabilizes the volume of substances, and also promotes the synthesis of coenzyme in the liver and brain. Because of this, the shortage of substances is completely replenished.
It is believed that good absorption of ademetionin does not occur without the vitamins of group B (B-12 is particularly important here) and folic acid.
The tableted form of Heptral is equipped with a variety of auxiliary substances. It is noteworthy that in the case of lyophilizate for dissolution such substances are not used. However, some additional components are contained only in the solvent itself. It consists of:
Water without ions of impurities, that is, deionized.
    sodium hydroxide
lysine - amino acid, which is part of the proteins.
Powder is painted white, a slight yellow tint is allowed - other shades are not possible. Consistency must be necessarily homogeneous, without all kinds of foreign impurities and impurities.
The powder is placed in a transparent bottle and, in combination with it, there is a solvent. It is a clear, colorless liquid, sometimes having a faint yellowish hue. The solvent is also sealed in a clear glass vial.
If at the opening of the pack with the medicine it is found out that there is a sediment in the solvent or foreign substances, it is not used for the dilution of the powder.
The packaging of the medicinal preparation for injection includes 5 vials of powdered substance and the same number of ampoules with a liquid to dissolve it.

Mechanism of action

The action of Heptral is aimed at replenishment of ademetionin deficiency in the body by stimulating its synthesis, especially in the brain and human baking. It also increases the amount of amino acid glutamine in the liver, and cysteine and taurine in the blood plasma. Under the action of the drug concentration in the serum of methionine, on the contrary, decreases, thereby contributing to the metabolism of the liver.
Has a choleretic effect, which can persist for three months after the withdrawal of treatment.
Extremely effective is heptral for poisoning with hepatotoxic drugs (these are drugs that use the higher the dosage, the higher the hepatotoxic effect).
Important! Patients with opiate addiction, who are clearly affected by liver damage, are shown to have heptral to improve their functioning.
The antidepressant effect of the drug is already evident from the first week of its use.

Indications for use

Heptral injections are prescribed in such cases:
Fatty degeneration of the liver is a chronic disease in which liver cells are converted to fat.
    Chronic hepatitis.
Toxic liver damage. Moreover, the factors can be absolutely different: alcohol poisoning, the harmful effects of viruses and other medications. For example, antitumor drugs (they are very toxic and harmful to the liver), antibiotics, antituberculosis and antiviral drugs, as well as some antidepressants and oral contraceptives.
Chronic cholecystitis without the formation of stones.
Inflammation of the bile ducts - cholangitis.
Cirrhosis of the liver (more precisely, either a condition preceding cirrhosis, or cirrhosis itself 1 degree of severity).
During pregnancy is used for congestion of bile in the ducts (intrahepatic cholestasis).
    Encephalopathy, caused by hepatic insufficiency.
Conditions that arise when a drug is withdrawn, alcohol or drugs are taken (abstinence syndrome).
    Prolonged depressive manifestations.
Damage to hepatocytes - stable liver cells.
Decreased or increased aggressiveness of bile acids.
Poisoning of the liver with toxic elements and poisons.

Contraindications for Heptral for injection

In the instructions for the use of heptral injection, such contraindications are described:
    Two initial trimester of pregnancy.
    Breastfeeding period of the baby.
Children and adolescence under 18 years.
Individual intolerance of individual components of the drug.
    Genetic disorders.
The exchange of vitamin B-12 failed.
Allergic reactions to ademetionin or other components of the drug.
Lack of cystionin-beta-cystase in the body.

Use with caution in the following cases:

    Bipolar affective disorders.
Old age (more than 65 years).
    Manic-depressive syndrome.
    Renal failure.

Healing properties

Neuroprotective action is aimed at protecting the nerve cells from all kinds of negative interventions. In this case, when using Heptral, the risk of encephalopathy of the liver is significantly reduced.
Cholekinetic - that is, choleretic effect. The use of this drug contributes to the normal excretion of bile.
Regenerating - restoring. Heptral promotes an increase in the division of liver cells (hepatocytes). This action is responsible for the acceleration of recovery processes in the liver.
The antioxidant effect is the stimulation of the production of amino acids, in the amount necessary for the regenerative processes in the liver.
Antidepressant - the life force of neurons rises, the necessary translation of nerve impulses is restored.

Heptral Injections

The medicinal form of Heptral in ampoules is made especially for intravenous or intramuscular injections. As mentioned earlier, the package contains leophilizate itself and a solvent to it. There are no differences in the preparation of the solution for intramuscular and intravenous use - all actions are absolutely identical.
The recommended daily dosage of Heptral is 400 ml or 800 ml of the preparation, which corresponds to one or two bottles of powder.
The course of treatment with injection (jet) is 2 weeks. If there is a need for continued treatment, the drug is prescribed already in tablets. They, in turn, can not drink for more than a month after injections.
Important! The most effective therapeutic effect is observed with intravenous infusion, it is believed that this minimizes the possibility of side effects.
Injection solution can not be prepared in advance - this is done immediately before the procedure itself. You should pay attention to the prepared solution, because if it stays ready, but it is not used, it will have to be thrown out.

Recommendations for preparation and administration of the drug:

To open the bottle with leofilizate, you need to remove the top metal cover. The ampoule with the solvent is opened by cutting the apex of the ampoule. Using the syringe, the necessary amount of solvent is taken. The rubber stopper of the vial with the powder is then punctured with a syringe needle, and the dissolution liquid is poured into the vial.
All components are thoroughly mixed - until a homogeneous mixture is obtained. It has a white color, a slight yellow tint is permissible. Do not allow undissolved powder lumps. Shaking should be done without removing the needle from the rubber stopper.
A solution with incomprehensible impregnations or undissolved powder is considered not suitable for injections.
After the dissolution is successfully completed, you need to take the entire mixture into the syringe.
Place for injection should be rubbed with alcohol. You need to stab in the upper part of the buttock or in the shoulder (upper part from the outside). Intravenous infusion of heptral is produced in the veins on the hands, but such injections are done only by the doctor.
After removing the needle, the site of the injection is again disinfected.
In order that after the introduction of Heptral intramuscularly there were no bruises or abscesses, the injection is very slow. When carrying out the next injection, you need to move away from the place of the previous one by 1 cm and so on.
Excess preparations, and dry, and solvent, and finished - everything must be destroyed.
At intravenous introduction 100% bioavailability of a preparation is observed. This means that all 100% of the administered medicinal substance will fall into the necessary place in the body and be absorbed. The maximum level of drug concentration in the blood is already observed on the 2-6th day from the start of treatment. In the liver, heptral undergoes a biological transformation and is excreted from the body by the kidneys during the next one and a half hours.
During the treatment with the drug, it is necessary to monitor the activity of the kidneys.
If therapy with droppers is carried out for a long time, then the blood can change the level of creatinine, urea and nitrogen - both in the smaller and larger.
To correctly calculate the dosage of the medicine, the doctor necessarily takes into account the individual characteristics of the patient: age characteristics, the presence of chronic diseases, metabolic processes. Self-administration of the drug is strictly prohibited.
In the treatment of pathological processes in the liver, appropriate dietary intake is necessarily prescribed.
The use of alcoholic beverages during treatment is strictly prohibited. This applies even to tinctures of medicines, which contain alcohol.
If there is a need for intensive care, then it is carried out for 2-3 weeks using intravenous or intramuscular injections.
In the treatment of patients with an abstinence syndrome that is related to the abandonment of alcoholic beverages, Heptral injections are part of a comprehensive therapy and protect the liver cells, helping to neutralize toxic effects. Such therapy significantly improves the patient's condition. With a severe withdrawal symptoms, intravenous fluids are prescribed, intramuscularly much less often.
Since even elderly patients tolerate Heptral treatment well, they will not need to reduce the dosage. But at the same time, start therapy better with minimal dosages and gradually increase them.
In the course of treatment, the patient's anxiety increases, however, this condition quickly passes with a change in the dose of the drug in a smaller direction.
Cancellation of the drug is not required.
It is not recommended to inject with Heptral at bedtime, this is due to the fact that the drug is able to cause sleep disorders, as it has a tonic effect.
When treating cirrhosis, you need to periodically take a biochemical blood test to detect the levels of nitrogen, urea and creatinine.
When treating the drug, patients are strongly recommended to drink B vitamins (in particular, B-12) and folic acid. This must be done for the reason that if they are deficient in the body, Heptral will be absorbed worse.
Because of the treatment with this drug, the patient feels dizzy, so you need to be extremely careful and careful when carrying out work associated with increased concentration of attention, and if possible, and at all for a while refuse to perform them.

Overdose and Interaction

No cases of overdose have been observed with intravenous or intramuscular injections of Heptral.
In the course of clinical trials, no interactions with other medication have been identified. However, since the active ingredient of Heptral is ademethionine, you need to be extremely careful with tricyclic antidepressants, selective serotonin reuptake inhibitors (SSRIs), and some herbs that contain tryptophan.

Side effects

Despite the high efficiency of parenteral injections for intravenous administration, the drug has a number of side effects. And, both minor and serious enough. Most often they appear in the form of abdominal pain, nausea and diarrhea.
Regardless of the form of the drug, the following deviations in the functioning of systems and organs are sometimes observed:
Musculoskeletal system - spasms in the muscles and joint pain.
Digestive system - flatulence, diarrhea, abdominal pain, dry mouth. Less commonly - bleeding in the digestive organs, difficulty or painful digestion.
Cardiovascular system - phlebitis of superficial veins, changes in the work of blood vessels and heart.
Nervous system - headaches and dizziness, sleep disorders, blurred vision, excessive anxiety.
The emergence of diseases of an infectious nature, especially often it is a urinary tract infection.
Skin - at the site of intravenous administration, some reactions are possible: urticaria, erythema, pruritus. It is unlikely necrosis of the skin in this place, but it is not excluded. Increased sweating occurs much more often.
The respiratory system - swelling of the larynx carries an immediate threat to the life of the patient.
The immune system is anaphylactic shock. This is a very dangerous condition, characterized by pressure spikes, pain behind the sternum and back, shortness of breath, narrowing of the bronchi due to muscle contraction (bronchospasm).
Other complications are fever, chills, asthenic syndrome.

Interaction with alcohol

Treatment with the drug Heptral for injection occurs with complete rejection of alcohol-containing and low-alcohol drinks. A diet prescribed before the start of therapeutic procedures, implies a full lock on alcohol. Otherwise, there may be various unpleasant consequences for the body.
The use of alcohol significantly reduces the benefits of the treatment received, and sometimes the treatment can go completely unnoticed.
Especially important is the refusal of alcohol, when therapy is prescribed for the treatment of diseases caused by alcohol dependence.
Drinking alcohol when receiving Heptral injections leads to a violation of the heart rate and increased blood pressure.
Joint use of alcohol and Heptral negatively affects the activity of the nervous system: there may be clouding of consciousness, signs of depression, sleep disorders.
Quincke's edema is indicated among the side effects of the drug, however, most often, it develops when both ethanol and heptral enter the bloodstream. In general, alcohol greatly increases the possibility of any side effects. Especially often there can be complications such as phlebitis, bleeding in the internal organs, kidney failure and others.

Reviews of doctors about the drug Heptral

Due to the fact that Heptral is one of the most promising drugs of the heparoprotective group among many proposed medications of the same action, the doctors' reviews are, for the most part, positive. According to their common opinion, this medication has an undoubted effect and its positive effect on the liver is obvious. This is especially noticeable in the treatment of conditions that subsequently cause the development of cirrhosis and fibrosis. Practitioners say that Heptral is indispensable in maintaining normal liver function and preventive measures aimed at preventing cirrhosis of the liver.
However, there are doctors who believe that Heptral has too powerful an effect, which is not always acceptable for certain liver diseases.
Here, opinions diverge. Another part of specialists believes that, on the contrary, it is very good, because it is the strong effect of the drug that causes one hundred percent recovery of patients.
However, all the doctors agree that Heptral is highly effective and can be applied and needed, but with mandatory control of blood tests.
If the patient does not have severe liver damage, then it is possible to replace Heptral with a lighter analogue.

Patient Reviews of Heptral

Patients who had to be treated with the drug left good reviews and even noted that it was Heptral - the only effective drug from the group of similar. Patients with cirrhosis believe that the use of courses (twice a year for 3 months) treatment with this drug helped them achieve excellent results.
The same is true for bile inflammation associated with stagnation of bile - patients are very happy, but it still depends on the duration of treatment and the number of courses taken.
